**Pterygium (Nourou)**
Pterygium arises from Heart Fire. Pterygium caused by Excess Fire presents with a deep red color, whereas that caused by Deficient Fire appears as pale red filaments. Patients often experience distending pain or pain in the eyes.
[Note] This condition occurs at the inner canthi of both eyes, where stagnant tissue protrudes and causes intermittent pain. It is fundamentally caused by Heart Fire. However, Fire can be either Excess or Deficient. If the pterygium is deep red, it indicates Excess Fire in the Heart Meridian, and Heishen Tang (Black Ginseng Decoction) is indicated. If the pterygium appears as pale red filaments, it indicates Deficient Fire in the Heart Meridian, and Jueming San (Cassia Seed Powder) is the primary treatment. Externally, Qingliang Wan (Cooling Pill) may be used for soaking and washing; with prolonged use, it will heal spontaneously.
**Heishen Tang (Black Ginseng Decoction)**
* *Shenqu* (Radix Adenophorae seu Codonopsis, Black Ginseng), *Kushen* (Cortex Sophorae Flavescentis), *Zhizi* (Fructus Gardeniae, crushed), *Juhua* (Flos Chrysanthemi), *Huanglian* (Rhizoma Coptidis), *Zhiqiao* (Fructus Aurantii, stir-fried with bran), *Caojue ming* (Semen Cassiae), *Cheqianzi* (Semen Plantaginis), *Fangfeng* (Radix Saposhnikoviae), *Dahuang* (Radix et Rhizoma Rhei, stir-fried), *Shengma* (Rhizoma Cimicifugae) (each 2 qian).
Boil in water and take after meals.
[Prescription Song] Heishen Tang treats painful large pterygium, formed internally by Excess Fire; it contains Kushen, Zhizi, Juhua, Huanglian, and Zhiqiao, along with Caojueming, Cheqianzi, Fangfeng, Dahuang, and Shengma.
**Jueming San (Cassia Seed Powder)**
* *Yuzhu* (Rhizoma Polygonati Odorati), *Huanglian* (Rhizoma Coptidis), *Zhiqiao* (Fructus Aurantii, stir-fried with bran), *Chuanxiong* (Rhizoma Chuanxiong), *Gancao* (Radix Glycyrrhizae, raw), *Lingyangjiao* (Cornu Saigae Tataricae, shaved) (each 1 liang); *Cheqianzi* (Semen Plantaginis), *Qingxiangzi* (Semen Celosiae), *Caojueming* (Semen Cassiae) (each 5 qian).
Grind into fine powder. Take 3 qian per dose. Take after meals, and take another dose before sleep.
[Prescription Song] Jueming San treats pterygium attacked by Deficient Fire; it contains Yuzhu, Huanglian, Zhiqiao, and Chuanxiong, as well as Cheqianzi, Qingxiangzi, Lingyangjiao, and Gancao. Taking it as a powder mixed with water yields great efficacy.
**Qingliang Wan (Cooling Pill)** (See section on Bacterial Toxins)
**Nasal Carbuncle (Biju)**
Nasal carbuncle arises between the nasal bridge, originating from the Du Meridian and triggered by Stagnant Fire in the Lung Meridian. The lesion is hard, purple in color, and often accompanied by numbness and pain. *Qianjin* Formula's Loulu Tang and Xianfang Huoming Yin can help expel toxins and promote healing.
[Note] This condition occurs at the nasal bridge and belongs to the Du Meridian. Since the nose is the orifice of the Lung, it is also related to the Lung. It is formed by the coagulation of Stagnant Fire in the Lung Meridian. Clinical manifestations include hardness, purple discoloration, and intermittent numbness and pain. Initially, *Qianjin* Loulu Tang should be taken to disperse and resolve toxic stagnation; subsequently, Xianfang Huoming Yin (Immortal Life Elixir) added with Zhizi, Mutong, Bohe, and Jiegeng should be used to dissipate the swelling. If pain and swelling do not subside and suppuration is imminent, Tuoli Tou Nong Tang (Interior-Supporting and Pus-Expelling Decoction) is indicated. External treatments follow the protocols for Ulcerative Sores and Carbuncles.
***Qianjin* Loulu Tang (Loulu Decoction from Qianjin Fang)**
* *Loulu* (Radix Euphorbiae Humifusae) (1 liang), *Zhiqiao* (Fructus Aurantii, stir-fried with bran) (1 liang), *Puxiao* (Natrii Sulfas) (1 liang), *Dahuang* (Radix et Rhizoma Rhei) (1.5 liang), *Gancao* (Radix Glycyrrhizae, raw) (1 liang), *Mahuang* (Herba Ephedrae) (1 liang), *Huangqin* (Radix Scutellariae) (1 liang), *Bailian* (Tuber Ampelopsis Japonicae) (1 liang), *Lianqiao* (Fructus Forsythiae, heart removed) (1 liang), *Shengma* (Rhizoma Cimicifugae) (1 liang).
Grind into powder. Use 2 qian per dose with one cup of water, three slices of fresh ginger, and one qian of thin lotus leaf. Decoct until reduced to half volume. Take warm. Stop when bowel movement is unobstructed.
[Prescription Song] *Qianjin* Loulu Tang treats the onset of nasal carbuncle; its efficacy is superior for cases with purple, hard, and painful lesions. It contains Loulu, Zhiqiao, Puxiao, Dahuang, and Gancao, along with Mahuang, Huangqin, Bailian, Lianqiao, and Shengma.
