(Two treatises, twenty-nine prescriptions, fifteen acupuncture and moxibustion methods)
The treatise states: Bing Qiu Yun pointed out that hematemesis (vomiting blood) has three types: internal nasal bleeding (neinü), lung abscess (feiju), and stomach injury (shangwei). Internal nasal bleeding refers to a condition where bleeding resembles epistaxis (nosebleed), but the blood does not flow from the nostrils; instead, it is expelled during or after vomiting when the patient feels chest and abdominal fullness and oppression, with a volume of approximately one he, half sheng, or one sheng. Stomach injury occurs after overeating, when cold in the stomach prevents digestion, causing clear fluid to naturally flow from the mouth. If this happens to a young person who does not feel mental restlessness, it usually resolves spontaneously. However, if the patient experiences irritability, mental confusion, frequent vomiting, and physical agitation, and the physician administers formulas such as Huangtu Tang (Yellow Earth Decoction) or Ejiao San (Ejiao Powder), the patient's distress may worsen, leading to a critical condition that becomes untreatable. In cases of severe distress caused by such mismanagement, immediate emesis (induced vomiting) should be performed.
Emergency Emesis Prescription:
Gua Di (Thladiantra fruit), 3 fen; Du Heng (Donglingia root), 1 fen; Renshen (Ginseng root), 1 fen.
Grind the above three herbs into powder and sieve. Administer one qianbi dose. If no water or liquid is available, swallowing is sufficient. Reduce the dose for frail individuals. Vomiting up green-yellow fluids or one to two sheng of blood will not cause harm.
Prescription for Treating Hematemesis:
Fulonggan (Calcined earth from the bottom of a cooking clay pot), equivalent to the size of an egg, two pieces; Guixin (Cinnamon twig), 1 liang; Ganjiang (Dried ginger rhizome), 1 liang; Danggui (Angelica sinensis root), 1 liang; Shaoyao (Peony root), 1 liang; Baizhi (Angelica dahurica root), 1 liang; Gancao (Licorice root), 1 liang; Ejiao (Donkey-hide gelatin), 1 liang; Xiongqiong (Ligusticum chuanxiong rhizome), 1 liang; Shengdihuang (Rehmannia glutinosa root prepared fresh), 2 liang; Xixin (Asarum root), 0.5 liang; Wuzhuyu (Evodia fruit), 2 sheng.
Decoct the above twelve herbs (chopped) in seven sheng of wine and three sheng of water. Reduce to three and a half sheng, strain, add Ejiao, and boil until three sheng remain. Divide into three doses. This prescription also treats epistaxis.
Another Prescription for Sudden Hematemesis and Epistaxis:
Fulonggan (Calcined earth from the bottom of a cooking clay pot), 0.5 sheng; Gancao (Licorice root), 3 liang; Ganjiang (Dried ginger rhizome) [Note: Zhang Zhongjing's original formula uses Dihuagn]; Baizhu (Atractylodes macrocephala rhizome), 3 liang; Ejiao (Donkey-hide gelatin), 3 liang; Huangqin (Scutellaria baicalensis root), 3 liang.
Decoct the above six herbs (chopped) in ten dou of water. Reduce to three sheng, strain, add Ejiao, and divide into three doses.
Prescription for Treating Hematemesis due to Grief and Anger, accompanied by Restlessness, Fullness, Shortness of Breath, and Chest Pain:
Shengdihuang (Rehmannia glutinosa root prepared fresh), 1 jin; Dazao (Jujube fruit), fifty pieces; Ejiao (Donkey-hide gelatin), 3 liang; Gancao (Licorice root), 3 liang.
Decoct the above four herbs (chopped) in ten dou of water. Reduce to four sheng. Divide into four doses: three times during the day and once at night.
