Jiechang syndrome (Intercalation Syndrome) is prolapse of the rectum. For Qi deficiency, use Renshen (Ginseng), [missing character], Danggui (Angelica Sinensis), and Baizhu (Atractylodes Macrocephala). For Blood deficiency, use Danggui (Angelica Sinensis) and Shaoyao (Peony). If there is deficient heat, add Huangbai (Phellodendron Bark). If there is sinking of Qi, assist with Shengma (Cimicifuga Rhizome). Externally, wash with a decoction of Bohe (Mint). Place slices of aged pickled radish to support the anus; this yields effective results. However, there are differences: if part has already retracted and the rest gradually forms scabs, occasionally falling off, it is Jiechang syndrome (Intercalation Syndrome); this is harmless and will heal and fall off spontaneously.
Bianyong (Perineal Abscess) arises below the lower abdomen, in the perineal cleft. This is the pathway where essence exits, belonging to the territory of the Foot-Jueyin Liver Meridian, and is also a tunnel for the Chong, Ren, and Du meridians. These meridians are rich in blood, hence named Xueshan (Blood Hernia), commonly called Biandu (Genital Swelling), meaning a toxin arising in an inconvenient place. When it ruptures, it resembles a fish's mouth; due to its opening and closing during movement like a fish's mouth, it gets this name. If it grows horizontally, it arises above the cleft. Former sages stated: "Due to being moved by scenery, dreaming without ejaculation, resulting from the stagnation of essence and blood." If Sun Simiao indeed argued thus, would monks, nuns, officials, bachelors, and widows not mostly suffer from this? Essentially, this syndrome must be caused by illicit intercourse; unclean genitalia of prostitutes transmit "libidinal fire." Some say it is caused by fatigue and exhaustion stirring up deficient fire, causing essence to shift outward and stagnate. If obtained through fatigue, pain occurs upon standing and walking, ceasing when sitting or lying down; this is merely a difference in presentation. Although so, cases due to fatigue account for only one or two in a hundred. For those caused by illicit intercourse, first disperse pathogens felt in the cou-li (skin and interstices) using Tougu Soufeng San (Bone-Penetrating Wind-Searching Powder); then remove toxins from the Liver and Kidney meridians using Jiawei Shenshou San (Modified Divine Bestowal Powder). Externally, moxa-burn with Yangsui Dingzi (Yang-Sun Moxa Plug) and apply Zhanzhu Gao (Green Pearl Ointment). If pus has formed, use Furong Neituo San (Hibiscus Internal-Expulsion Powder) or Buzhong Yiqi Tang (Middle-Warmer Boosting Qi Decoction) plus Shegan (Belamcanda Rhizome). If ulcerated but not perforated, use Jiawei Tonong San (Modified Puncture-Pus Powder). If ulcerated but not healing, use Jiawei Dabu Tang (Modified Great-Nourishing Decoction). For those caused by fatigue, use Buzhong Yiqi Tang (Middle-Warmer Boosting Qi Decoction) plus Chuanshanjia (Pangolin Scales). For those caused by sexual excess, use Liuwei Dihuang Tang (Six-Ingredient Rehmannia Pill) plus Taoren (Peach Kernel), doubling the dose of Niuxi (Achyranthes Root). For those caused by forced retention of essence (abstinence), use Wuling San (Five-Ingredient Powder) [Baizhu, Fuling, Zhuling, Zexie, Rougui each 1 qian], plus Niuxi (Achyranthes Root) 2 liang, decocted in water, taken with wine.
Pixuetong (Spleen-Blood Pain) is a condition specific to women. Its presentation involves a mass in the lower abdomen, or a sensation of obstruction, or swelling of both thighs near the entrance and the anus, accompanied by dribbling urination. Use Shuangjie San (Double-Release Powder). If menstruation is irregular, with internal heat, thirst, poor appetite, hypochondriac distension, night sweats, leukorrhea, alternating chills and fever, and a fine, rapid pulse, it is a deficiency pattern. Regulate with Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Jiawei Guipi Tang (Modified Heart-Spleen Nourishing Decoction). For heavy bearing sensation in the lower abdomen, use Buzhong Yiqi Tang (Middle-Warmer Boosting Qi Decoction) plus Qingpi (Green Tangerina Peel), Zhizi (Gardenia Fruit), Fuling (Poria), and Cheqianzi (Plantago Seed).
