Nao Fa (Hair Abscess on the Vertex)
This carbuncle occurs at the Niwan Gong acupoint on the vertex of the head. It is associated with the Foot-Taiyang Bladder Meridian and acts in conjunction with the Du Mai (Governing Vessel). This meridian is characterized by abundant blood and deficient qi. The condition presents like a fire scorching the area. The pustule resembles a cannonball in shape, about the size of a coin, with color and form similar to grapes. The apex has a honeycomb-like pore the size of a rice grain. The surrounding tissue is hard. If the color is crimson, it is treatable. If there is blood stagnation causing mental confusion and restlessness, the prognosis is fatal. If pus forms after eight days and can be lanced, survival is possible.
Naohou Fa (Hair Abscess at the Back of the Head)
This carbuncle also involves the Foot-Taiyang Bladder Meridian, along with the Du Mai (Governing Vessel) and Yangwei Mai (Yang Linking Vessel). The area is rich in blood but poor in qi. It is located between the Yuzhen (Double 9) acupoints and the Fengfu (Du 16) acupoint. Pain and itching vary, and both favorable and unfavorable signs may appear simultaneously. For exterior conditions, induce sweating; for interior conditions, use dispersing methods. Treatment should follow the detailed principles described previously.
Erhou Fa (Abscess Behind the Ear)
This ulcer is caused by toxic heat accumulation in the Hand-Shaoyang Sanjiao Meridian. It occurs above and below the Jiaosun (TE 17) and Luxi (TE 18) acupoints behind the ear, as well as on both sides of the ears. This meridian is abundant in qi and deficient in blood. Ulcers occurring here are difficult to heal. In severe cases, it may lead to deafness, swelling and pain of the throat, cheek pain, and pain in the little finger, ring finger, shoulder, and elbow. Treatment should focus on dispelling wind and suppressing fire, using internal purgatives or tonic-depurative methods, tailored to specific symptoms.
You Bin Fa (Abscess at the Right Temporal Hairline)
This ulcer occurs at the right temporal hairline and belongs to the Foot-Yangming Stomach Meridian. This meridian is abundant in both qi and blood. It is located above and below the Touwei (ST 8) and Xiaguan (ST 7) acupoints. Initially, it appears as large as a furuncle, then gradually enlarges. The surroundings are yellowish-red and hard as stone. If an abscess forms, the surface may resemble pig bristle pores. If the patient experiences delirium and trance during sleep, vomiting fresh blood that flows into the limbs, death is inevitable.
Zuo Bin Fa (Abscess at the Left Temporal Hairline)
This ulcer occurs at the left temporal hairline, involving the same meridians and acupoints as the You Bin Fa without difference. Generally, toxic qi binds on the right side, which belongs to the Foot-Yangming Meridian; on the right side of the meridians, toxin binds in the Qi level; the left side belongs to the Blood level. When prescribing medication, one must observe the exterior-interior and deficiency-excess patterns, then carefully distinguish the Qi and Blood involvement on the left and right sides to guide treatment, achieving miraculous results.
