The verse states: Tianran San (Natural Powder) contains Qianfen (Lead Carbonate), possessing miraculous efficacy to cure various sores and toxins. If the patient fears pain, add Qingfen (Calomel) and Moyao (Myrrh); for itching, Tonglv (Malachite) and Yaoxianmo (Medicated Thread Powder) are effective. Stir-fry Qianfen until fine, and apply flexibly according to clinical conditions. Mastering the flexible use of this formula for various strange diseases ensures one will not fear poverty while traveling the world.
[Prescription]
Qianfen (Lead Carbonate) (1 liang)
[Preparation]
Stir-fry in a pan over fire until yellow, store in a bottle for future use.
[Usage]
(1) For pain, add Qingfen (Calomel) 1 qian, Zhiruxiang (Processed Frankincense) 1 qian, Zhi moyao (Processed Myrrh) 1 qian, Xuexie (Dragon's Blood) 1 qian, Chishizhi (Red Halloysite) (sieved) 1 qian, Bingpian (Borneol) 1 fen.
(2) For itching, add a small amount of Tonglv (Malachite) (boiled with Katea [Catechu] water, then stir-fried to golden yellow), or add Yaoxianmo (Medicated Thread Powder) 3 qian, Jinbo (Gold Leaf) 3 tie.
(3) For sores with exudate, add Haipiaoxiao (Cuttlebone) 1 qian, Wenha (likely referring to a specific shell or mineral, contextually grouped with drying agents) 1 qian, Lingyao (Spirit Medicine) 5 fen.
(4) For sores that fail to heal, are non-red but itchy, add Yincui (Silver Green/Copper Acetate) 1 qian.
(5) To promote tissue regeneration and flatten the wound margin, add Longgu (Fossilized Bone) 1 qian, Xiangpi (Elephant Skin) 1 qian; adding Muli (Oyster Shell) is also beneficial.
Gu Shicheng’s *Yangyi Daquan* (Comprehensive Collection of Surgeon's Medicine) contains a formula specifically treating hemorrhoids, fistulas, back carbuncles, ding abscesses, and sores breaking down with yellow exudate spreading into eczema-like lesions. This is essentially Tianran San before being named. The formula uses Hangfen (Refined Lead Carbonate) 1 liang, melted in a silver pot until old golden-yellow, cooled and powdered, mixed with Bingpian (Borneol) 3 fen. Apply by sprinkling on the lesion and rubbing with hand; fluid stops, pain subsides, and new sores do not form. This is Tianran San.
Another family manuscript contains "Jintang San" (Brocade Hall Powder), claimed to remove toxins, generate flesh, close wounds, and drain pus, serving as an important external medicine. It uses Shang Jintang Fen (Top-grade Jintang Powder, i.e., Lead Carbonate, Jintang being the brand name) 1 liang, stir-fried in a copper ladle until yellow, wrapped in paper, and exposed under a water vat overnight before storage. It acts immediately upon use.
Another manuscript records: For persistent oozing from pus pits, unceasing itching, and spreading lesions, this powder is exceptionally effective. It uses Gongfen (Palace Powder, i.e., premium Lead Carbonate) 1 liang, stir-fried to black-yellow, mixed with Bingpian (Borneol) 3 fen, powdered and applied. This is also an unnamed version of Tianran San.
*Shenqi Liangfang* (Miraculous Excellent Prescriptions) contains a formula for traumatic injuries, which is an additive variation of Tianran San. It uses Zhu Banyou (Pork Lard) 4 liang and scallions with roots 1 bunch. Pound the lard and scallions on a stone slab into a paste. Then, stir-fry Qianfen (Lead Carbonate) 4 liang in a clay pot until yellow, mix into the lard and scallion paste to form a cake, and apply to the injury site for healing.
*Yi Hutian Shu* (Book of Yi Hutian) contains "Yuanlong Dan" (Primordial Dragon Pill), specialized in generating flesh and closing wounds. It consists of Qianfen (Lead Carbonate) 1 liang stir-fried yellow, and Bingpian (Borneol) 2 qian, ground into fine powder for dry application. This is another name for Tianran San.
Ge Hong’s *Zhouhou Beiji Fang* (Emergency Prescriptions Kept Up One’s Sleeve): For falls causing blood stasis, falling from heights with blood rushing to the heart, presenting with blue complexion, shortness of breath, and near-death state, administer Qianfen (Lead Carbonate) 1 qian mixed with water.
*Beiji Fang* (Emergency Prescriptions): Treat dampness in the groin by dusting with Hufen (White Lead). Also treats wet and dry tinea/crusted sores.
*Weisheng Yijian Fang* (Simple Hygiene Prescriptions): For setting bones, knitting sinews, relieving pain, and invigorating blood, use Dingfen (Fixed Powder/Lead Carbonate) 1 qian, Danggui (Angelica Sinensis) 1 qian, Pengsha (Borax) 1.5 qian, mixed with Sumu (Sappan Wood) decoction. Drink Sumu soup frequently. Heji Bureau Formula and Hongshi Jiyan Fang both contain this formula, named "Shenshou San" (Divine Bestowal Powder). Hong stated it was highly effective with detailed instructions.
Qingnang Milu Formula: For fractures and bone setting, use Gongfen (Palace Powder) and Pengsha (Borax) in equal parts, powdered. Take 1 qian per dose with Sumu (Sappan Wood) decoction, continuing to drink Sumu soup frequently. Highly effective, identical to the above formula.
