In the initial stage of a Yang syndrome, there is redness and pain; the base of the lesion is well-defined, and the swelling is elevated like a bow. After seven days, pain may persist or cease intermittently; by fourteen days, pus gradually forms within the lesion. As the pathogenic toxin is expelled with the pus, the spirit becomes refreshed, necrotic tissue sloughs off, new flesh grows, and Qi and Blood become abundant. The tender new flesh resembles pearls in fullness, with a beautiful color, fresh and moist like the red of pomegranate seeds. Naturally, the "Seven Evils" (ominous signs) will not occur, and the "Five Goods" (auspicious signs) should appear. It must be known that this belongs to a Pure Yang syndrome; with appropriate medical regulation, efficacy will naturally be achieved.
Generally, for carbuncles and sores in their initial stage, if there is heat, redness, and pain, with a gathered base that does not disperse, and swelling that is localized without spreading, but elevated high like a bow, this indicates a favorable course (Shun Zheng) where ulceration and healing are easy. Detailed observation and examination will naturally lead to visible results.
In the initial stage of a Yin syndrome, the lesion is small like millet grains, without redness or swelling, presenting as stiff nodules; it is woody and hard without pain, with a flat, broad base, dark in color and lacking luster. After twenty-one days, it fails to ulcerate and slough; the surface is soft and sunken without pus, forming an empty cavity. The granulation tissue on the sore resembles shed nails, and the induration in the pore looks like a lotus pod. If purple-black pus is thin and foul-smelling, and the "Seven Evils" are observed, death is certain. It must be known that this belongs to a Pure Yin syndrome; even with the medical skills of Qibo or Huangdi, life expectancy cannot be prolonged.
Generally, for carbuncles and sores in their initial stage, if there is no redness, swelling, or heat, resembling millet grain nodules, woody and hard without pain, and after seven days neither ulcerating nor healing, with granulation tissue on the sore resembling shed shells, and the opening resembling a flower bud knotting, this belongs to an unfavorable course (Ni Zheng). If the "Seven Evils" appear, death is inevitable; if only one or two of the "Five Goods" are present, rescue is barely possible; otherwise, efforts are futile.
A condition involving half-Yin and half-Yang belongs to dangerous syndromes. Yang characteristics indicate auspiciousness, while Yin characteristics indicate ominous outcomes, clearly distinguishing life and death. Symptoms resemble Yang syndrome with slight pain and swelling, yet resemble Yin syndrome with partial hardness and elevation. Swelling without ulceration is due to Spleen deficiency; ulceration without healing is due to thick, difficult-to-expel pus. Although the "Five Goods" may coexist, how can one escape disaster if the "Seven Evils" appear? If appetite remains good and urination and defecation are regulated, treatment is feasible. Following proper methods yields immediate effectiveness; as Yang increases and Yin decreases, recovery will naturally follow.
Half-Yin and half-Yang conditions all belong to dangerous syndromes. They resemble Yang syndrome but lack intense heat, and resemble Yin syndrome but lack extreme stiffness. Favorable courses often allow for rescue, whereas unfavorable courses are difficult to treat. In cases of Yin rising and Yang leaking, although many dangers exist, there remains a glimmer of hope for survival.
