Lacquer dermatitis initially presents with itching, resembling urticaria (wind wheals). Subsequently, the head and face exhibit flaccid swelling, while the entire body suffers from skin breakdown, oozing fluid, and pain similar to leprosy. In severe cases, alternating chills and fever occur. This condition arises because fresh lacquer is acrid, hot, and toxic; combined with the patient's constitutional bias and loose interstices (skin texture and pores), it contracts the pathogenic qi of the lacquer. It is appropriate to take Huaban Jiedu Tang (Spot-Resolving and Toxin-Unblocking Decoction) and crush raw crab to mix with wine, filtering out the dregs before drinking. If the sores have ulcerated and are oozing, wash the affected area with three jin of fresh willow leaves decocted in water, or one jin of dried lotus leaf decocted in water, or soak and wash with four to five liang of alum dissolved in water. For areas that are not broken but show red spots and itching, apply Erwei Xiaodu San (Two-Ingredient Toxin-Resolving Powder).
Blood Arrow syndrome is caused by blazing Heart Fire forcing blood to shoot out from the hair follicles. Urgently use half a longan shell, burn it flat, fill it with Jingmo juice, cover the affected area, and bind tightly. Remove it after three to five days. Internally, administer Mahuang (Ephedrae Herba) two to three liang mixed with good wine, or drink ginger juice, which leads to immediate recovery. Subsequently, take medications for clearing Heart heat and cooling blood. If excessive blood loss has occurred, Huangqi Buxue Tang (Astragalus Blood-Nourishing Decoction) is suitable.
Huangqi Buxue Tang treats conditions where excessive blood loss causes tendons to lose nourishment, resulting in limb spasms, contractures, and trismus resembling tetany.
Huangqi (Astragali Radix, 1 liang) Danggui (Angelicae Sinensis Radix, 5 qian; if complicated by wind or broken-skin tetanus, equal parts added with Qianghuo and Fangfeng 1 qian each, Jingjie Sui 2.5 qian).
All ingredients are decocted in water and taken orally.
Blood Nevus begins as a red spot resembling a mole, gradually enlarging to the size of a bean, bleeding continuously when rubbed. This is caused by Liver Meridian stagnant fire and blood heat. Use a caustic medicine to pick away the nevus, then apply Shengji San (Flesh-Generating Powder) to close the wound. For those with severe internal heat, take medications for clearing Liver heat and cooling blood.
The *Mai Jing* states: Head trauma with internal blood stasis causes abdominal distension and fullness. A strong pulse indicates survival, while a small and weak pulse indicates death. Trauma without distinguishing the twelve meridians results in blood returning to the Liver and accumulating under the hypochondrium. Severe pain leads to spontaneous sweating, which is due to Wind transformation. The treatment principle is first to expel evil blood and unblock the meridians, then harmonize the blood to stop pain, followed by regulating Qi and nourishing blood, and tonifying Stomach Qi, which naturally yields efficacy. If abdominal pain worsens upon pressure, it indicates stasis, and purgatives should be used. If pain persists after purgation and remains tender on pressure, stasis is incomplete, and Fuyuan Huoxue Tang (Restore Vitality and Invigorate Blood Decoction) should be used to regulate. If abdominal pain does not worsen upon pressure, it indicates Qi and Blood injury; Siwu Tang (Four Substances Decoction) added with Renshen, Huangqi, and Baizhu should be used for tonification and harmonization. If using Dahuang and other purgatives fails to move the blood and instead causes chest distension and asthma, one should follow the Qi mechanism, using Rougui and Muxiang (2 qian each) taken with wine to expel black blood, along with previously ingested medicines, leading to revival. For the weak, when using purgatives, add Muxiang and Rougui to decoct together, borrowing their heat to move Qi. However, if taking Qi-moving drugs leads to chest oppression, shortness of breath, poor appetite, fatigue, dark complexion, and thin pus, this indicates deficiency of Form and Qi, requiring tonification.
Chest and abdominal distension and pain, constipation, wheezing, coughing, and hemoptysis indicate blood stasis obstruction; use Danggui Daoshi Tang (Angelica Guiding Stagnation Decoction) to unblock. If pain worsens in the chest and abdomen after purgation, it indicates Liver Blood injury; use Sijunzi Tang (Four Gentlemen Decoction) added with Chuanxiong and Danggui for tonification. Fever after purgation indicates Yin Blood injury; use Siwu Tang added with Renshen and Baizhu for tonification. Chills after purgation indicate Yang Qi injury; use Shiquan Dabu Tang (Ten Complete Tonifying Decoction) for tonification. Nausea after purgation indicates Stomach Qi injury; use Liujunzi Tang (Six Gentlemen Decoction) added with Danggui for tonification. Diarrhea after purgation indicates Spleen and Kidney injury; use Liujunzi Tang added with Roudoukou and Pugu Zhi for tonification. Cold extremities, confusion, and sweating after purgation indicate Yang Qi deficiency and cold; use Shenfu Tang (Ginseng and Aconite Decoction) for warming. Vomiting, diarrhea, cold limbs, and cyanotic nails indicate severe deficiency-cold; double the dose of Shenfu Tang. Trismus, open hands, enuresis, cyanotic lips, profuse phlegm, and cold body indicate extreme deficiency and critical collapse; administer a large dose of Shenfu Tang, with many survivors.
