Commentary states: Human muscles and skin rely on Qi and Blood for warming and nourishment. When a carbuncle (Yong) persists without healing, it is because the heat-toxin has not been fully cleared. Wind-cold pathogens take advantage of this weakness to invade and lodge in the wound aperture. Consequently, the muscles fail to receive warmth, leading to swelling and induration that does not subside. The condition fluctuates, appearing to heal and then recurring, known as "Chronic Carbuncle" (Jiu Yong). If left untreated, it may transform into "E Rou" (fleshy growths or necrotic tissue/fistulas).
