Sit upright facing east, holding your hands to your heart. One person stands in front to press and stabilize both knees, while another person stands behind to support the head. Gently pull until lying supine, with the head touching the ground; repeat this three times.
For lower back pain, it is advisable to puncture the blue and red vessels located in the middle of the crease between the knee and the waist (referred to as "knee-waist hook") to let out blood, which will lead to recovery.
For lower back pain that prevents bending forward or backward, have the patient stand upright. Place a bamboo stick vertically against the ground and measure up to the navel. Break the bamboo at this point, then use this segment to measure along the spine. Moxibustion should be applied to the top end of the bamboo segment, with the number of moxa cones corresponding to the patient's age. After moxibustion, hide the bamboo so that others do not know what was done.
For lower back pain, apply ten moxa cones to the white flesh margin in the center of the horizontal crease on the heel; this is effective.
Additionally, apply seven moxa cones to Zu Juyang (Bladder 64, located below the outer ankle).
Additionally, apply seven moxa cones to Yao Mu (an extra point located approximately on either side above the sacrum).
Additionally, apply moxibustion to Ba Liao (the four pairs of apertures in the sacral foramina) and to the midpoint of the bone above the outer ankle.
For sudden acute lower back pain, apply seven moxa cones one cun above the coccyx (Qiong Gu), and seven moxa cones one cun to the left and right of this point respectively.
(One Treatise, Fifty-Nine Formulas)
The Treatise states: Tonifying formulas generally treat Five Exhaustions, Six Extremes, and Seven Injuries causing deficiency and damage. The Five Exhaustions refer to diseases of the five Zang organs; the Six Extremes refer to diseases of the six Fu organs; the Seven Injuries refer to illnesses affecting both the interior and exterior. The Five Exhaustions are: first, mental labor (Zhi Lao); second, overthinking (Si Lao); third, worry (You Lao); fourth, heart strain (Xin Lao); fifth, physical fatigue (Pi Lao). The Six Extremes are: first, Qi extreme (Qi Ji); second, Blood extreme (Xue Ji); third, Sinew extreme (Jin Ji); fourth, Bone extreme (Gu Ji); fifth, Marrow extreme (Sui Ji); sixth, Essence extreme (Jing Ji). The Seven Injuries are: first, Liver injury leading to frequent dreaming; second, Heart injury leading to forgetfulness; third, Spleen injury leading to excessive drinking; fourth, Lung injury leading to flaccidity syndromes; fifth, Kidney injury leading to excessive spitting; sixth, Bone injury leading to excessive hunger; seventh, Vessel injury leading to frequent coughing. Generally, distant thinking and forced deliberation harm the body; grief, indignation, and sorrow harm the body; excessive joy and pleasure harm the body; unresolved anger harms the body; anxious striving for desires harms the body; constant worry about afflictions harms the body; and irregular exposure to cold and heat harms the body. Thus they are called the Five Exhaustions, Six Extremes, and Seven Injuries. There are many discussions on injuries, but here only a brief overview is given; subsequent formulas address them all.
Formula for treating Five Exhaustions and Seven Injuries, with urgent pain in the lower abdomen, deficiency-fullness of the Bladder, cold extremities, bitter vomiting of sour phlegm after eating/drinking, retching, diarrhea with shortness of breath, dizziness, tinnitus, dry mouth, and free urination.
Maltose (8 liang), Huangqi (Astragali Radix) (3 liang each), Dried Ginger (Ganjiang) (3 liang each), Danggui (Angelicae Sinensis Radix) (3 liang each), Renshen (Ginseng Radix) (2 liang each), Banxia (Pinelliae Rhizoma) (2 liang each), Jupi (Citri Reticulatae Pericarpium) (2 liang each), Shaoyao (Paeoniae Radix Alba) (2 liang each), Gancao (Glycyrrhizae Radix et Rhizoma) (2 liang each), Fuzi (Aconiti Lateralis Radix Praeparata) (1 liang), Dazao (Ziziphi Jujubae Fructus) (15 pieces).
Chop the above eleven ingredients. Decoct in one dou of water to obtain three sheng and half. Remove the dregs, add Maltose, melt and boil, and divide into four doses. (Shenshi has Guixin (Cinnamomi Ramulus) 6 liang, Shengjiang (Zingiberis Rhizoma Recens) 1 jin. Without Jupi and Ganjiang.)
Another formula for treating deficiency damage with shortness of breath, abdominal distension with internal urgency pulling to the lower abdomen causing coldness, inability to flex and extend, inability to eat or drink, alternating chills and fever, headache, cold extremities, difficulty in urination and defecation, or recurrent dysentery, dry mouth, seminal emission during dreams, occasional retching with confusion, withered complexion, slight swelling, and soreness and acid pain in all joints.
Fuzi (Aconiti Lateralis Radix Praeparata) (1 liang each), Houpo (Magnoliae Officinalis Cortex) (1 liang each), Renshen (Ginseng Radix) (2 liang each), Gancao (Glycyrrhizae Radix et Rhizoma) (2 liang each), Guixin (Cinnamomi Ramulus) (2 liang each), Danggui (Angelicae Sinensis Radix) (2 liang each), Fuling (Poria) (2 liang each), Maidong (Ophiopogonis Radix) (3 liang each), Huangqi (Astragali Radix) (3 liang each), Longgu (Fossilia Ossis Mastodi) (3 liang each).
Chop the above fifteen ingredients. Decoct in one dou two sheng of water to obtain four sheng. Remove the dregs, add maltose. Take eight he per dose, three times a day and once at night. If there is cough, add double the amount of Shengjiang (Zingiberis Rhizoma Recens).
Another formula for treating Five Exhaustions and Seven Injuries, with urgency in the lower abdomen, distension below the umbilicus, fullness in both hypochondria,牵引 (pulling sensation) in the lower back and spine, dryness of nose and mouth, dim vision, irritability and unhappiness,急促 (rapid) breathing in the chest with inability to swallow food and drink, stabbing pain in the urethra, yellow-red urine, dribbling after urination, dreams of intercourse with spirits, loss of essence, fear, palpitations, deficiency, and exhaustion.
Yitang (Maltose) (8 liang), Huangqi (Astragali Radix) (3 liang each), Yuanzhi (Polygalae Radix) (3 liang each), Danggui (Angelicae Sinensis Radix) (3 liang each, absent in Yifang), Zexie (Alismatis Rhizoma) (3 liang each), Shaoyao (Paeoniae Radix Alba) (2 liang each), Gancao (Glycyrrhizae Radix et Rhizoma) (2 liang each), Renshen (Ginseng Radix) (2 liang each), Longgu (Fossilia Ossis Mastodi) (2 liang each).
Chop the above eleven ingredients. Decoct in one dou of water to obtain two sheng and half. Remove the dregs, add maltose and melt. Take eight he per dose, and consider taking another dose based on condition. (Shenshi lacks Yitang, Yuanzhi, Zexie, and Longgu, but has Guixin (Cinnamomi Ramulus) 6 liang, Banxia (Pinelliae Rhizoma) 1 sheng, and one Fuzi (Aconiti Lateralis Radix Praeparata)).
