Shuangbin Fa (Double Temporal Carbuncle)
Simultaneous occurrence on both temples is named "Shuang Fa" (Double Temporal Carbuncle). Its toxicity is heavier than that of a single-sided onset. If the toxic force is lighter than the single-sided type and no malignant symptoms are present, the prognosis is favorable; if severe, it is untreatable.
Duikou Fa (Opposite Mouth Carbuncle)
This sore arises at the lower back of the head, directly opposite the mouth in front. It is located within the Du Meridian (Governing Vessel), where toxic vapors condense, situated between the Fengfu (GV16) and Yamen (GV15) acupoints. The symptoms are most ferocious. If pus forms within eight or nine days, it is treatable; if the toxic force is too fierce, it inevitably leads to the calamity of throat penetration and skull detachment. If not treated early, death is imminent.
Bian Li Fa (Hairline Margins Carbuncle)
Tianzhu (BL10) belongs to the Foot-Taiyang Bladder Meridian, which has abundant blood but scarce qi. Whether occurring on the left or right, bilateral occurrence is more severe. At the initial stage, urgent rescue is required; delay may allow the toxin to enter the brain, with pathogenic heat attacking upward, causing redness, swelling, hardness around the periphery, extending to behind the ears, accompanied by chills, fever, and suppuration. Urgent application of Tuo Li method (Supporting Interior/Boosting Qi to expel toxin) is necessary; otherwise, the toxin may wander between the waist and back, and recurrence may be incurable.
Er Fa (Ear Carbuncle)
It belongs to the Foot-Shaoyang Gallbladder Meridian, which has abundant qi but scarce blood. The sore develops above and below the Xuanli (GB7) and Zhuoke auxiliary points. After five or six days, it gradually grows into a honeycomb shape, with purple skin, heat, and burning pain like fire. If punctured within ten days and pus appears, survival is possible; if there is no pus but bleeding occurs, along with loss of taste and poor spirit, death will occur within twenty-four days, irretrievable. The situation is the same on both sides.
Taiyin Fa (Taiyin Carbuncle)
This sore develops at the Tongziliao (GB1) acupoint on both left and right sides, identical to the previous description, belonging to the Foot-Shaoyang Gallbladder Meridian, which has abundant qi but scarce blood. If there is redness, swelling, pus, and pain perception, the prognosis is good, and Tuo Biao method (Surface-Supporting/Pushing out toxin via surface) is suitable. If extreme thirst, mental confusion, uncontrolled bleeding, or blackish-green color without pain, forming worm-eaten holes, with blood exhausting but no pus appearing, then death ensues.
Lian Fa (Face Carbuncle)
It belongs to the Foot-Yangming Stomach Meridian, which has abundant qi but scarce blood. The sore develops at the Sibai (ST2) and Juliao (ST3) acupoints, identical on both sides. The color resembles grapes, the shape gradually enlarges, or mother-and-son sores (primary sore with satellite lesions) appear. Death occurs within eight days. Treatment should be based on the severity indicated by Wu Shan Qi E (Five Good Signs and Seven Evil Signs). For lesions on the left, double the dosage of Danggui (Angelica sinensis) in internal support formulas; for lesions on the right, triple the dosage. This is the subtlety of the treatment.
