This condition presents with a nodule below the groin. Initially, it is as large as a bullet, gradually expanding to the size of an egg, with little pain. At the onset, taking Zhongjiu Wan (Nine-Pill Decoction) along with Jinfeng Wan (Golden Centipede Pill), and applying Maliang Gao (Hemp and Coolant Ointment) externally can lead to internal resolution; if it does not resolve, the "Five Tigers Descend from Western Sichuan" method should be primarily employed: Geckus (Wugong), Scorpion (Quanxie), Bombyx Batryticatus (Jiangcan), Cicada Slough (Chantui), Pangolin Scales (Shanjia), Angelicae Sinensis Radix (Danggui), Red Peony Root (Chishao), Scutellariae Radix (Huangqin), Gardeniae Fructus (Zhizi), Forsythiae Fructus (Lianqiao), Aurantii Fructus (Zhiqiao), Lonicerae Japonicae Flos (Yinhua), Saposhnikoviae Radix (Fangfeng), Schizonepetae Herba (Jingjie), Rehmanniae Radix (Shengdi), Akebiae Caulis (Mutong), Polyporus (Zhuling), Pharbitidis Semen (Erchou), Rheomi Radix et Rhizoma (Dahuang), Mirabilite (Mangxiao), Coptidis Rhizoma (Huanglian), Angelicae Dahuricae Radix (Baizhi), Glycyrrhizae Radix et Rhizoma (Gancao). Decoct these ingredients in water and take on an empty stomach, three times daily, and the toxicity will naturally dissipate. If suppuration occurs, take the pus-draining agent for Dujuo Ju (Abdominal Corner Carbuncle) mentioned above, or apply Huarou Gao (Flesh-Transforming Ointment) during incision and drainage; when the head breaks, wash with Xunxi Tang (Fumigation and Washing Decoction) and apply Jiedu Gao (Detoxifying Ointment); after the pus is exhausted, use Tianran San (Natural Powder) to level the wound and generate flesh, while avoiding triggering foods.
This condition arises between the thighs, resembling Biandu (Groin Abscess), but Biandu is round while Yukou (Fish Mouth) is elongated, which is their difference; after ulceration, the shape resembles a fish mouth, hence the name. The treatment method is also similar to Biandu, involving external application, internal administration, fumigation, and pasting, aiming to generate flesh and level the wound. Pangolin Scales (three large slices, stir-fried), Angelicae Sinensis Radix Tail (San Qian), Rheomi Radix et Rhizoma (San Qian), Bombyx Batryticatus (Yi Qian, stir-fried), Black Pharbitidis Semen (Yi Qian), Elaeocarpus Chinensis Seed (Tu Shimie, three pieces), Glycyrrhizae Radix et Rhizoma Node (San Qian). Decoct these in one cup of wine and one cup of water until reduced to eight-tenths, take on an empty stomach, decoct the dregs again and take, wait until there are four bowel movements, then consume thin porridge to replenish. Besides Huangjia Chuan, Jiulong Dan (Nine Dragons Pill) is also an excellent formula for internal resolution of Yukou. In the past, a certain doctor in Chongqing manufactured this formula into "Yukou Neixiao Wan" (Yukou Internal Resolution Pill) for sale, making considerable profit, and many colleagues purchased it as a common clinic item, indicating its efficacy.
Jiulong Dan: Costus Root (Muxiang), Frankincense (Ruxiang, processed), Myrrh (Moyao, processed), Catechu (Ercha), Dragon's Blood (Xuexie, ground separately), Croton Seed (Badou, oil removed but heart retained, equal parts). Grind all into powder, mix with raw honey to form a mass, store in porcelain. When needed, roll into pills the size of peas, take one pill per dose, swallow with hot wine on an empty stomach. Another version states: roll into pills the size of mung beans, take nine pills per dose, hence the name "Nine Dragons Pill". Based on the editor's past experience, one pill seems too few, nine pills seem too many; generally, three to seven pills are most appropriate depending on the patient's constitution strength; occasionally nine pills are used, although the disease recedes quickly, the patient suffers from side effects like incessant diarrhea, which is unnecessarily painful.
Another Jiulong Dan formula from Wen Shulai's *Surgery Excerpts* differs slightly, specifically treating Yukou, Biandu, Xuan Yong (Hanging Perineum Abscess), and Heng Xuan (Horizontal Groin Swelling), suitable for those in the early stage without suppuration. The formula consists of Catechu, Dragon's Blood, Frankincense, Myrrh, Costus Root, and Pangolin Scales, each one liang, ground together. Mix with a paste made from decocting three liang of Angelicae Sinensis Radix Tail and two liang of Carthami Flos in wine, roll into pills the size of mustard seeds, take two qian per dose, swallow with hot wine on an empty stomach, and it will spontaneously ulcerate after several doses. I believe that once Biandu has formed, before suppuration, drastic measures must be taken to suppress its momentum and prevent suppuration. Removing the harsh purgative Badou seems to lose the original intent of the formula, but the part using Angelicae Sinensis Radix Tail and Carthami Flos decocted in wine to make pills is indeed commendable.
In clinical practice, the editor often uses the family heirloom "Yukou Neixiao Wan" to treat Biandu, with extremely reliable and safe efficacy. The formula is introduced as follows: Phellodendri Cortex (Huangbai), Schizonepetae Herba (Jingjie), Saposhnikoviae Radix (Fangfeng), Trichosanthis Radix (Huafen), Cicada Slough (Chantui), Tribuli Fructus (Jili), Akebiae Caulis (Mutong), Sophorae Flavescentis Semen (Huaizi), Pangolin Scales (Shanjia), Mustard Beetle (Banmao, equal parts), Geckus (Wugong), Scorpion (Quanxie), Black Sand (Heisha, double the dosage of previous herbs). Grind all into powder, roll into pills the size of mustard seeds, take twenty pills per dose, swallow with hot yellow wine.
