Sheyan Ding (Ding [carbuncle] appearing at the nail edge) arises beside the nail plate; those arising behind the nail root are named Shebei Ding (Ding [carbuncle] on the dorsal nail fold). Zhujie Ding (Ding [carbuncle] of the joint) occurs at the middle phalanx. Shefu Ding (Ding [carbuncle] of the finger pulp) presents with swelling in the finger resembling a fish belly. Niqiu Ju (Ju [abscess] resembling a loach) manifests as diffuse swelling of the entire finger, with heat and pain radiating to the elbow and arm. One must observe which finger is affected to determine the involved meridian; generally, these conditions arise from Fire-Toxin accumulated in the Zang-Fu organs.
[Note] There are five types of this condition:
1. Sheyan Ding arises on both sides of the nail plate, shaped like a bean, purple in color, half-exposed, and hard like an iron nail.
2. Shebei Ding arises behind the nail root, shaped like half a jujube, red and swollen.
3. Zhujie Ding, also known as Shejie Ding, arises at the middle phalanx, with circumferential swelling of the finger; the color may be yellow or purple.
4. Shefu Ding, also known as Yudu Ding, arises on the anterior aspect of the middle phalanx, swelling like a fish belly, red and painful.
5. Niqiu Ju involves diffuse swelling of one finger, purple in color, shaped like a loach, with heat and pain radiating to the elbow and arm.
In the early stage, it is appropriate to administer Chansu Wan (Toad Venom Pill) to induce sweating, apply Xionghuang San (Realgar Powder) externally, and subsequently take Xianfang Huoming Yin (Immortal Life Decoction). Once pus has matured, incise and drain, then apply Hupo Gao (Amber Plaster) to promote pus formation and tissue regeneration. For patients with deficiency who cannot heal, tonification therapy should be used. These five conditions all stem from the coagulation of Fire-Toxin. Regarding which meridian or Zang-Fu organ is involved, diagnosis should be made clinically based on the specific finger affected.
Daizhi (Subungual abscess leading to nail shedding) typically occurs within the nail bed. Initially, there is swelling, heat, and pain that seems to affect the heart. Mild cases may ulcerate with slight pus discharge, while severe cases result in nail loss. The etiology is Heat in the Blood vessels and meridians.
[Note] This condition arises inside the fingernail due to the coagulation of Heat in the Blood vessels and meridians. In the initial stage, there is swelling and heat, with pain radiating to the heart region. It is advisable to boil Gancao (Radix Glycyrrhizae) and Puxiao (Natrii Sulfas), each 5 qian, in water for soaking and washing, which will lead to recovery. If pain persists, after three to four days, a small yellowish-white spot may appear on the dorsal surface of the nail, indicating that internal pus has formed but lacks an outlet. Urgently use a thread needle to prick a small hole near the pus on the nail plate to allow drainage. Squeeze out any remaining pus, then apply Huanglian Gao (Coptis Ointment) for easy healing. Alternatively, if treatment is delayed or excessive cooling herbs are applied, causing Cold coagulation in the muscles and infiltration of pus-toxin into healthy tissue, the nail may become hollowed out and inevitably shed. Applying Hupo Gao (Amber Plaster) will lead to recovery within one to two months.
