Commentary states: The disease of "Zhong Men Jue" (sudden collapse due to heat stroke), which causes sudden fainting and loss of consciousness, is caused by heat-toxin damaging the Zang organs. The Heart governs the Shen (Spirit) and dislikes heat. If one is exposed to summer-heat toxin, which accumulates within the Five Zang Organs, it prevents the ascending and descending of Yin and Yang Qi, and obstructs the unimpeded flow through the Meridians. Consequently, there is sudden collapse and unconsciousness. However, as long as the True Zang Qi has not been destroyed, there are methods for rescue and treatment; once Qi circulation is restored, the patient will regain consciousness. One must not wait for spontaneous recovery, nor should cold be introduced. If external cold is encountered, it will conflict with internal heat, causing Qi obstruction to worsen, leading to death.
Formula for treating severe heat-induced collapse, restlessness, and confusion:
Sheng Di Huang (Fresh Rehmanniae Radix, washed, pounded, and juice extracted, one cup).
Take the above single herb. Administer slowly by灌 (pouring into the mouth).
Formula for treating summer-heat toxin heat and unconsciousness:
Bai Mian (White Flour), any quantity.
Take the above single herb. Mix with hot water to form a paste, take two Qian Bi (approx. 6g) and administer.
Formula for treating Zhong Re (Heat Stroke) and unconsciousness:
Da Suan (Garlic Bulb), any quantity, peeled and pounded.
Take the above single herb. Mix with fresh well-water and administer.
Formula for treating Zhong Re (Heat Stroke) and unconsciousness:
Apply mud around the person's umbilicus. Have three or four persons urinate into it. Alternatively, use a curved leather belt, the bottom of a lacquer bowl, or a removed cart wheel placed over the umbilicus to collect urine without letting it flow away. This method applies when no hot water is available on the road, requiring urine to be warmed by body heat. If hot water is available, human urine is unnecessary. Zhang Zhongjing mentioned using mud and carts because these objects are naturally cold. Humans fear cold; therefore, if mud is mixed with hot water, or the cart wheel is heated by fire, they can be used.
Another formula:
Take Sheng Chang Pu (Fresh Acori Tatarinowii Rhizome), any quantity, pound and squeeze to extract juice. Warm slightly and administer one cup.
Another formula:
First dig a pit in the ground. Pour half a cup of honey into the pit. Add two cups of fresh well-water, stir evenly, let settle, and extract one cup of the clear liquid for administration.
Another formula:
Use an object to pry open the mouth. Slowly administer warm water. Slightly elevate the head so that upon swallowing, the patient regains consciousness.
Another formula:
Take Sheng Liao (Fresh Polygoni Orientalis Herba), any quantity. Boil strongly in water, squeeze out the juice, and administer one to two cups.
Formula for treating fatal Zhong Re (Heat Stroke):
Take hot dust from the road and pile it over the heart region. Replace when cool. Stop when Qi circulation resumes.
Formula for treating severe heat-induced collapse, restlessness, and confusion:
Blow air onto the patient's heart region. Then pound Sheng Di Huang (Fresh Rehmanniae Radix) to extract one cup of juice and administer. Moxibustion should be applied to both nipples, seven moxa cones each.
Formula for treating critical Zhong Re (Heat Stroke). Chen Ju Pi Tang (Aged Tangerine Peel Decoction):
Chen Ju Pi (Aged Citri Reticulatae Pericarpium, soaked in water, white pith removed, dried) Gan Jiang (Dried Zingiberis Rhizoma, processed) Gan Cao (Licorice Root, honey-fried).
Take the above three herbs in equal parts. Coarsely crush and sieve. Each dose is three Qian Bi (approx. 9g). Add one cup of water, boil for five to seven boils, remove dregs. Allow the patient to swallow slowly, do not give all at once. Continue until the patient recovers.
Formula for treating critical Zhong Re (Heat Stroke). Bei Ji Jiu Sheng Wan (Emergency Rescue Pill):
Gan Jiang (Dried Zingiberis Rhizoma, processed) Gan Cao (Licorice Root, honey-fried) Huang Yao Zi (Dioscoreae Hypoglaucae Rhizoma) Ban Lan Gen (Isatidis Radix) Xi Jiao (Rhinocerotis Cornu, sliced).
The first four herbs are each four Liang (approx. 120g); Xi Jiao is one Liang (approx. 30g). Grind all into fine powder. Refine honey to make pills the size of a bullet. Each dose is one pill, dissolved in hot water and taken. Effect is immediate.
Formula for treating Zhong Re (Heat Stroke) causing mental cloudiness and oppression. Huang Lian San (Coptis Powder):
Huang Lian (Coptidis Rhizoma, chicken-claw shape preferred, any quantity, remove roots).
Grind the above single herb into fine powder. Each dose is two Qian Bi (approx. 6g). Take with thick decoction of Deng Xin Cao (Junci Medullae) prepared tea. Recovery occurs upon urination. This is because the Heart dislikes heat, bitter flavor enters the Heart, and heat transmitting to the Small Intestine allows Qi to descend. Thus, urination indicates recovery, as Deng Xin Cao promotes diuresis.
Formula for treating summer-heat toxin, restlessness, and lethargy. Ning Shui Shi Wan (Halloysitum Pill):
Ning Shui Shi (Halloysitum, calcined in a bottle until powdery) Gan Jiang (Dried Zingiberis Rhizoma, burned to ash, each one Liang) Gan Cao (Licorice Root, honey-fried, three Qian) Tian Xiao (Sal Nitri, half Liang).
Grind the above four herbs into powder. Refine honey to make pills the size of a bullet. Each dose is one pill, dissolved in Ginger Decoction.
Formula for treating summer-heat toxin, restlessness, and unconsciousness. Wan Jin San (Ten Thousand Gold Powder):
Liu Huang (Sulfur, ground) Ge Fen (Margarita Pulvis, each one Liang).
Grind the above two substances into fine powder. Each dose is one Qian Bi (approx. 3g). Take with fresh well-water.
Formula for treating Zhong Re (Heat Stroke) with coma, vomiting, headache, and cold sweat. Yu Zhu San (Jade Pearl Powder):
Liu Huang (Sulfur, imported) Hua Shi (Talcum, white color) Ning Shui Shi (Halloysitum, calcined, each one Liang).
Grind the above three herbs into powder. Each dose is two Qian Bi (approx. 6g). Take with Ai Ye Decoction (Artemisia Argyi Leaf Decoction).
