The etiology of *Gu Zhu* (Gu Infestation) refers to the stagnation and immobility of pathogenic factors. The term "stagnation" implies that the disease progresses slowly, while "Zhu" also denotes the transmission of the disease to others. "Gu" refers to the accumulation of venomous creatures such as snakes and insects, placed in a vessel to devour each other until only one survivor remains, which is then called "Gu". This Gu can transform its form. Individuals who create Gu for malicious purposes often administer it through food and drink. Those infected experience chest oppression, abdominal pain, and if the five Zang organs are completely eroded, death ensues. The condition varies in acuity: acute cases develop rapidly, leading to death within ten days; chronic cases drag on over years, with the pathogen wandering within the abdomen. Patients suffer from general weakness, fatigue, and heavy joints. Upon onset, they experience mental restlessness, irritability, and pain. It is believed that ingested food transforms into Gu, gradually consuming the viscera until death. After death, the pathogenic qi flows and infects others, hence the name "Gu Zhu". (From Volume 24)
Fan Wang's treatment for various diseases caused by Gu Zhu, including *Jia Ji Ju* (abdominal masses), emaciation of flesh and bone, difficulty in urination and defecation, sudden exposure to evil wind, abdominal distension, dripping sensations resembling water flow, and the spread of illness throughout households affecting men, women, and grandchildren beyond medical cure, prescribes the Zi Shen Wan (Purple Ginseng Pill) formula consisting of seventeen ingredients.
Zi Shen (Codonopsis Radix or similar, literal: Purple Root), Ren Shen (Ginseng Radix), Ban Xia (Pinelliae Rhizoma, washed), Li Lu (Veratri Rhizoma), Dai Zhe Shi (Haematitum), Jie Geng (Platycodonis Radix), Bai Wei (Cynanchi Atrati Radix), Rou Cong Rong (Cistanche Caulis), each at three fen; Shi Gao (Gypsum Fibrosum) at one fen; Da Huang (Rhei Radix et Rhizoma) at one fen; Mu Li (Ostreae Concha, calcined) at one fen; Dan Shen (Salviae Miltiorrhizae Radix) at one fen; Xia Ma Hui (Bufo Skin Ash); Wu Tou (Aconiti Lateralis Radix Praeparata, processed) at four fen; Ye Lang Du (Stellerae Radix) at one fen.
Pulverize and sieve the above herbs, mix with honey to form pills. Take one pill the size of a small bean, dissolved in water, three times daily. Dosage for elderly and children should be adjusted accordingly. For stings from bees or scorpions, apply topically for excellent results. Avoid mutton and cold water. (Another version excludes Bufo Skin Ash and includes Gan Jiang [Zingiberis Rhizoma] at four fen; From Volume 14).
The Xiong Huang Wan (Realgar Pill) from the *Duan Ju* text treats Gu Zhu characterized by edema of the four limbs, wasting of skin and muscle, cough and counterflow qi, abdominal distension resembling water retention, leakage/diarrhea, and the transmission of disease to family members after death. This formula is also known as the Gu Zhang (Gu Distension) Formula.
Xiong Huang (Realgar, pulverized), Ba Dou (Crotonis Fructus), Mang Cao (Illicii Anisi Stellati Herba, roasted), Gui Jiu (Podophylli Peltati Rhizoma), each at four fen; Wu Gong (Scolopendra, three pieces, roasted).
Pulverize and sieve the five herbs into powder, mix with honey, and pound for three thousand strokes. Seal tightly in a container to prevent gas leakage. Fast overnight before administration. Take one pill the size of a small bean.
Scallion and Fermented Soybean Porridge
The Jiao Yu Pi San (Shark Skin Powder) from the *Ji Yan* text treats Gui Zhu and Gu Zhu, where toxic vapors change unpredictably.
Jiao Yu Pi (Skin of Shark/Seabream, specifically Que Yu Ban Pi), Xi Jiao (Rhino Horn, substituted typically with Buffalo Horn in modern practice but translated literally here), She Xiang (Moschus, pulverized), Long Gu (Fossilia Ossis Mastodi et Scapulae), Dan Sha (Cinnabaris, pulverized), Xiong Huang (Realgar, pulverized), He Ye (Nelumbinis Folium), Lu Jiao (Cervi Cornus Pantotrichum, roasted, each one fen), Wu Gong (Scolopendra, one piece, roasted), Jiao (Zanthoxyli Pericarpium, one fen), Gan Jiang (Zingiberis Rhizoma, one fen), Bei Zi (Concha Meretricis et Cyclinae, ten pieces), Ji She Xiang (Cardamomi Semen, one fen).
Pulverize and sieve the thirteen herbs into powder. Take one qian bi (a traditional measure) with wine on an empty stomach, three times daily. (From Volume 1).
