The occurrence of sores and ulcers is due to excessive consumption of rich and greasy foods, strong wine, and roasted or fried items; excessive sexual activity; emotional stagnation transforming into fire; yin deficiency with yang hyperactivity; essence deficiency and qi depletion; decline of the Fire of the Gate of Vitality (Mingmen) failing to warm Spleen Earth; and weak Ying and Wei (Defensive and Nutritive) qi allowing external pathogens to invade, resulting in damage to Qi and Blood. One must examine the meridians affected by the syndrome and treat according to the principles of addressing the root and branch, prioritizing urgency and slackness. If the disease is acute but the Yuan Qi (Original Qi) is substantial, treat the branch first; if the disease is chronic but the Yuan Qi is deficient, treat the root first; or if the disease is acute yet the Yuan Qi is also deficient, one must first treat the root while simultaneously addressing the branch. Generally speaking, if the swelling is high and painful, and the pus is thick and sticky, the Yuan Qi remains undamaged, making treatment easy. If the swelling is diffuse without a head and slightly painful, and the pus is clear and thin, the Yuan Qi is deficient, making treatment difficult. If there is no swelling or pain, or if the swelling is diffuse, dark, and does not ulcerate, the Yuan Qi is extremely deficient, which is the most difficult to treat. The main therapeutic methods are as follows: For high swelling and pain, first use Xianfang Huoming Yin (Immortal Life Elixir) to resolve toxicity, followed by Tuoli Xiaodu San (Support-Interior and Detoxify Powder). For diffuse swelling with slight pain, use Tuoli San (Support-Interior Powder); if ineffective, add Shengjiang (Zingiberis Rhizoma Recens, Fresh Ginger) and Rougui (Cinnamomi Cortex, Cinnamon Bark). If pain persists after pus discharge, it indicates Qi and Blood deficiency; use Bazhen Tang (Eight-Treasure Decoction). If pus does not form and tissue does not slough, it indicates Yang Qi deficiency; use Sijunzi Tang (Four Gentlemen Decoction) plus Danggui (Angelicae Sinensis Radix, Dong Quai), Huangqi (Astragali Radix, Astragalus), and Rougui (Cinnamomi Cortex, Cinnamon Bark). If new flesh does not grow and wounds do not close, it indicates Spleen Qi deficiency; use Sijunzi Tang plus Shaoyao (Paeoniae Radix Alba, White Peony Root) and Muxiang (Aucklandiae Radix, Aucklandia). If aversion to cold and shivering occur, it indicates Yang Qi deficiency; use Shiquan Dabu Tang (Ten Complete Tonifying Decoction) plus Shengjiang (Zingiberis Rhizoma Recens, Fresh Ginger) and Rougui (Cinnamomi Cortex, Cinnamon Bark). If afternoon fever or internal heat occurs, it indicates Yin Blood deficiency; use Siwu Tang (Four Substances Decoction) plus Renshen (Ginseng Radix, Ginseng) and Baizhu (Atractylodis Macrocephalae Rhizoma, Atractylodes). If nausea or vomiting occurs, it indicates Stomach Qi deficiency; use Liujunzi Tang (Six Gentlemen Decoction) plus Paojiang (Zingiberis Rhizoma Preparatum, Prepared Ginger). If spontaneous sweating or night sweats occur, it indicates deficiency of the Five Zang Organs; use Liuwei Dihuang Wan Formula plus Wuweizi (Schisandrae Chinensis Fructus, Schisandra). If food intake is poor and the body feels tired, it indicates Spleen Qi deficiency; use Buzhong Yiqi Tang (Tonify the Middle and Augment Qi Decoction) plus Fuling (Poria, Poria cocos) and Banxia (Pinelliae Rhizoma, Pinellia). If panting, rapid breathing, and coughing occur, it indicates Spleen and Lung deficiency; use the aforementioned formula plus Maidong (Ophiopogonis Radix, Ophiopogon) and Wuweizi (Schisandrae Chinensis Fructus, Schisandra). If nausea and poor appetite coexist, it indicates Spleen and Stomach deficiency; use Renshen Lizhong Tang (Ginseng Regulate the Middle Decoction). If abdominal pain and diarrhea occur, it indicates Spleen and Stomach deficiency-cold; use Fuzi Lizhong Tang (Aconite Regulate the Middle Decoction). If lower abdominal fullness and edema of the lower legs occur, it indicates Spleen and Kidney deficiency; use Shiquan Dabu Tang plus Shanzhuyu (Corni Fructus, Cornus), Shanyao (Dioscoreae Rhizoma, Yam), and Rougui (Cinnamomi Cortex, Cinnamon Bark). If diarrhea and cold feet occur, it indicates Spleen and Kidney deficiency-cold; use the aforementioned formula plus Rougui (Cinnamomi Cortex, Cinnamon Bark) and Fuzi (Aconiti Lateralis Radix Praeparata, Prepared Aconite Root). If heat, thirst, dysuria, and urinary retention occur, it indicates Kidney deficiency with Yin fire; use Jiajian Bawei Wan (Modified Eight-Ingredient Pill). If panting, coughing, dysuria, and urinary retention occur, it indicates Lung and Kidney deficiency with false fire; use Buzhong Yiqi Tang and Jiajian Bawei Wan. In general, for frail individuals, regardless of whether swelling or ulceration is present, one should first tonify Stomach Qi. Some suspect that Renshen (Ginseng Radix, Ginseng) causes stagnation in the middle jiao; occasionally, practitioners use it but also add dispersing and detoxifying herbs, such that the benefit does not compensate for the harm. Others, adhering rigidly to the notion that their constitution is inherently robust or that they have phlegm, refuse tonifying agents, often leading to misdiagnosis and mistreatment. It is unknown that sores and ulcers arise from the depletion of Yin and Yang; once pus has drained, Qi and Blood become even more deficient. How can tonification be inappropriate? Thus, Master Danxi stated: Whenever swelling and pain are observed, if pulse and symptoms indicate deficiency, provide tonification immediately. As long as Qi and Blood are not depleted, auspicious outcomes can be ensured until the end.
