Sanli Fa (Abscess at the Sanli Region) presents with swelling in the shape of an ox's eye, located below the knee eye, characterized by coldness and congealed pain. This condition is caused by injury to the sinews due to labor combined with Stomach Heat. The swelling appears blue-black or purple, and upon ulceration, it discharges purple blood and pus.
[Note] This syndrome occurs three cun below the knee eye, between the two tendons on the anterior aspect of the lateral side. Initially, the swelling resembles an ox's eye, accompanied by stiffness, urgency, and cold pain. It arises from injury to the sinews due to labor and the congealing of Stomach Heat. As the swelling and pain gradually increase, the color turns blue-black; upon ulceration, purple blood is discharged first, followed by thin pus. Both internal and external treatment methods follow those for abscesses and ulcers in the corresponding chapters.
Pi Fa (Abscess at the Pi Region) occurs in the calf muscle, presenting with aversion to cold, irritability, and accumulation of heat. Swelling with pain that ulcerates and discharges pus and blood is auspicious; diffuse swelling that is flat and discharges clear fluid is ominous.
[Note] This syndrome occurs in the Pi region, which refers to the calf muscle. It is caused by insufficient Kidney Water and the congealing of accumulated Heat in the Bladder. Ancient texts consider it incurable. If the area is red, high, swollen, painful, and upon ulceration discharges normal pus mixed with blood, it is auspicious and represents a favorable progression (Shun). If the swelling is diffuse and flat, dark purple, painful, and upon ulceration discharges clear fluid, it is ominous and represents an unfavorable progression (Ni). In the initial stage, administer Xianfang Huoming Yin (Immortal Life Elixir Decoction); after ulceration, administer Bazhen Tang (Eight-Treasure Decoction). For patients with Qi and Blood deficiency, administer Shiquan Dabu Tang (Ten Great Tonifying Decoction); for deficiency in the Lower Jiao, tonify with Guifu Dihuang Wan (Cinnamon Twig and Aconite Pill Rehmanniae). External treatment methods are the same as those for abscesses and ulcers.
Huangqiu Yong (Yellow Loach Carbuncle) occurs on the side of the calf, characterized by pain, induration, swelling, and a length resembling a loach. It is caused by Damp-Heat in the Liver and Spleen, appearing slightly red. Discharging thick pus upon ulceration indicates a favorable course (Shun), while discharging putrid serous fluid indicates an unfavorable course (Ni).
[Note] This syndrome occurs on the medial side of the calf, characterized by pain, induration, and swelling, several inches in length, resembling a loach, with a slightly red color. It is formed by the congealing of Damp-Heat in the Liver and Spleen meridians. Ulceration occurring at the appropriate time with the discharge of thick pus is considered favorable (Shun); discharge of turbid water or putrid serous fluid is considered unfavorable (Ni). In the initial stage, administer Wuxiang Liuqi Yin (Five-Aromatic Qi-Flowing Decoction). Subsequent internal and external treatment methods follow those for abscesses and ulcers in the corresponding chapters.
