A patient with weak stomach qi and abundant phlegm developed sores on the mouth and tongue. After taking Gun Tan Wan (Rolling Phlegm Pill), the condition worsened, accompanied by incessant vomiting and diarrhea, aversion to food, and fatigue. This indicated injury to the stomach qi. I prescribed Xiang Sha Liu Jun Zi Tang (Aromatic Sand Six Gentlemen Decoction) for several doses, which brought slight relief. Subsequently, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) was administered with added Fu Ling (Poria) and Ban Xia (Pinellia Rhizome) for over twenty doses, resulting in a cure.
Insufficient stomach qi leads to improper digestion of food, which can also generate phlegm. Therefore, tonifying the middle and augmenting qi is a method for treating such phlegm. If峻利之药 (harsh purgative/draining drugs) are used for deficiency syndromes, danger is rarely avoided.
Another patient, over forty years old, had a robust appearance but weak constitution. He experienced chills when exposed to wind and developed sores on the mouth and tongue upon exertion. He often had phlegm in the chest and red, dry eyes. He was cured after taking Ba Wei Wan (Eight-Ingredient Pill). (This case relied on symptom differentiation.)
A third patient with weak spleen and stomach initially responded well to Yang Wei Tang (Nourish Stomach Decoction) and Zhi Zhu Wan (Immature Bitter Orange and Atractylodes Macrocephala Pill). However, long-term use led to further deficiency, sores on the mouth and tongue that worsened with exertion, and vomiting upon taking bitter-cold detoxifying drugs. This was due to deficiency-cold in the middle jiao.
Treatment began with Li Zhong Tang (Regulate the Middle Decoction), which provided slight improvement. Then, Bu Zhong Yi Qi Tang with added Ban Xia (Pinellia Rhizome) and Fu Ling (Poria) was given, leading to stability within a month.
Yang Wei Tang consists of aromatic and drying herbs. If there is food stagnation or cold stagnation in the middle jiao, it can dry dampness, open the spleen and stomach, and digest retained food, which is somewhat reasonable. Zhi Zhu Wan is a digestive and guiding herb formula. Although it contains Bai Zhu (Atractylodes Macrocephala Rhizome), it is ultimately a drying agent. Thus, long-term use of these two formulas dries body fluids further and weakens stomach qi. Moreover, using them on a patient whose stomach qi is already deficient would inevitably aggravate the disease. There are also cases where excessive sexual activity leads to exhaustion of true yang, or innate insufficiency fails to steam upwards to the middle jiao, causing impaired transport and transformation, resulting in poor appetite. These are treated with Bu Zhen Wan (Tonify True Essence Pill) to enable the fire of the Dantian to steam the spleen earth. When the spleen earth becomes warm, the middle jiao regulates itself, and appetite returns. The Classic states: Poor appetite, chest and diaphragm fullness and obstruction, indigestion, or loose stools are all caused by the failure of true fire to steam the spleen earth. If kidney qi is strong, the fire of the Dantian steams the spleen earth, and this disease does not occur.
