For itching accompanied by pus and exudate spreading, employ the method of eliminating wind and dampness.
For itching and pain without pus, employ the method of dispelling wind and moistening dryness.
For itching or pain that worsens in the afternoon, employ the method of nourishing yin and descending fire.
For pain with constipation and difficult defecation, employ the method of nourishing yin and purging fire.
For scratching leading to white scales and tinnitus sounding like cicadas, employ the method of dispelling wind and clearing heat.
A woman suffered from this condition with itching and incessant pus discharge; her pulse was floating and powerless. She was treated with Xiaofeng San (Eliminating Wind Powder) [Formula 80] for four doses, showing slight improvement, and then cured after taking more than a month of Sisheng Wan (Four Life Pill) [Formula 86]. (This diagnosis relied on symptoms and pulse.)
A man experienced less itching but more pain, with no pus discharge. He was cured using Huangqin (Scutellariae Radix), Lianlian (Coptidis Rhizoma), Jingjie (Schizonepetae Herba), Fangfeng (Saposhnikoviae Radix), Shanzhi (Gardeniae Fructus), Bohe (Menthae Haplocalycis Herba), Shaoyao (Paeoniae Radix Alba), and Guishen (Angelicae Sinensis Radix). (This diagnosis relied on symptoms.)
A woman suffered from itching that was particularly severe in the afternoon. She showed slight improvement after several doses of Danggui Yinzi (Angelica Decoction), and was stabilized after several doses of Renshen Jingjie San (Ginseng and Schizonepeta Powder). (This diagnosis relied on pulse and symptoms.)
A man suffered from chronic non-healing lesions, scratching up white scales, and tinnitus. His itching stopped after several doses of Sisheng San (Four Life Powder), and he recovered after several doses of Danggui Yinzi (Angelica Decoction).
Lesions in one man were mostly located on the lower body, causing pain that was most severe at dusk. The tendons around his ankles were purple and swollen. Pricking the purple areas to release blood stasis, followed by Siwu Tang (Four Substances Decoction) added with Huangqin (Scutellariae Radix) and Huanglian (Coptidis Rhizoma) for four doses, brought relief. If lesions were on the upper body, such as purple and swollen tendons around the wrists, pricking to release blood was also indicated. Healing occurred with the previous formula added with Chaihu (Bupleuri Radix) and Huangqin (Scutellariae Radix). (This diagnosis relied on symptoms.)
One man developed sores from scratching, with severe pain at dusk. Pain ceased after several doses of Siwu Tang (Four Substances Decoction) added with Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Jingjie (Schizonepetae Herba), and Fangfeng (Saposhnikoviae Radix). He recovered after twenty doses of Siwu Tang (Four Substances Decoction) added with Jili (Tribuli Fructus), Heshouwu (Polygoni Multiflori Radix), and Huangqi (Astragali Radix). (This diagnosis relied on symptoms.)
One man suffered from dizziness and blurred vision, skin itching, and sores caused by scratching. He was cured using Bafeng San (Eight Winds Powder).
One man had sores and scabies mostly on both feet, with severe pain in the afternoon and purple, swollen ankle tendons. His pulse was large and forceful, indicating blood heat. Toxic blood was released by scarification at the purple sites. He recovered using Siwu Tang (Four Substances Decoction) added with Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Chaihu (Bupleuri Radix), and Digupi (Lycii Cortex). (This diagnosis relied on pulse and symptoms.) For arm sores with purple wrist tendons, scarification is also appropriate. For elderly or weak individuals suffering from painful conditions, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) added with blood-cooling herbs should be used.
A one-year-old infant first developed sores and scabies on the head, which gradually spread over the whole body and failed to heal for a long time. He was treated with Siwu Tang (Four Substances Decoction) [Formula 9] added with Fangfeng (Saposhnikoviae Radix), Huangqin (Scutellariae Radix), and Shengma (Cimicifugae Rhizoma), externally applied with Du Yao San (Poisonous Herb Powder). He recovered after more than a month. (This diagnosis relied on symptoms.)
A child’s sore toxins did not heal or recurred after healing. This was due to the mother consuming roasted, fried, spicy foods or having internal heat. The mother’s heat should be treated first. If the child cannot take medicine, Loulu (Euphorbiae Humifusae Herba) can be added to the mother’s prescription for decoction, and the child’s sores will also heal.
An infant had several itchy sores on the head and face with accumulated crusts, named Nian Chuang (Sticky Sores). Equal parts of Kufan (Alumen) and Huandan (Minium) were mixed with sesame oil for external application. He recovered after taking Baidu San (Defeat Toxin Powder). (This diagnosis relied on symptoms.)
