Discourse states: All categories of heart pain syndromes are caused by pathogenic factors lodging in the vessels of the Hand Jueyin Pericardium (Pericardium Meridian of Hand-Jueyin), which corresponds to the Heart. The Heart is the sovereign official of the Zang-Fu organs, the residence of the Shen (Spirit), and the convergence point of all Yang meridians. Its substance is solid and resistant, making it difficult for pathogens to directly injure it. Therefore, when various pathogens invade the Heart, they typically affect the Pericardium (the outer covering of the Heart), manifesting as disorders of the Pericardium vessels. The clinical manifestations vary: there are cases where cold pathogen suddenly lodges in the Zang-Fu, causing sudden severe pain; cases of Yang deficiency with Yin rebellion, where pain radiates to the throat accompanied by cold limbs; cases where heart pain and back pain radiate to each other, associated with spasms and stooping; cases where abdominal distension leads to severe pain centered in the heart region; cases of acute stabbing pain as if pierced by a needle or awl; cases where the complexion is pale and the patient cannot take deep sighs throughout the day; cases where lying down causes pain from the epigastrium that worsens with movement; and cases where masses appear and disappear, moving up and down, with intermittent pain. These conditions may arise from dietary irregularities or external wind invasion. When the internal Zang-Fu are deficient, pathogens enter and cause Qi stagnation that does not dissipate, leading to obstruction where flow should be free, resulting in pain. True "Zhen Xin Tong" (True Heart Pain) usually does not present as ordinary pain but rather as a struggle between excess pathogens, characterized by cold hands and feet; this condition is beyond the reach of standard treatments and must be carefully distinguished.
Discourse states: The Heart is the sovereign official and the mansion of the Shen (Spirit). The regular meridians of the Heart do not accept pathogens, but its branch collateral vessels can be invaded by wind-cold pathogens, causing heart pain. This occurs because cold pathogenic factors become stagnant and do not dissipate, invading the meridians internally, leading to heart pain characterized by alternating mildness and severity of symptoms.
