Discussion states: Pediatric wandering swelling (Xiao'er Youzhong) is caused by wind-heat pathogens invading the meridians, moving with the flow of Qi, and flowing into the skin and muscles, resulting in redness, swelling, and pain. The treatment method should involve internal administration of medicines to clear accumulated heat within the body, and external application of poultices to dissipate local pain and swelling, thereby causing the wandering swelling to subside naturally.
For treating pediatric wandering swelling due to wind-heat with red coloration, the Xijiao Yin (Rhinoceros Horn Drink) formula:
Xi Jiao (Horn of Rhinoceros, sliced) [Note: Modern practice substitutes with other herbs; WHO term often uses "Rhinoceros Horn" historically or substitutes like Shuiniujiao] Huang Qin (Scutellariae Radix, cleaned of black heart) Sheng Ma (Cimicifugae Rhizoma) Shan Zhi Zi Ren (Gardeniae Semen) Huang Qi (Astragali Radix, cut) Niu Huang (Calculus Bovis, ground) Fang Ji (Stephaniae Tetrandrae Radix) [Each 1 fen] Po Xiao (Natrii Sulfas) [3 fen]
The above eight ingredients, except for Niu Huang, are coarsely crushed and sifted. Each dose consists of two qian bi (approx. 6g), boiled in one zhan (approx. 200ml) of water until reduced to six parts. Strain the dregs, then add a quantity of Niu Huang equivalent to a large soybean. Divide into two doses, taken in the morning and evening. Adjust dosage according to the child's age and condition.
For treating pediatric wandering swelling, the Zhizhi Gao (Pig Fat Ointment) formula:
Zhu Zhi (Suet, refined, 4 he) Fu Zi (Aconiti Lateralis Radix Preparata, raw, peeled and navel removed) Shu Jiao (Zanthoxyli Pericarpium, raw, selected ones without closed eyes/sides, 1 fen each) Yan Shi (Halitum, ground, 3 fen)
The above four ingredients: crush and grind the three solid ingredients into powder, mix into the pig fat, and simmer. Allow to cool before applying topically. Continue until healed.
For treating pediatric toxic wandering swelling on the head and body with redness and pain, the Shengma Gao (Cimicifuga Ointment) formula:
Sheng Ma (Cimicifugae Rhizoma, 2 liang) Xi Jiao (Horn of Rhinoceros, sliced) She Gan (Belamcandae Rhizoma) Huang Qin (Scutellariae Radix, cleaned) Zhi Zi Ren (Gardeniae Semen) Xuan Shen (Scrophulariae Radix) [Each 1 liang 1 fen] Shao Yao (Paeoniae Radix) Da Huang (Rhei Radix et Rhizoma) Lan Shi (Isatidis Folium/Fructus contextually likely Isatis leaf/seed product) Ling Yang Jiao (Saigae Tataricae Cornu, sliced) [Each 1 liang] Da Qing (Baphicacanthis Cusii Folium, 0.5 liang) Sheng Gan Di Huang (Rehmanniae Radix, fresh/dried, 2 liang 3 fen)
The above twelve ingredients are finely cut. Add 2.5 jin of pig fat into a pan, simmer over low heat while stirring constantly until it forms an ointment. Strain and store in porcelain vessels. Apply to the swollen area until healed; amount is not restricted.
For treating pediatric red wandering swelling due to wind-heat, the Gualou San (Trichosanthes Powder) formula:
Gua Lou (Trichosanthis Fructus, cut)
The above single ingredient is crushed and sifted into a powder. Mix with strong vinegar and apply topically.
For treating pediatric red wandering swelling that flows across the body. If not treated urgently, invasion into the abdomen leads to death. Use the Baidou San (White Bean Powder) topical formula:
Take powder from white beans, mix with water to form a paste for application. Do not let it dry out. Continue until healed.
For treating pediatric red wandering swelling that gradually spreads continuously, use the topical application formula:
Niu Xi (Achyranthis Bidentatae Radix, cut) Gan Cao (Glycyrrhizae Radix et Rhizoma, cut) [Each 2 liang]
The above two ingredients are crushed and sifted into a powder. For each use, take two qian bi. Boil in one zhan of water until reduced to six parts. Strain the dregs, mix with yellow earth from the stove bottom, and apply topically. Change when dry.
For treating pediatric wind-heat red wandering swelling, another formula:
Use wheat flour powder mixed with water for topical application.
Another formula:
Take Fu Long Gan (Dragon's House Earth/Hearth Earth), mix with egg white for topical application. Change when dry.