A man had wet sores on his legs that did not heal for years. His Chi pulse seemed large when lightly palpated but powerless when heavily pressed. This was kidney deficiency attacked by wind pathogens, named Shen Feng (Kidney Wind). He was treated with Sisheng San (Four Life Powder). Disbelieving this, he self-medicated with Huangqin (Scutellariae Radix) and Huanglian (Coptidis Rhizoma), causing daily weakening of qi and blood, increased pus, and worsening physical exhaustion. Two years later, upon re-consultation, the previous medication cured him. (This diagnosis relied on pulse and symptoms.)
The limbs have upper and lower distinctions, and the zang-fu organs have deficiency and excess patterns. People generally know that bitter-cold medicines can eliminate sore toxins, but do not understand that Kidney Wind is caused by kidney insufficiency. Using Jili (Tribuli Fructus) as the chief herb, Huangqi (Astragali Radix) as the minister herb, and Baifuzi (Aconiti Kusnezoffii Radix) and Duhuo (Angelicae Pubescentis Radix) as assistant and guide herbs is appropriate. Taking Baidu San (Defeat Toxin Powder) etc. would further deplete original qi and hasten death.
One man had painful, feverish lesions with a floating and rapid pulse. He showed slight improvement after four doses of Renshen Baidu San (Ginseng Defeat Toxin Powder) [Formula 64] and recovered after several doses of Danggui Yinzi (Angelica Decoction). (This diagnosis relied on pulse and symptoms.)
One man had whole-body itching, scratching into sores with fluid discharge, and a floating, rapid pulse. This was caused by wind-heat in the Hand and Foot Yangming meridians. He was treated with Renshen Baidu San (Ginseng Defeat Toxin Powder) combined with Siwu Tang (Four Substances Decoction) [Formula 9] added with Huangqin (Scutellariae Radix) and Huanglian (Coptidis Rhizoma). Externally, Songxiang (Rosin), Kufan (Alumen), and Qingfen (Calomel) were powdered and mixed with sesame oil for application. He recovered after more than a month. (This diagnosis relied on symptoms.)
An infant had several water blisters on the head, face, chest, and abdomen that ulcerated into sores. This was wind pathogen invading the skin, named Biao Chuang (Blistering Sores). He recovered after drinking Jingfang Baidu San (Schizonepeta and Saposhnikovia Defeat Toxin Powder) [Formula 7] and applying powder made from burnt cow dung. (This diagnosis relied on symptoms.)
Biao Ju (Blistering Carbuncle) is extremely toxic. It resembles millet grains, or larger ones like chestnuts, appearing unpredictably, mostly on fingers. It ulcerates and bleeds. In severe cases, bones are exposed, or there is delirious speech and restlessness. Powdered Nanxing (Arisaematis Rhizoma), Banxia (Pinelliae Rhizoma), and Baizhi (Angelicae Dahuricae Radix) are applied.
An infant had sores under the nose, frequently rubbed, spreading to both ears, unresponsive to various medicines. He recovered after taking Lushui Wan (Aloe Pill) and applying Songxiang (Rosin) and Lvdou (Vigna mungo) powder.
One man had damp-heat pouring downward, causing sores on both legs. He took Renshen Baidu San (Ginseng Defeat Toxin Powder) added with Cangzhu (Atractylodis Rhizoma) and Huangbai (Phellodendri Cortex), and externally applied Jinhuang San (Golden Yellow Powder). (This diagnosis relied on symptoms.)
One man had painful lesions, chills, fever, constipation, and a rapid, forceful pulse. He showed slight improvement after two doses of Fangfeng Tongsheng San (Saposhnikovia Unimpeded Success Powder) and recovered after four doses of Jingfang Baidu San (Schizonepeta and Saposhnikovia Defeat Toxin Powder) [Formula 7] added with Huangqin (Scutellariae Radix) and Shanzhi (Gardeniae Fructus). (This diagnosis relied on symptoms.)
Some patients presented only with a deep and solid pulse. They gradually improved with the previous formula added with Dahuang (Rhei Radix et Rhizoma) and recovered after taking Renshen Baidu San (Ginseng Defeat Toxin Powder). (This diagnosis relied on pulse.)
A monk suffering from sores and scabies burned Xionghuang (Realgar) and Aiyè (Artemisiae Argyi Folium) in his bedding for fumigation. The next day, his whole body swelled, skin broke with fluid discharge, and he could not eat. I administered detoxifying medicine, but it was ineffective, and he died.
Another case of chronic non-healing sores was cured by applying powder made from Cuanban Hui (Ship Board Ash) preserved by burning (1 liang) and Qingfen (Calomel) (3 qian) mixed with sesame oil, along with oral administration of Zimu (Anemarrhenae Rhizoma), Huangbai (Phellodendri Cortex), Fangji (Stephaniae Tetrandrae Radix), Longdan (Gentianae Radix), Fuling (Poria), Danggui (Angelicae Sinensis Radix), Zhike (Aurantii Fructus), and Baizhu (Atractylodis Macrocephalae Rhizoma).
