Washing has the function of cleansing and scouring. It unblocks the flow of Qi and Blood, thereby relieving toxicity. This facilitates the rupture and dispersion of the lesion without stagnation. For swelling in the four limbs, soak (Zi) the area; for swelling in the waist, abdomen, and back, pour (Lin) over the area; for swelling in the lower body, bathe (Yu). For example, use two liang of herbs with two sheng of water, decoct until one and a half sheng remains. Dip cloth or cotton into the solution to wash the affected area, replacing it when slightly cooled. Use three to five times daily; for severe cases, wash continuously day and night. Continue until the swelling subsides and pain ceases. If the ulcer has already ruptured or after cupping removal, first remove old medications. Place a square pan against the body under the wound to secure it. Then, use Pig Trotter Soup (Zhuti Tang) to pour over the wound with soft silk, irrigating into any sinuses. Gently press to clear internal pus, allowing necrotic tissue and old purulent matter to exit with the soup, continuing until clean. Fold soft cloth seven or eight layers, ensuring it is not too dry, and cover the wound while retaining moisture. Press gently with both hands for a moment, replacing the cloth when it warms. Repeat this washing and pressing four to five times to promote Qi and Blood circulation, resolve toxicity, relieve pain, remove stasis, and slough off necrosis. This manual technique is essential and indispensable for large ulcers. In winter, use strong fire to drive out cold qi during washing; in summer, ensure bright ventilation to guard against wind and cold. Neglecting these precautions may, at best, hinder wound closure, or at worst, lead to a Pure Yin syndrome (a critical condition characterized by extreme Yang deficiency).
Pig Trotter Soup (Zhuti Tang) is used because the fishy smell accelerates the putrefaction of toxins. If necrotic tissue is not yet clean, apply Xuanzhu Ointment (Xuanzhu Gao); if clean, apply Yuhong Ointment (Yuhong Gao). Cover both with Black Paste (Hei Gao). If the induration at the margins has resolved, encircling agents (Gu Yao) are unnecessary. With appropriate tonic therapy, the therapeutic effect will be even greater.
If an ulcer is untreated for a month, worms may develop. Wash with Mugwort Decoction (Ai Ye Tang) added with three qian of Alum (Bai Fan), as Mugwort (Ai Ye) possesses insecticidal properties.
For ulcers that fail to close due to cold Qi and Blood, wash with Mugwort Decoction (Ai Ye Tang) and fumigate with burning Mugwort, as Mugwort is acrid and hot, assisting Yang Qi.
Before rupture and after incision/drainage, wash with Scallion and Mugwort Soup (Cong Ai Tang).
Wenge Powder (Wenge San): Treats painful swelling (Zhongyang). Whether ruptured or not, washing reduces swelling and alleviates pain.
Wenge Shell (Wen Ge) (3-5 liang, crushed, insects removed) Scallion White (Cong Bai) (18 stalks)
Decoct in water and pour/wash over the area.
Pig Trotter Soup (Zhuti Tang): Used when ulcers have ruptured and are discharging pus. It disinfects, removes foul flesh, revitalizes dead muscle, moistens the wound margin, disperses wind, and reduces swelling. Continue until all necrotic tissue is eliminated.
Dang Gui (Angelicae Sinensis Radix) Baizhi (Angelicae Dahuricae Radix) Qianghuo (Notopterygii Rhizoma et Radix) Gancao (Glycyrrhizae Radix et Rhizoma) Chishao (Paeoniae Radix Rubra) Huangqin (Scutellariae Radix) Fengfang (Vespa) (Equal parts)
Grind into coarse powder. First, boil one front pig trotter in six bowls of water until soft. Remove surface oil and cloudy sediment from the bottom. Take the clear broth. Add one liang of the herbal mixture, boil several times until boiling. Filter through silk to remove dregs. When warm, wash with soft silk.
For mild cases of Back Carbuncle (Beiju), this formula can replace the previous soup.
Dang Gui (Angelicae Sinensis Radix) Baizhi (Angelicae Dahuricae Radix) Duhuo (Angelicae Pubescentis Radix) Gancao (Glycyrrhizae Radix et Rhizoma) (2 qian each) Congtou (Scallion Head) (5 pieces)
Decoct in two bowls of water for use.
1. For Yin Ulcer (Yinchuang) presenting as black, depressed, and painless: Use one jin of Mugwort Floss (Aijiu), five qian of Realgar (Xionghuang), and five qian of Sulfur (Liuhuang). Grind into powder. Boil with Mugwort water for half a day. When water is nearly dry, remove the Mugwort, pound it into a paste, and apply warmly to the affected area. Re-boil and re-apply. After more than ten applications, if pain is felt, there is hope for recovery; if purple blood flows without pain, the prognosis is fatal. However, internal administration of major tonics to restore Yang is necessary to assist success.
2. For ulcers with putrid and foul odor: Wash with a decoction of two liang of Cnidium Fruit (Shechuangzi) and one liang of Mirabilite (Pixiao).
3. Use four liang of Alum (Bai Fan) and one liang of Realgar (Xionghuang) ground into powder. Rinse with a sufficient amount of boiling water.
Junchi Lu: Used for ulcers where necrotic tissue has been completely cleared and new granulation tissue appears like pearls. Wash with this to promote tissue regeneration.
Ruxiang (Olibanum) Moyao (Myrrha) Ercha (Catechu) Qingfen (Calomelas) (Equal parts)
Grind into powder. Use three to five qian per dose. Decoct in water until yellow and wash. Using Pig Trotter Soup (Zhuti Tang) for decoction and washing is particularly effective.
