Shetou Ding (Snakehead Carbuncle) presents with purple discoloration, induration, and pain; Tianshe Du (Heavenly Snake Poison) presents with severe pain accompanied by a sensation of stifling distension, along with redness and swelling. Both conditions are associated with Fire-Heat in the Spleen Meridian, and the specific meridian involved can be determined by identifying which finger is affected.
[Note] These two conditions both occur at the tips of the fingers. Although each finger has its own dedicated meridian, they all involve the accumulation of Fire-Toxin from the Spleen Meridian. Shetou Ding originates from deep within the sinews and bones, characterized by a deep root and heavy toxicity. Initially, the lesion is small, purple in color, painful, and hard as a nail. In the early stage, it is appropriate to administer Chansu Wan (Toad Venom Pill) to induce sweating, and apply Xionghuang San (Realgar Powder) externally. Tianshe Du originates from the muscle layer, with slightly less severe toxicity. Initially, it presents as stifling swelling without a distinct head, red in color, with burning pain. In the early stage, it is appropriate to administer Chansu Wan (Toad Venom Pill) to induce sweating, and apply Xionghuang Muli San (Realgar and Oyster Shell Powder) externally. When pus formation is imminent for either condition, Xianfang Huoming Yin (Immortal Life Decoction) should be taken orally. Once the pus is ripe, incision and drainage should be performed, followed by the application of Hupo Gao (Amber Plaster) to promote pus discharge and tissue regeneration. If the patient's deficiency prevents wound contraction, tonification therapy should be employed. However, the fingers are areas where skin and flesh are thin; moxibustion is contraindicated, and premature incision should be avoided. If moxibustion is mistakenly applied or incision is performed too early, causing skin rupture and evisceration of flesh, resulting in doubled pain, rapid healing will not be possible. Caution is advised.
The songs describing the meridians of the fingers are found in the first volume.
Xionghuang San (Realgar Powder)
Ingredients: Ming Xionghuang (Realgar, 2 qian), Qingfen (Calomel, 5 fen), Chansu (Toad Venom, 2 fen), Bingpian (Borneol, 1 fen).
Preparation and Administration: Grind all ingredients into a fine powder. Mix with fresh cold water to form a thick paste. Warm the mixture in a double boiler. Apply to the affected finger, cover with thin paper, and change 3–4 times daily.
[Prescription Song] Xionghuang San treats Shetou Ding, with purple pain and a hard root due to attacking Fire-Toxin; Borneol, Toad Venom, and Calomel ground into powder, mixed with drawn water and applied, sealed with paper.
Xionghuang Muli San (Realgar and Oyster Shell Powder)
Ingredients: Muli (Oyster Shell, calcined, 4 qian), Ming Xionghuang (Realgar, 2 qian).
Preparation and Administration: Grind each ingredient separately into a fine powder, then mix and grind uniformly. Mix with honey water to form a thick paste. Warm in a double boiler. Apply to the affected finger to relieve pain. Use 5–6 times daily.
[Prescription Song] Xionghuang and Muli treat Tianshe Du, for red, stifling, swollen, and painful fingertips; these two ingredients ground finely with honey water, applied topically to stop pain with effective and miraculous results.
