The clinical manifestations of tinea (Xuan) mainly include six types, with root causes generally attributed to Wind-Heat, Dampness, and Parasites (Chong). These specifically include Dry Tinea, Wet Tinea, Wind Tinea, Psoriasis-like Tinea (Niu Pi Xuan), Pine Bark Tinea (Song Pi Xuan), and Knife-shaped Tinea (Dao Xuan). Among them, the facial tinea occurring in spring is often referred to as Peach Blossom Tinea.
[Note] This condition is generally caused by Wind-Heat-Damp pathogens invading the skin. After prolonged stagnation, Wind becomes excessive and transforms into "Parasites" (referring to pathogenic microorganisms or pathological products), resulting in incessant itching. There are six specific names: First, Dry Tinea, where scratching produces white scales and the skin appears dry and withered; Second, Wet Tinea, where scratching exudes sticky fluid that spreads and resembles worm tracks; Third, Wind Tinea, which refers to chronic, stubborn tinea unhealed for years, characterized by numbness and insensitivity to pain or itch upon scratching; Fourth, Niu Pi Xuan (Psoriasis-like Tinea), where lesions resemble the thick and hard skin of a cow's neck; Fifth, Song Pi Xuan (Pine Bark Tinea), where lesions resemble the bark of an old pine tree, with connected red and white spots and frequent itching; Sixth, Dao Xuan (Knife-shaped Tinea), where lesion contours are indistinct and distribution is irregular without fixed shape. The general treatment principle involves applying herbs that kill parasites, drain dampness, and detoxify externally. For mild cases, Yangti Gen San (Shepherd's Purse Root Powder) is used; for chronic and stubborn cases, Bixiao San (Surely Effective Powder) is applied topically. In some cases where Spleen and Lung Wind-Dampness are excessive, leading to swelling and pain, Sanfeng Kushen Wan (Wind-Dispersing Sophora Pill) is suitable to disperse Wind-Dampness, thereby resolving swelling and pain. Additionally, there is Wind Tinea on the face, initially appearing millet-sized or gradually forming small sores, accompanied by intermittent pain and itching, occurring in spring. It is also called Chuihua Xuan (Blowing Flower Tinea), commonly known as Peach Blossom Tinea, and is more common in women. This condition arises from Wind-Heat in the Lungs and Stomach rising with Yang Qi. It is suitable to take Shufeng Qingre Yin (Wind-Dispersing Heat-Clearing Decoction) internally and apply Xiaofeng Yurong San (Wind-Dispersing Jade-Face Powder) externally, washing daily for effective results.
