This sore arises at the junction of the foot and the calcaneus (heel bone). This area has little flesh over the bone, hence it is called "Qiong Gu" (Poor Bone). The condition is mostly caused by Wind-Dampness. Initially, the area becomes hard and itchy. If accidentally scratched open, blood and serum will flow out, often failing to dry up. Furthermore, exposure to wind causes unbearable itching, which may persist for years without cure. Clinically, if the tissue color is red and vibrant with pain, it is easy to treat; if the color is dark black and painless, it is difficult to treat. Internally, administer Jiu Wan (Nine Pills) and the following Qingjie Decoction (Clearing and Resolving Decoction); if the sore has raised edges and a depressed center, indicating necrotic bone (Jian Gu), apply Hongsheng Dan (Red Ascending Cinnabar Powder) externally and cover with Gezhi Gao (Paper-Interposed Plaster).
Fangfeng (Saposhnikoviae Radix), Jingjie (Schizonepetae Spica), Suhexiang (Styrax), Huangqin (Scutellariae Radix), Zhizi (Gardeniae Fructus), Lianqiao (Forsythiae Fructus), Chishao (Paeoniae Radix Rubra) (each 5 qian); Huangbai (Phellodendri Cortex), Kushen (Sophorae Flavescentis Radix), Niuxi (Achyranthis Bidentatae Radix), Mugua (Chaenomelis Fructus), Yinhua (Lonicerae Japonicae Flos), Cangzhu (Atractylodis Rhizoma), Huafen (Trichosanthis Radix), Baizhi (Angelicae Dahuricae Radix), Mutong (Akebiae Caulis) (each 3 qian).
Decoct the above ingredients in water and take on an empty stomach. Externally, fumigate and wash with Xunxi Tang (Fumigation and Washing Decoction), then apply Gezhi Gao, changing once daily; if the healthy flesh at the edges itches, apply Taisui Mo (Taisui Ink) to the edges, the redness and swelling will subside naturally, and the itching will stop immediately. At this time, Jiawei Tianran San (Modified Natural Powder) can be used to promote granulation and level the wound. If the sore is not red and vibrant but appears dark black, accompanied by foul-smelling discharge, primarily use a formula to dispel Dampness and expel Wind:
Chishao (Paeoniae Radix Rubra), Fangfeng (Saposhnikoviae Radix), Jingjie (Schizonepetae Spica), Baizhi (Angelicae Dahuricae Radix), Bohe (Menthae Haplocalycis Herba), Zisu (Perillae Folium), Huangqin (Scutellariae Radix), Huangbai (Phellodendri Cortex), Jili (Tribuli Fructus), Cangzhu (Atractylodis Rhizoma), Dahuang (Rhei Radix et Rhizoma), Yinhua (Lonicerae Japonicae Flos), Dafengzi (Crotonis Fructus), Shechuangzi (Cnidii Fructus).
Cut the above ingredients into slices. Take one large bowl of strong vinegar and one earthenware jar. Place the herbs in the jar and decoct for the time it takes to burn one stick of incense. Take about one hundred to one hundred twenty leaves of Firmiana simplex (Tongzi Ye), soak them in the vinegar until soft, let them cool, and apply them to the sore, changing once daily. Continue until the sore no longer itches and the margins are red and vibrant. Then apply Gezhi Gao, use the Xunxi Tang for washing and fumigating, sprinkle Tianran San, to promote granulation and level the wound.
This sore also arises on the calcaneus. When occurring in women, it is named "Qunbian Chuang" (Skirt-Edge Sore); when occurring in men, it is named [Missing Character] Chuang. Therefore, the treatment method is the same as that for [Missing Character] Chuang.
This sore arises above the ankle, hence its name. The treatment method is the same as that for Ruhua described previously. In the early stage, use 3 liang each of Pugnongying (Taraxaci Herba) and Yeyutou (Arisaema Cum Bulbo or similar wild tuber), crush them together, mix evenly with Jiuzao (Wine Lees), and apply to the sore. Change when dry; there is considerable hope for recovery.
