Commentary states: The disease of Hou Bi (throat obstruction) is characterized by swelling and blockage in the throat, bi pain (obstructive pain), inability to swallow water or fluids, obstructed breathing, and alternating chills and fever. This results from wind-heat pathogens invading the Spleen and Lungs, steaming and scorching the pharynx and larynx. Children possess a "pure yang" constitution, making them particularly susceptible to this ailment. If not treated promptly, toxic pathogens may enter the Heart, causing restlessness, stifling sensation, and mental agitation, leading immediately to critical danger.
To treat pediatric Hou Bi, which arises from accumulated heat in the Spleen and Lungs causing Qi and Blood stagnation and blockage, do not puncture with needles; instead, treat with the following formula. Mutong Tang (Wood Vessel Decoction):
Mutong (Caulis Akebiae, crushed, 1 liang), Shengma (Rhizoma Cimicifugae, 1 fen), Dahuang (Radix et Rhizoma Rhei, crushed and stir-fried, 1 fen), Mahuang (Herba Ephedrae, roots and joints removed, 1 fen), Xijiao (Cornu Rhinocerotis, shaved into flakes, 1 fen), Shigao (Gypsum Fibrosum, crushed, 0.5 liang), Gancao (Radix Glycyrrhizae, honey-fried, 1 fen).
The above seven ingredients are coarsely pounded and sieved. For each dose, take 2 qianbi. Add 1 zhan of water, decoct until 0.7 zhan remains, strain the dregs, add 1 qianbi of Puxiao (Natrii Sulfas), and mix well. Boil for one or two more boils, divide into two warm doses. Take one dose after morning meal and one before bed. For children over five years old, adjust dosage according to circumstances.
Beiji Sanwu Wan (Emergency Three-Ingredient Pills) for treating Hou Bi where fluids cannot be swallowed:
Dahuang (Radix et Rhizoma Rhei, crushed and stir-fried), Ganjiang (Rhizoma Zingiberis, processed), Badou (Semen Crotonis, skin and heart removed, separately pounded into paste).
The above three ingredients are used in equal proportions. First, pound the first two into fine powder, add Badou and grind again. Form pills using refined honey, sized like mung beans. Take 3 pills per dose with warm water, stopping when bowel movement is achieved.