Generally, sores in the lower body belong to damp-heat but invariably arise from spleen and kidney deficiency.
One man had sores on both legs; every time he took Baidu San (Defeat Toxin Powder), his food lost flavor, and swelling increased. This was spleen deficiency with damp-heat pouring downward. He recovered after taking Liujunzi Tang (Six Gentlemen Decoction) added with Cangzhu (Atractylodis Rhizoma), Shengma (Cimicifugae Rhizoma), and Jiuchao Shaoyao (Wine-fried Paeoniae Radix Alba), and externally applying a plaster made of Huangla (Yellow Beeswax), Mazhongyou (Sesame Oil), and Qingfen (Calomel). (This diagnosis relied on symptoms.)
Sores on both legs causing pain, or whole-body pain, respond well to Danggui Niangong Tang (Angelica Relieving Pain Decoction).
A man over fifty had sores on both legs that did not heal for a long time. Due to irregular diet, labor, or taking diuretic and detoxifying agents, the condition worsened due to spleen qi weakness. This was spleen deficiency complicated with damp-heat. He showed slight improvement after several doses of Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) and recovered after treatment with Liujunzi Tang (Six Gentlemen Decoction) added with Cangzhu (Atractylodis Rhizoma), Shengma (Cimicifugae Rhizoma), and Shenqu (Medicated Leaven).
Generally, for sores in the lower body with pain, chills/fever, or swollen/painful Hong Kong foot (podagra), Renshen Baidu San (Ginseng Defeat Toxin Powder) added with Binglang (Arecae Semen), Zisu (Perillae Folium), Cangzhu (Atractylodis Rhizoma), and Huangbai (Phellodendri Cortex) is effective. For chronic cases, Siwu Tang (Four Substances Decoction) [Formula 9] is used. After recovery, Bushen Wan (Kidney Tonifying Pill) is taken to prevent recurrence.
Sore Formula: Xiangru (Frankincense), Moyao (Myrrh), Shuiyin (Mercury), Danggui (Angelicae Sinensis Radix) (each 0.5 liang); Chuanxiong (Ligustici Chuanxiong Rhizoma), Beimu (Fritillariae Cirrhosae Bulbus), Huangdan (Minium) (each 2.5 qian); Zhen Mazhongyou (Pure Sesame Oil) (5 liang).
Another Formula: Longgu (Fossilized Bones, raw), Xuexie (Dragon's Blood), Chishizhi (Red Halloysite) (each 1 liang); Hair (finger-thick), Huangla (Yellow Beeswax), Baijiaoxiang (White Copal Resin) (each 1 liang). Boil hair for three to five boils, remove hair, add wax and resin. Stir in Longgu, Xuexie, and Chishizhi evenly. Place in a basin of water to cool. Store in porcelain. Mold into thin slices for application on sores, covered with bamboo leaves. Replace after three days. Continue taking blood-invigorating medicine.
Another Formula: Decoct Dongqingye (Aquifolii Folium) in sugar water for three to five boils. Remove, press flat with stone, and apply to sores. Change twice daily.
Another Formula: Burn scalp scabs to ash, mix with jujube flesh to make a paste. Wash with decoction of Congjieye (Scallion and Pepper leaves), sprinkle Qingfen (Calomel), then apply the paste spread on paper.
Another Formula: Digupi (Lycii Cortex) (1 liang), Gancao Jie (Licorice Joint) (0.5 liang). Boil in sesame oil until cooked, strain. Add Huangdan (Minium) (1.5 liang) and Baila (Beeswax) (0.5 liang). Boil vigorously until black, lift out, and spread on white paper for application. Next, boil Dongqingye (Aquifolii Folium) in vinegar and spread medicine for application.
Chilblains: Apply Mituoseng (Litharge) powder mixed with boiled tung oil.
Psoriasis: Apply liquor mixed with ox gall.
For unbearable pain in all sores, use bitter-cold herbs Huanglian (Coptidis Rhizoma) and Huangqin (Scutellariae Radix), selecting guiding herbs according to the location. Furthermore, for all sores, Huanglian (Coptidis Rhizoma) and Danggui (Angelicae Sinensis Radix) are chief herbs; Lianqiao (Forsythiae Fructus), Gancao (Licorice Root), and Huangqin (Scutellariae Radix) are assistant herbs. Add Huangbai (Phellodendri Cortex) for lower body lesions.
For those with robust constitution, Siwu Tang (Four Substances Decoction) [Formula 9] added with Dachengqi Tang (Major Order the Qi Decoction) should be used to purge.
