Yangsui Dingzi (Copper Mirror Pill), Zhanzhu Gao (Azure Pearl Ointment), Buzhong Yiqi Tang (Decoction to Supplement the Middle and Augment the Qi), and Liuwei Dihuang Tang (Six-Ingredient Rehmannia Decoction) are all recorded in the first volume of this book. Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Jiawei Guipi Tang (Modified Restore the Spleen Powder) are both recorded in the Scrofula Chapter.
Hengxun (Transverse Abscess) arises below the lower abdomen, slightly above the inguinal fold. In the early stage, use 1 liang of Halophytum orobanchoides (Huluba), decocted with one cup of wine and one cup of water. Drink until intoxicated; sweating will cause immediate resolution.
If an abscess has formed, use Hugong Miaoji San (Master Hu's Miraculous Powder). (See Swelling and Toxicity Chapter)
Gan Chuang (Chancre) is also known as Du Jing Chuang (Jealousy Semen Ulcer) or Chi Chuang (Shame Ulcer). The Classics state: "The anterior yin is governed by the sinews." It also states: "The Du Mai (Governing Vessel) has a collateral that circulates around the genitalia and meets at the anus." Furthermore, it states: "The Kidney opens into the two yins." Therefore, ulcers arising here belong to the Liver, Du Mai, and Kidney meridians. However, their names differ and their forms vary, leading to diverse treatment methods. An ulcer occurring below the Makou (urethral meatus) is called Xiagan (Lower Chancre); one occurring on the shaft of the penis is called Zhu Gan (Woodworm Chancre). Those with a purple color and painful erection are caused by the application of aphrodisiac pastes during sexual practices. Those accompanied by turbid urination and dysuria are caused by unfulfilled sexual desire. All these conditions are suitable for Longdan Xiegan Tang (Gentian Liver-Draining Decoction), with external applications and washes. For severe cases, Lushui Wan (Aloe Pill) is used. This condition presents with swelling, pain, and fever/chills as branch symptoms (biao), while Liver-Kidney Yin deficiency is the root (ben). Thus, for severe swelling, pain, and ulceration, Bazhen Tang (Eight-Treasure Decoction) is added with Chai Hu (Bupleurum Root) and Zhi Zi (Gardenia Fruit). For fatigue in the late afternoon, Buzhong Yiqi Tang is added with Long Dan Cao (Gentian Root) and Zhi Zi. For evening heat due to Yin deficiency, Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) is used.
Furthermore, ulcers on the penis where the skin swells and wraps around the lesion are named Xiukou Gan (Sleeve-Mouth Chancre). Those that become extensively ulcerated over time are named Lazhu Gan (Candle Chancre). Cases where pain radiates to the testicles and the scrotum becomes swollen and heavy are named Jiyan Gan (Chicken-Eye Chancre). These conditions are all caused by illicit intercourse and poor hygiene of the genitalia. Qinggan Shenshi Tang (Liver-Clearing and Dampness-Draining Decoction) is appropriate; for severe cases, Erzi Xiaodu San (Two-Seed Detoxification Powder) is used. If there is pain with intense itching, superficial ulceration resembling a peeled rotten apricot, it is named Sao Gan (Itching Chancre). This is caused by Damp-Heat pouring downward. Only external treatment is required.
If Gan Chuang persists without healing and Bi Du (Inguinal Buboes) recur, it indicates the presence of Meidu (Syphilis). One should first detoxify; once the toxin is eliminated, the chancre will heal. Yangmei Yi Ji San (One-Dose Chinese Bayberry Powder) is suitable. When contracting syphilis initially, the mistaken use of fumigation and rubbing medications leads to rotting, known as Yangmei Gan (Bayberry Chancre). Pain within the urethra during urination, caused by taking Jie Yao (potent aphrodisiacs or suppressive drugs), is named Yangmei Nei Gan (Internal Bayberry Chancre). Both conditions are treated similarly to Jiedu (Syphilitic Gumma). Jiawei Wubao Dan (Modified Five-Treasure Elixir) or Erzi Xiaodu San may be used. A hole like a palm-eye appearing beside the Makou, which itches inside and exudes a small amount of pus when pressed, is named Xuangen Gan (Spinning-Root Chancre). This results from Liver-Kidney Yin deficiency and downward-pouring Damp-Heat. Jiawei Xiaoyao San and Liuwei Dihuang Wan should be taken for a long period to achieve cure.
Yinting refers to the penis remaining erect and unable to retract, caused by Heat injury. Clear the heat using Xiao Chai Hu Tang (Minor Bupleurum Decoction) added with Huang Lian (Coptis Rhizome), assisted by Huang Bai (Phellodendron Bark). If the penile shaft is hard, use Qing Pi (Tangerine Peel) as the sovereign herb, assisted by Wind-dispelling herbs. Wash with Xiaomiao (Mirabilite) and Jingjie (Schizonepeta Spike), and apply Wen Ge Mo (Clam Shell Powder) mixed with Si Gua Zhi (Luffa Juice). Yinsuo refers to the penis retracting into the abdomen, caused by Cold injury. Although warming is indicated, if the tongue curls and the testes retract, it is a fatal sign. Yinwei (Impotence) is caused by excessive depletion, injuring the Liver. It is also stated: "An excessively large Kidney pulse indicates Impotence." Furthermore, "When Taiyin (Spleen/Earth) governs the year, Dampness descends, and Kidney Qi declines, resulting in failure to erect." Chai Hu Shengshi Tang (Bupleurum Dampness-Overcoming Decoction) and Guzhen Tang (Solidify the True Decoction) are appropriate.
