The condition characterized by severe redness, swelling, and pain of the gingival flesh is Ya Yong (Dental Abscess). Prick to release toxic blood, apply Zhenzhu San (Pearl Powder) by insufflation, and administer Longdan Xiegan Tang (Gentian Liver-Draining Decoction) internally for recovery. If there is pain in the dental bone and inside the cheek without redness or swelling, and the pain radiates to the facial bone, this is Gu Cao Feng (Bone-Cavity Wind). Treating it as an abscess would cause harm. Therefore, treating disease lies in accurately identifying symptoms. Ma said: Ya Yong (Dental Abscess) is caused by the mutual attack of Stomach Fire and Wind-Heat; it heals immediately upon pus discharge. If the swelling in the cheek does not subside and externally ulcerates, it becomes Chuan Sai Lou (Cheek-Penetrating Fistula).
The lesion occurs in the cheek, presenting as bilateral or unilateral. One type is called Zhe Sai (Cheek-Covering), and the other Fa Yi (Mumps/Parotitis); they should be treated differently. Soreness and pain inside the cheek indicate Zhe Sai (Cheek-Covering); apply Nen Gao (Tender Ointment) on top, and it will heal the next day. If both cheeks swell but without soreness or pain, this is Fa Yi (Mumps/Parotitis); it is appropriate to take formulas that disperse wind and detoxify. Use Baizhi (Angelicae Dahuricae Radix), Tianma (Gastrodiae Rhizoma), Fangfeng (Saposhnikoviae Radix), and Jingjie (Schizonepetae Spica), each 1 qian, decocted with aged wine to half a bowl, taken with Xingxiao Wan (Wake-Up Elimination Pill) for recovery.
Ma said: The main treatment for Zhe Sai (Cheek-Covering) is dispersing wind and clearing stomach heat. If Fa Yi (Mumps/Parotitis) occurs after an illness, starting below the ear root with swelling extending to the cheek and neck, it is due to the accumulation of Shaoyang pathogenic heat; one must attend to the underlying condition. Tianma (Gastrodiae Rhizoma), Baizhi (Angelicae Dahuricae Radix), Fangfeng (Saposhnikoviae Radix), and Xingxiao Wan (Wake-Up Elimination Pill) are not suitable. This condition arises during warm-heat diseases where sweating to resolve the muscle layer has failed; heat diseases easily damage Yin, so acrid-warm treatments must not be used.
Pain in the Shenzi (Testis) that does not ascend, with external redness, indicates Zi Yong (Testicular Abscess). Delay leads to complications, and ulceration can be fatal. For cases where pus has not yet formed, one dose of Gouju Tang (Citri Grandis Fructus Decoction) brings recovery.
Ma said: Zi Yong (Testicular Abscess) differs from Nang Yong (Scrotal Abscess). Zi Yong presents with hard, painful testicles without swelling of the testicle itself; Nang Yong presents with a swollen scrotum. Gouju (Citri Grandis Fructus) and similar herbs are the correct method for treating Zi Yong (Testicular Abscess).
When the scrotum develops toxicity, rots, and breaks open, causing the testicles to fall out, wash daily externally with Zisu Tang (Perilla Decoction). Take Zisu Ye (Perillae Folium) and Zisu Geng (Perillae Caudex), grind into powder, and apply externally daily. Wrap with fresh green lotus leaves, and administer Xi Re Tang (Heat-Draining Decoction) internally.
Ma said: Xi Re Tang (Heat-Draining Decoction) is quite effective for treating Nang Tuo (Scrotal Prolapse) and can be used.
Newborn infants, due to receiving toxins in the womb, develop red, swollen patches on the legs accompanied by fever, named Chi You (Red Roaming). "Roaming" refers to migration; if it roams over the entire body, it leads to death. Apply the foul water from inside hatched eggs once or twice for recovery. Alternatively, if an infant develops Gan (Mouth Thrush) in the mouth or a red lump in the cheek named Tang Lang Zi (Mantis Child), these are also fetal toxins. Use Dihuang (Rehmanniae Radix) 5 qian and Sheng Jun (Rhei Radix et Rhizoma) 1 qian, soak thoroughly in aged wine, remove, pound together into a paste, and apply to the sole of the foot. For males, the left foot; for females, the right foot. Bind well with silk cloth, change when dry, and stop when healed. Applying Nen Gao (Tender Ointment) to Chi You (Red Roaming) is also effective. For Kou Gan (Mouth Thrush), use Sheng Xiangfu (Cyperi Rhizoma) and Banxia (Pinelliae Rhizoma) in equal parts, grind into powder, mix with egg white to form a cake-like shape, and apply to the Yongquan point (KI-1) on the left foot of males and the right foot of females. It heals in one week. Yongquan (KI-1) is the center of the sole.
Ma said: Using foul water from eggs in the formula is difficult to obtain at times; instead, pounding Bajiao Gen (Musa Basjoo Radix) to extract juice and sweeping it can eliminate Tang Lang Zi (Mantis Child). Do not cut with a knife.