For irritable, dark-skinned, thin individuals with blood-heat and sore pain, use Siwu Tang (Four Substances Decoction) added with Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Dalizi (Arctii Fructus), and Gancao (Licorice Root).
For obese individuals with sore pain, it is damp-heat. Use Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), Jingjie (Schizonepetae Herba), Baizhi (Angelicae Dahuricae Radix), Cangzhu (Atractylodis Rhizoma), and Lianqiao (Forsythiae Fructus), utilizing the principle that wind overcomes dampness. For upper body lesions, Fangfeng Tongsheng San (Saposhnikovia Unimpeded Success Powder) is often used; for lower body, purgatives are often necessary.
External Application Medicines: For pustules, treating heat, drying dampness, and resolving stagnation is primary. Wumingyi (Ilmenite), Songpitan (Pine Bark Charcoal) also treat pus. For dry scabies, resolving stagnation is primary, using Wuzhuyu (Evodiae Fructus). For excessive swelling, add Baizhi (Angelicae Dahuricae Radix) to resolve stagnation. For dry itching with bleeding, add Dahuang (Rhei Radix et Rhizoma), Huanglian (Coptidis Rhizoma), and Zhuzhi (Pork Fat) mixed for application. For excessive dampness, mix with oil. For severe itching, add Kufan (Alumen). For severe pain, add Baizhi (Angelicae Dahuricae Radix) and Fangjieshi (Calcitum). To stop itching and kill parasites, use Shechuangzi (Cnidii Fructus).
Parasitic sores resembling tinea: Primary treatment is clearing heat and killing parasites. Use Wuyi (Quisqualis Fructus), Heigouji (Acanthopanacis Senticosi Caulis), Baifan (Alumen), Xionghuang (Realgar), Liuhuang (Sulfur), Shuiyin (Mercury), Zhangnao (Camphora), and Songxiang (Rosin). For heavy parasite load, add Lulü (Veratri Radix) and Banmao (Mylabris). Or use Xihui (Tin Ash) and Binglang (Arecae Semen). For red color, add Huandan (Minium). For blue/green color, add Qingdai (Indigo Naturalis). For head sores, add Huanglian (Coptidis Rhizoma) and Fangjieshi (Calcitum). For scrotal sores, add Wuzhuyu (Evodiae Fructus). For swollen legs with bleeding, differentiate damp-heat for medication. For purulent sores, treating heat is primary. Huanglian (Coptidis Rhizoma), Huangqin (Scutellariae Radix), Dahuang (Rhei Radix et Rhizoma) (each...), Wumingyi (Ilmenite, stir-fried slightly), Baizhi (Angelicae Dahuricae Radix), Qingfen (Calomel).
Sha Chuang (Sand Sores): Wuyi (Quisqualis Fructus), Hanshuishi (Aqua Calcitum) (each 2 qian); Jiancao (Paederiae Scandentis Caulis), Wuzhuyu (Evodiae Fructus), Huangbai (Phellodendri Cortex), Kufan (Alumen) (each 1 qian); Cangzhu (Atractylodis Rhizoma), Houpo (Magnoliae Officinalis Cortex), Xionghuang (Realgar) (each 5 fen); Qingfen (Calomel) (10 patches). Powder them, mix with oil, and apply.
Another Formula: Wuyi (Quisqualis Fructus), Kufan (Alumen), Ruangao Shi (Soft Gypsum), Dahuang (Rhei Radix et Rhizoma), Zhangnao (Camphora). Powder them. First wash off crusts, mix with oil, and apply.
Yi Shang San (One Up Powder): Xionghuang (Realgar), Liuhuang (Sulfur) (each 3.5 qian); Hanshuishi (Aqua Calcitum), Baijiaoxiang (White Copal Resin), Heigouji (Acanthopanacis Senticosi Caulis), Shechuangzi (Cnidii Fructus) (each 1 liang); Huanglian (Coptidis Rhizoma), Kufan (Alumen) (each 5...); Wuzhuyu (Evodiae Fructus) (3 qian); Banmao (Mylabris, 14 pieces, wings and feet removed). Except for Liuhuang (Sulfur), Xionghuang (Realgar), and Hanshuishi (Aqua Calcitum) which are ground separately into fine powder, grind all others into extremely fine powder. Then grind Banmao (Mylabris) uniformly with the other medicines. First wash the sore, ensuring hot water penetrates to remove crusts. Heat腊猪油 (Cured Pork Fat) in hands, smell it twice or three times, then rub onto the sore. One application cures it. If pain and swelling are severe and high, double the amount of Hanshuishi (Aqua Calcitum). If not very itchy, add only Gouji (Acanthopanacis Senticosi Caulis).
