Commentary states: Pediatric gan disease (a wasting disorder in children) arises from excessive consumption of rich and sweet foods, hence the name "various gans." The involvement of the five zang organs varies. When affecting the Liver, it is termed Wind Gan (Liver Gan), characterized by head shaking and eye rubbing, a white membrane obscuring the pupils, greenish-yellow complexion, dry and erect hair, visible tendons, heat in the brain, a preference for lying on one's side, abdominal masses, frequent diarrhea, spontaneous sweating, and progressive emaciation if untreated. This is Liver Gan. If the eyeballs appear blue, the left hypochondrium is hard, there is vomiting of saliva, and black qi at the corners of the eyes, it is incurable. When affecting the Heart, it is termed Startled Gan (Heart Gan), characterized by high fever throughout the body, irregular vomiting and diarrhea, red cheeks with a sallow face, chest and diaphragm fullness and irritability, dry nose, mental restlessness, sores in the mouth and tongue, occasional night sweats, or false fright. If untreated, it progresses to dysentery with pus and blood, which is Heart Gan. If accompanied by frequent startled crying, excessive thirst, preference for pungent flavors, visible pulses behind the ears, and black spots on the tongue, it is incurable. When affecting the Spleen, it is termed Food Gan (Spleen Gan), characterized by visible abdominal veins, rapid and coarse crying, poor milk/food intake, chest and abdominal distension, frequent crying and coughing with rebellious qi, sallow complexion, skeletal thinness with dry hair, withered form and weak strength, chest and diaphragm congestion and stuffiness, indigestion, disharmony of the intestines and stomach, sour-smelling diarrhea, dry nose and mouth, preference for darkness over light, poor mood, and pica (eating dirt). This is Spleen Gan. If accompanied by a distended abdomen, colorless lips, a flat philtrum, incontinence of diarrhea, dry skin, and exposed bones, it is incurable. When affecting the Lung, it is termed Qi Gan (Lung Gan), characterized by coughing and rebellious qi, dry and scorched skin and hair, profuse nasal discharge with frequent crying, throat discomfort, nose rubbing and nail biting, high fever with aversion to cold, sores in the mouth and nose, red and itchy lips, abdominal bloating, reduced appetite with diarrhea, and millet-like rashes on the skin. This is Lung Gan. If accompanied by coughing with shortness of breath, white frothy diarrhea, and black rice-grain-sized spots on the body, it is incurable. When affecting the Kidney, it is termed Urgent Gan (Kidney Gan), characterized by muscle and bone emaciation, sores on the gums, dry nose and mouth upon exposure to cold or heat, burning heat in the brain, ice-cold feet, poor intake with vomiting and rebellious qi, occasional diarrhea, sores in the lower body, and rectal prolapse. This is Kidney Gan. If accompanied by craving for sour and salty flavors, uncontrolled milk drinking, turbid white urine, blackish teeth, dry ears and brain, shoulder shrugging, and skeletal withering, it is incurable. These five gans have five signs of terminal prognosis: 1. No pain when pinching the sole of the middle toe; 2. Limp limbs when held; 3. Body remains cold despite illness; 4. Continuous passage of green saliva and foam from the viscera; 5. Stiff neck and tendons with weakness. All such cases are incurable. Beyond these five gans, variations occur according to the twelve meridians and qi-blood pathogenesis, hence the term "various gans."
