Discussion: Wind-induced pruritic urticaria (Feng Sao Yin Zhen). Its clinical manifestations present in two forms. Both arise from heat pathogenic factors accumulating within the muscle layer. If this heat is constrained by cold-dampness, and the heat becomes knotted and fails to dissipate, white wheals (Bai Zhen) form. If the condition is caused by wind-pathogen invasion, where wind and heat contend with each other, red wheals (Chi Zhen) form. Red wheals worsen upon exposure to heat and subside upon exposure to cold. This is because the heat qi is郁结 (stagnant/knotted) internally, thus it dislikes heat and favors cold treatment. White wheals aggravate during cloudy or rainy weather and dissipate during sunny or warm weather. This is because the heat qi scatters to the exterior, thus it dislikes cold and favors heat treatment. Although the presentations of cold and heat syndromes differ, they both belong to the category of urticaria. The term "wind-induced pruritus" refers to the body experiencing itching, where scratching causes subtle elevations to appear.