Floating and tight pulse, chills, fever, and body pain indicate concurrent external pathogens. Treatment should focus on dispersing and releasing. Spring: Wuji San (Five Accumulations Powder), Xiangsu Yin (Perilla and Liquorice Drink); Summer: Xiangsu Yin; Autumn: Zhengqi San (Upright Qi Powder); Winter: Hejie San (Harmonizing and Resolving Powder). Adjust according to symptoms.
Bleeding from lesions or orifices indicates Liver Fire; use Jiawei Xiaoyao San (Modified Free Wanderer Powder). Insufficient Middle Qi causing blood to lack attachment requires Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). Internal collapse of Yuan Qi failing to control blood requires Dushen Tang (Single Ginseng Decoction) added with Paojiang (Prepared Ginger). If ineffective, add Fuzi. Internal blood accumulation causing hematemesis requires Siwu Tang added with Huangqin and Chaihu. For severe injuries with coma, although there is blood stasis, do not purge. Urgently tonify with Dushen Tang, then resolve stasis with Huarui Shi San (Flower Coral Stone Powder), fearing Yang collapse from purgation.
Injuries from labor, anger-induced distension, or accidental ingestion of Dahuang damage Yang collaterals, causing hematemesis, epistaxis, hematuria, and melena; damaging Yin collaterals causes blood clots, blood accumulations, and dark muscle discoloration. This indicates organ deficiency and meridian dysfunction; urgently tonify Spleen and Lung, with potential for survival. Regrettably, patients often fail to understand this and refuse this method.
For severe injuries causing breathlessness and aphasia,灌 warm Child Urine. Regardless of constitution strength or presence of stasis, for hypochondriac distension/pain, fever, irritability, and dry mouth, this method surpasses others, promoting elimination of the old and generation of the new with miraculous effects. Adding it to blood-invigorating formulas ensures immediate results. For upper body injuries, add Leizhi Juice (Chinese Chive Juice); for coma, use Wujin Wan (Black Gold Pill) dissolved in Honghua Wine and Child Urine.
For bone fractures with broken skin, decoct medicines in water; for intact skin, decoct in wine. However, Child Urine must be added to activate stasis. Pure wine decoction may cause distension. Combined with bone-setting herbs, it may cause constipation. (Use non-ashy wine, red or white).
For trauma with stagnant stasis, promote urination and defecation first; do not wait for constipation, lest it become a serious disease. Over-purgation makes the pulse stronger and larger, leading to misdiagnosis of stasis and repeated purgation, often resulting in premature death.
Herbal medicines are contraindicated in trauma; ingestion causes newly formed bones to be too large to fit into joints. Ziran Tong (Natural Copper) aids bone setting but is drying; use cautiously. Avoid in mild injuries. Wumingyi (Iron Oxide) powder taken with wine dissipates toxic blood in the limbs. Rushang and Moyao are essential for trauma; Jiangzhenxiang can substitute them and also substitutes Xuexie. Pain and swelling are caused by blood stasis; wash with hot herbal decoctions. For unresolved stasis and swelling, use Bian (stone pricking). Otherwise, pain occurs on cloudy days post-recovery. Avoid pears and raw/cold foods to prevent blood coagulation. Avoid beef, as consumption prolongs pain. Avoid moxibustion and wind exposure.
For ruptured intestines with eviscerated fat, manually remove the fat; reinsertion is harmful. For fractured bones with connected tendons, cut the tendons promptly to prevent rotting healthy flesh. For eviscerated intestines, have the patient rest their hand on the physician's shoulder, guiding the intestine back. Wash the intestine clean with barley porridge soup. Apply sesame oil to hands and wound, blow Tongguan San (Pass-the-Gate Powder) into the nose to induce sneezing, allowing the intestine to return. Use mulberry bark thread coated with chicken blood for suturing the inner layer, leaving the outer layer open, applying chicken blood and Shengji medicines. If the intestine is damaged and gas escapes when lit by lamp, it is incurable.
For scrotal rupture, apply Chicken Egg Yellow Oil, sprinkle Jinmao Gouji Mao (Dog Fern Root Hair), and wash with Perilla Leaf decoction. Internally take pain-relieving medicines; use blood-breaking medicines for stasis.
For loose teeth, rub with burned Tribulus Root until carbonized; long-term use stabilizes them.
For eyelid trauma with cyanosis, swelling, and pain, use Zijin Pi soaked in Child Urine for seven days, dried. For other injuries, no preparation needed; mix equally with Shengdi, powder, and apply with tea water.
For lumbar injury pain unresponsive to other medicines, mix Baishao, Chishao, Weilingxian, Rushang, and Moyao equally, powder, and take with wine for relief.
