If the affected area presents with swelling, pain, or fever, it is due to damp-heat in the Liver channel. The treatment should clear the Liver and eliminate dampness.
If swelling and pain are accompanied by aversion to cold and fever, it indicates that pathogenic factors have injured the exterior. The treatment should employ dispersing methods.
If swelling and pain are accompanied by red, scanty, and painful urination, it indicates stagnation of damp-heat in the Liver channel. The treatment should soothe the Liver and guide out dampness.
A patient suffered from this condition with local swelling and hardness, pain, and alternating chills and fever. Initially, two doses of Renshen Baidu San (Ginseng Detoxification Powder) were administered to stop the symptoms, followed by Xiaochaihu Tang (Minor Bupleurum Decoction) added with Huanglian (Coptidis Rhizoma) and Qingpi (Aurantii Fructus Pericarpium), resulting in a cure. (This treatment was based on syndrome differentiation according to symptoms and meridians.)
Another patient had ulceration but still experienced swelling, pain, and red, scanty, painful urination. Jiajian Longdan Xiegan Tang (Modified Gentiana Liver-Draining Decoction) added with Qingpi and Huanglian was given for two doses, leading to slight improvement. Subsequently, Xiaochaihu Tang added with Huangbai (Phellodendri Cortex), Zhimu (Anemarrhenae Rhizoma), Danggui (Angelicae Sinensis Radix), and Fuling (Poria) was administered for several doses, resulting in a cure. (This treatment was based on syndrome differentiation according to symptoms and meridians.)
One patient had persistent penile swelling; another had ulceration with swelling, pain, fever, and short, scanty, painful urination, with fever in the afternoon (Shi hour); and a child suffered from swelling and pain unresponsive to various medications. All three were cured after taking several doses of Xiaochaihu Tang (Minor Bupleurum Decoction) taken with Lushui Wan (Aloe Pill).
One patient experienced intermittent penile swelling, pain, or flaccidity with inability to retract. Another patient suffered from pain in the urethra, tendon tension with retraction, itching, and discharge of white fluid resembling semen during urination. This is classified as Jin Shan (Tendon Hernia). Both cases were cured using Longdan Xiegan Tang (Gentiana Liver-Draining Decoction). (This treatment was based on the specific symptoms.)
Zhang Zihe stated: Nocturnal emission, long bi (difficulty in urination), impotence, bladder obstruction, seminal leakage, and leukorrhea are all hernias in men and should not be casually attributed to Kidney cold. In women, amenorrhea, cessation of menstruation, heat in the upper waist and knees, heat in the feet, dry throat, difficulty in urination, masses in the lower abdomen that may be fixed or mobile, protrusion of the anterior yin, and hemorrhoidal nodules in the posterior yin are all considered hernias in women. However, these conditions in women are not called hernias but are termed Jia (masses).
One patient had ulceration with swelling, pain, and fever, which was particularly severe in the afternoon. The case was cured using Xiaochaihu Tang (Minor Bupleurum Decoction) added with Huangbai, Zhimu, and Danggui. (This treatment was based on the symptoms.)
One patient had recovered, except for a hard nodule in the penis that did not dissipate. It was prevented and treated with Xiaochaihu Tang (Minor Bupleurum Decoction) added with Qingpi and Jingjie (Schizonepetae Herba). Additionally, a decoction of Jingjie, Fangfeng (Saposhnikoviae Radix), Niuxi (Achyranthis Bidentatae Radix), Heshouwu (Polygoni Multiflori Radix), Shigao (Gypsum Fibrosum), and Gancao (Glycyrrhizae Radix et Rhizoma), each five qian, was used for fumigation and washing. After several doses, the nodule dissipated. (This treatment was based on the symptoms.)
A patient over forty years old, who typically suffered from lin syndrome, developed a chancre with pain and fatigue. The case was cured using Xiaochaihu Tang (Minor Bupleurum Decoction) added with Huanglian, Huangbai, Qingpi, and Danggui. (This treatment was based on the symptoms.)
One patient, due to overwork, experienced itching at the urethral orifice, intermittent discharge of white fluid, fever, and dry mouth. The condition was stabilized using Qingxin Lianzi Yin (Clear Heart Lotus Seed Decoction). (This treatment addressed the site of labor-induced injury.)
One patient had penile swelling and pain, urinary symptoms resembling lin syndrome, severe spontaneous sweating, occasional slight hematuria, and a chi pulse that was surging and slippery but became choppy upon deep pressure. Initially, Qingxin Lianzi Yin (Clear Heart Lotus Seed Decoction) added with Niuxi, Shanzhi (Gardeniae Fructus), Huangbai, and Zhimu was administered for several doses, leading to slight improvement. Subsequently, Zishen Wan (Kidney-Nourishing Pill) one dose resulted in recovery. (This treatment was based on the symptoms.)
Former sages stated: If there is spontaneous sweating and scanty urine, diuretics should not be used forcibly. Since spontaneous sweating has already occurred, body fluids are lost externally, and urine naturally becomes scanty. If diuretics are used, defensive qi and fluids will be exhausted, failing to control deficiency fire, leading to increased irritability and heat. One should wait for the heat to subside and sweating to cease, after which urination will normalize naturally. Moreover, this belongs to the Yangming channel, where the use of diuretics is strictly contraindicated.
An elderly patient suffered from a chancre with dribbling urine, a fine pulse, and physical fatigue, indicating Qi deficiency combined with damp-heat. The condition was cured using Qingxin Lianzi Yin (Clear Heart Lotus Seed Decoction) and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
For chancre, Danxi used Qingdai (Indigo Naturalis), Gafen (Concha Meretricis), Miduoseng (Lithargyrum), and Huanglian, ground into powder, applied externally. He also burned Ji Dun Pi (Gallus gallus domesticus gizzard lining) until preserved in carbonized state, ground it into powder, and applied it externally.
For chancre and accompanying sores: Gafen, Latiao Cha (Wax Tea), Kushen (Sophorae Flavescentis Radix), and Miduoseng were ground into powder. The affected area was washed clean with river water, then applied with rendered lard from a sow's ear.
Another prescription: Wash the sore clean with rice water. Wash hair with salt water to remove oil, then rinse with clear water, dry, burn to ash, and apply to the sore to promote healing and scabbing.
Another prescription: For lower-jet chancre causing erosion, foul odor, rotting, and unbearable pain. Also treats pediatric chancre.
Ingredients: Huangbai (Honey-fried), Huangdan (Minium), Qingfen (Calomelas), Ruxiang (Olibanum), Miduoseng, Gaomo Cha (High-end Tea), each 3 fen; Muxiang (Musk), a small amount.
Preparation: Grind all ingredients into fine powder. Wash the sore with green onion soup, then apply this medicine.
Washing Medicine: Huanglian, Huangbai, Danggui, Baizhi (Angelicae Dahuricae Radix), Duhuo (Angelicae Pubescentis Radix), Fangfeng, Jingjie, equal parts. Decoct in water, add fifty cash coins, five Wumei (Chinese Plum), and one spoonful of salt. Decoct together. Wash warm, five to seven times daily. Apply the following dressing after washing.
Dressing Medicine: Muxiang, Binglang (Arecae Semen), Huanglian, Tongqing (Verdigris), Qingfen, Kufan (Alumen Exsiccatum), Haipiaoxiao (Sepiae Endoconcha), Muxiang.
