Honeycomb Carbuncle (Fengwo Fa)
This sore arises at the Huozhong acupoint. This is an acupoint of the Foot-Shaoyin Kidney Meridian. This meridian has abundant Qi but scarce Blood. If the honeycomb carbuncle appears between the chest and breasts, either on the left side, on the right side, or on both sides simultaneously, it is due to excessive and urgent Heart Fire. It should be treated with herbs that internally drain Heart Fire. The symptoms include a sensation of emptiness in the heart resembling hunger, yet a lack of desire to eat; dry tongue and swollen throat. If not treated early, the pathogenic toxin will return to the Heart, leading to incurability.
Biao Ju Carbuncle (Biaoju Fa)
This sore arises on both sides of the back. These are acupoints of the Foot-Taiyang Bladder Meridian and others. This meridian has abundant Blood but scarce Qi. the Jizhong acupoint belongs to the Du Mai (Governing Vessel). Initially, it appears like a bean, small ones the size of a plum seed, large ones the size of a plum fruit. The color is indeterminate, being black or white. The pain radiates to the heart. The surrounding swelling resembles the lips of an ox. If occurring before the age of forty, it is treatable; if occurring after forty, it is feared to be difficult to cure due to physical decline. The situation is the same whether it occurs on the left or right.
Ti Ju Carbuncle (Tiju Fa)
This sore arises on the Foot-Taiyang Bladder Meridian, which has abundant Blood but scarce Qi. It extends from Feishu (BL13) upwards to Ganshu (BL18) downwards, involving five acupoints. The condition is identical on both left and right sides. It can be punctured on the eighth day. If it does not suppurate, death will occur within twenty days. Alternatively, if upon initial onset the swelling resembles chili seeds, death will ensue.
Upward-Developing Honeycomb Carbuncle (Fengxiang Shangfa)
This sore arises on the back, involving the Du Mai (Governing Vessel) and the Foot-Taiyang Bladder Meridian. It has abundant Blood but scarce Qi. It is most inadvisable for the head of the sore to develop upward; this is termed "Reverse Syndrome" (Fan Zheng), which is extremely malignant. If there is redness, swelling, and pain without the manifestation of the "Seven Evil Signs" (Qi E), it is treatable. If there is no discharge of pus or phlegm, and if the infection spreads to the shoulders and arms, it is absolutely incurable. If it penetrates into the membranes, it is also incurable. For any sore where the opening fails to close, regardless of whether it develops upward or downward, one should use formulas to gather the toxin and cause it to wither and die.
Downward-Developing Honeycomb Carbuncle (Fengxiang Xiafa)
Sores with heads developing downward are less common and tend to follow a smoother course. However, rapid spread is still undesirable. One must carefully use herbs to support and tonify the healthy Qi, fearing that the toxin may return to the Heart. If the membrane is penetrated without spreading, but pus is present, and the patient feels pain and can eat, it is treatable. If the patient cannot eat, there is no pus, and the pulse is weak, death will occur.
