Commentary: The Sanjiao (Triple Burner) is the pathway for water and grain, and the beginning and end of Qi circulation. If affected by wind-cold or dietary injury to the stomach, causing disharmony in the Middle Jiao, the righteous Qi cannot be maintained, allowing pathogenic factors to interfere. In a moment, this leads to chaos, hence it is named Huoluan (Cholera-like syndrome). Generally, when clear and turbid Qi interfere with each other between the intestines and stomach, heart pain causes vomiting, abdominal pain causes diarrhea, and in severe cases, both vomiting and diarrhea occur simultaneously. Those who do not vomit nor have diarrhea are commonly referred to as Gan Huoluan (Dry Cholera), which is also caused by cold Qi struggling in the middle and unable to exit. However, if the pulse shows a Dai pulse (intermittent pulse), it indicates insufficient Qi; if the pulse is large and the patient can speak, it is treatable; if the pulse is faint, Qi is weak, and the patient does not wish to speak, it is difficult to treat.
Commentary: Huoluan with vomiting and diarrhea results from the failure of Sanjiao Qi to ascend and descend, causing interference between clear and turbid Qi in the intestines and stomach. If heart pain appears first, vomiting occurs; if abdominal pain appears first, diarrhea occurs; if both heart and abdomen hurt, vomiting and diarrhea occur together. To treat this, one should focus on the Middle Jiao as the root. Once the Middle Jiao is harmonized, clear and turbid Qi will separate naturally, and vomiting and diarrhea will cease.
