Discussion: When the Kidney essence is deficient, Qi transformation fails to transmit and convert fluids; heat accumulates in the Bladder, obstructing the water passages. Thus, urinary retention occurs, causing fullness and distension in the lower abdomen, with Qi rushing upward, leading to epigastric and abdominal bloating and痞 (stagnation). In severe cases, it results in panting and urgency, disrupting the ascending and descending of Qi. The *Neijing* states: "When the Bladder is not smooth, it is called 'long' (urinary retention)," referring to this condition.
For treating urinary retention due to accumulated heat in the Bladder, use Tongguan Qumai Decoction:
Qumai Suis (Herba Dianthi), Shaoyao (Radix Paeoniae Rubra), Dahuang (Radix et Rhizoma Rhei, crushed and stir-fried), Danggui (Radix Angelicae Sinensis, sliced and baked), Kuizi (Semen Malvae), Gancao (Radix Glycyrrhizae Preparata, honey-fried), Yubai Pi (Cortex Ulmi), Zhizi Ren (Semen Gossypii Herbacei), Mutong (Caulis Akebiae).
The above eleven ingredients are coarsely crushed and sifted. Take five qianbi per dose, add one and a half zhan of water and a few stems of Dengxincao (Medulla Junci), decoct until one zhan remains, strain, and take warm.
For treating urinary retention due to heat knot in the Bladder, use Liying San:
Jingquanshi (Lapis Aquae Fontis), Cheqianzi (Semen Plantaginis), Huashi (Talcum, each banliang), Tingli (Semen Lepidii et Descurainiae, fried on paper for one qian), Haijinsha (Concretio Bileus Gallinae, one qian).
The above five ingredients are crushed, pulverized, and sieved into a powder. Take one qianbi per dose, mix with fresh cold water before meals. If urination does not resume, take another dose.
For treating urinary retention with Qi rushing upward and chest/epigastric fullness, use Yuri Wan:
Zhusha (Cinnabaris, ground), Xionghuang (Realgar, ground), Huashi (Talcum, ground), Qumai Suis (Herba Dianthi), Haijinsha (Concretio Bileus Gallinae, each one fen).
The above five ingredients are crushed and ground into powder. Separately grind Bimazi Ren (Semen Ricini) and form pills the size of Sophora seeds. Take ten to fifteen pills per dose, administered with wine decocted with Congbai (Bulbus Allii Cepae). Reduce dosage for children.
For treating urinary retention, use Kuizi Yin:
Mutong (Caulis Akebiae, crushed), Dongkuizi (Semen Malvae), Gansui (Radix Kansui), Qumai Suis (Herba Dianthi). (These four combined make banliang, finely crushed and slightly stir-fried), Huashi (Talcum, ground, two qian).
The above five ingredients are coarsely crushed and sifted. Take two qianbi per dose, add one zhan of water and Dengxincao (Medulla Junci), decoct together until seven parts remain, take with dregs warm. If not resolved, take again.
For treating urinary retention, use Xiangzi San:
Xiangzi (Fructus Foeniculi, fried), Malinhua (Flos Iris Tectorii, fried), Tingli (Semen Lepidii et Descurainiae, fried on paper, equal parts).
The above three ingredients are crushed, pulverized, and sieved into a powder. Take two qianbi per dose, mix with warm wine, take before meals. Stop when urination resumes.

The above two ingredients: first grind Suisui Zi finely, then add Qiandan and grind evenly. Mix with a small amount of honey to form balls, store in a porcelain jar, seal tightly, bury in a pit in a shady place, pile snow and ice on top (the more, the better). Prepare in the twelfth lunar month, retrieve at the end of spring. Grind evenly again, refine honey separately, and form pills the size of Sophora seeds. Take fifteen to twenty pills per dose, administer with Mutong Tang decoction, at any time. For severe cases, no more than two doses are needed. For rapid effect, dissolve and take directly.
