Discourse states: Dry retching (gan ou, dry heaving without vomiting) in cold damage (Shanghan) refers to the sensation of wanting to vomit but with nothing coming out. This is often caused by pathogenic heat remaining in the stomach, or after sweating to resolve the exterior, the stomach qi becomes disharmonious and retains accumulated heat. The heat steams upward to the upper jiao, causing palpitation and knotting below the heart (epigastric region), thus leading to dry retching. Furthermore, various pathogenic factors invading the stomach cause stomach qi to rebel upward, all resulting in vomiting. If it is due to cold, it leads to the stagnation of cold phlegm, vomiting clear fluid.
