Commentary: In all cases of postpartum women, after the placenta has been expelled, regardless of whether the infant is male or female, or if there was a desire for a boy but a girl was born, or a desire for a girl but a boy was born, knowing these facts may cause displeasure. This emotional disturbance can lead to other diseases. If the amount of lochia (postpartum uterine discharge) discharged is small, administer Huaxue Tang (Slippery Blood Decoction); if the amount discharged is large, administer Heqi Tangji (Harmonizing Qi Decoction). Alcohol should not be consumed, as it may guide the lochia to scatter into the four limbs, leading to fainting and dizziness. One should not sleep immediately; instead, wait until the qi and blood are harmonized and stable. After a short while, assume a kneeling supine position with the head elevated on pillows, use thick bedding, and tightly seal the walls to prevent drafts. Frequently prepare a vessel of strong vinegar and quench it with charcoal fire inside the bed curtain to suppress Xueyun (blood stasis-induced vertigo/fainting). Additionally, have someone use warm hands to push downwards from the heart region to below the umbilicus to expel retained blood. For three days, consume only white rice porridge, occasionally taking Huaxue and Heqi formulas. After three days, occasionally drink a small amount of strong wine and eat soft rice. After about ten days, gradually introduce flavorful foods, such as eggs boiled in vinegar, as vinegar can break up blood stasis. However, prolonged consumption of rich food may diminish one's complexion. Within one month, avoid leaving the house, walking excessively, or engaging in laborious needlework. Within one hundred days, strictly abstain from sexual intercourse and adhere to dietary taboos. Also, guard against emotions such as anger, joy, worry, resentment, sorrow, and grief, lest they cause illness. For one month postpartum, one must always cover oneself with clothes and blankets when lying down; even in summer months, the body must not be exposed, especially avoiding wind, cold, yin-dampness. Bathing should only occur after three months; at minimum, after sixty days. Yang Shi stated: "Yingyue" (full month) refers to three full months, not thirty days.
Commentary: Postpartum lochia discharge, whether excessive or insufficient, can lead to fainting and dizziness because blood follows qi. Excessive bleeding leads to qi deficiency, while insufficient bleeding leads to qi rebellion. Symptoms include spinning vision, mental confusion, and in severe cases, silence and unconsciousness. Treatment principles: For deficiency, regulate qi and tonify blood; for qi rebellion, regulate qi and descend blood. This approach covers more than half the treatment logic.
To treat postpartum scanty lochia with qi rebellion, causing dizziness, blurred vision, chest oppression, and heavy head unable to lift: Danggui Tang (Angelica Decoction).
Danggui (Radix Angelicae Sinensis), Chuanxiong (Rhizoma Chuanxiong), Taoren (Semen Persicae), Dahuang (Radix et Rhizoma Rhei), Guizhi (Ramulus Cinnamomi), Shaoyao (Radix Paeoniae Rubra/Alba).
Coarsely crush and sieve the above seven herbs, dividing into three doses. For each dose, decoct with five cups of water, five slices of Shengjiang (Rhizoma Zingiberis Recens), and five fractured Zao (Fructus Jujubae) until three cups remain. Strain and take one cup warm.
To treat postpartum excessive lochia with qi deficiency, causing dizziness, lethargy, and unconsciousness, regulating qi and tonifying blood: Chenjupi Tang (Aged Tangerina Peel Decoction).
