A person fell from a horse. Pain in both hypochondria (lateral chest areas). After taking two doses of Fuyuan Huoxue Tang (Revitalize the Collaterals to Invigorate Blood Decoction), the pain stopped abruptly. Then, taking two doses of Xiaochaihu Tang (Minor Bupleurum Decoction) added with Danggui (Angelicae Sinensis Radix) and Taoren (Prunus Persica Seed) led to recovery. (This treatment was based on symptoms.)
An elderly person fell from a horse. Abdominal pain. Using Fuyuan Tongqi San (Revitalize the Collaterals to Circulate Qi Powder), administered twice with diluted urine (Tongbian) as a vehicle, resulted in slight improvement. Then, taking four doses of Siwu Tang (Four Substances Decoction) added with Chaihu (Bupleuri Radix), Taoren (Prunus Persica Seed), and Honghua (Carthami Flos) led to recovery. (This treatment was based on symptoms.)
A person suffered a fall. The skin remained intact. Pain in both hypochondria accompanied by fever, dry mouth, and spontaneous sweating. One must first drink a bowl of diluted urine (Tongbian); the vexation and thirst would stop immediately. Then administer Fuxue Tang (Restore Blood Decoction), doubling the dosage of Chaihu (Bupleuri Radix) and Qingpi (Citri Reticulatae Pericarpium Viride) for one dose. The distending pain would completely resolve. Another dose ensured stability. The classic states: Falling from a height causes blood to flow internally, regardless of the twelve meridians. The Sages consider this wind-strike causing liver blood to stagnate under the hypochondria. It is treated using methods for wind-strike. Blood is primarily governed by the Liver; evil blood inevitably returns to the Liver. Regardless of which meridian is injured, blood inevitably stagnates under the hypochondria because the Liver governs blood. Severe pain necessarily leads to sweating. However, sweating in humans is generally considered a wind syndrome. All pains belong to Liver Wood.
A person presented with blue-black swelling and pain. Applied mustard radish juice mixed with Zhizi (Gardeniae Fructus) powder externally. Internally, Siwu Tang (Four Substances Decoction) added with Chaihu (Bupleuri Radix), Huangqin (Scutellariae Radix), Tianhuafen (Trichosanthis Radix), and Chuanshanjia (Manis Squama) was taken. After two doses, there was slight improvement. Then, using Tuoli San (Support the Interior Powder) and spleen-strengthening herbs led to recovery. (This treatment was based on symptoms.)
Distension and fullness in the chest and diaphragm should be treated by promoting blood circulation. For the elderly and weak, promote qi circulation and invigorate blood. Additionally, drink diluted urine (Tongbian) with wine. For intestinal abscesses, purge blood stasis. For those who have lost much blood and present with irritability, tonify blood. If ineffective, use Dushen Tang (Single Ginseng Decoction). (45) For necrotic flesh that does not ulcerate, or ulcerates but fails to heal, strongly tonify qi and blood.
A person with a cane wound (wound from beating) had necrotic flesh that did not rot. Therefore, strong tonification was applied. Gradually, it rotted. Then, using Tuoli Jianpi medicine (Support the Interior and Strengthen Spleen herbs) led to recovery. (This treatment was based on symptoms.)
A person contracted wind pathogen into a cane wound, resulting in trismus (lockjaw). Taking Yuzhen San (Jade True Powder) once brought slight improvement. A second dose ensured stability.
An official received beating for remonstrating against the Emperor's southern tour. The blood stasis had dispersed, but necrotic flesh did not ulcerate. Using interior-supporting herbs caused slight ulceration. The pus was clear. This indicated deficiency of qi and blood. Without large doses of Renshen (Ginseng Radix et Rhizoma), his condition could not be supplemented due to fear of abdominal fullness. I insisted he take it. His appetite slightly returned. Then, strong tonification was increased. The flesh ulcerated, pus became thick, and he recovered. (This treatment was based on symptoms.)
I once treated several people beaten with canes. They all first had their blood stasis dispersed. Subsequently, pus-draining and interior-supporting medicines were gradually used. All recovered. Shouting injures qi; enduring pain indicates qi and blood deficiency, which is clear. Moreover, the Spleen governs muscles. If Spleen qi is injured, dietary intake must decrease. Once blood coagulates/stagnates, muscles do not grow. Therefore, regulating the Spleen is essential. When the Spleen functions well, muscles naturally regenerate. Without cultivating Spleen earth using herbs like Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), and Danggui (Angelicae Sinensis Radix), how would muscles arise? One must also differentiate between excess and deficiency patterns, and the presence or absence of blood stasis. Generally, after falls or drops, if the skin and flesh are intact but abdominal pain occurs, there must be internal blood stasis. Fuyuan Huoxue Tang (Revitalize the Collaterals to Invigorate Blood Decoction) (33) should be used to attack it. For the elderly and weak, add Taoren (Prunus Persica Seed), Honghua (Carthami Flos), and Chuanshanjia (Manis Squama). Tonify while moving blood. If blood loss is excessive and irritability occurs, this is blood deficiency. Dushen Tang (Single Ginseng Decoction) (45) should be used to tonify. In cases of mild falls or drops without other blood stasis symptoms, but only persistent pain, harmonizing qi and blood and regulating meridians will naturally stop the pain. Follow up with nourishing qi and blood and strengthening the spleen and stomach; there will be no failure. There are also cases where pain injures Stomach qi causing vomiting or anorexia; these are treated with Sijunzi Tang (Four Gentlemen Decoction) added with Huoxiang (Agastaches Herba), Sharen (Amomi Fructus), and Danggui (Angelicae Sinensis Radix). If blood stasis exists and tonics are added without first dispersing the stasis, it results in the disaster of "excess reinforcing excess." Conversely, if there is no blood stasis but purgatives are recklessly used, it results in the disaster of "deficiency attacking deficiency."
A person had buttock and knee wounds from beating that ulcerated. Pus and stasis had not been discharged, and they frequently experienced fainting/delirium. This was internal action of pus toxin. Urgent opening (drainage) was performed, but the fainting worsened. This indicated deficiency. One dose of Bazhen Tang (Eight Treasures Decoction) (14) brought slight relief. After several doses, dead flesh naturally ulcerated and was removed. Washed with pig trotter soup. Applied Shenxiao Danggui Gao (Miraculous Angelica Ointment). Took Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction). Recovered in two months. If blood-breaking agents were further administered, it would be critical. (This treatment was based on symptoms.)
Generally, cane wounds are caused by blood stasis. Prompt treatment is suitable. Superficial ones should be pricked with bi-shi (stone needles). Deep ones should be pierced with needles. Treat with blood-invigorating and qi-circulating herbs. For internal ulceration, remove necrotic flesh and support with stomach-strengthening and blood-generating herbs to expel toxins. Safety can be ensured. For injuries to sinews and bones causing pain, Moyao Jiangsheng Dan (Myrrh Supreme Sage Pill) is used. For trismus or opisthotonos (backward arching of back), Yuzhen San (Jade True Powder) is used. Both are effective.
For cane wounds with hot blood causing pain, cooling blood and removing stasis is the priority. Medicines like Jiming San (Crowing Rooster Powder) should be taken. Huangbai (Phellodendri Cortex), Shengdihuang (Rehmanniae Radix Recens), and Zijinpi (Broussonetiae Cortex) are essential herbs.
Ointment formula: Zijinpi (Broussonetiae Cortex), Shengdihuang (Rehmanniae Radix Recens), Huangbai (Phellodendri Cortex), Rushxiang (Boswelliae Resina), Moyao (Myrrha Resina), Dahuang (Rhei Radix et Rhizoma), etc. Another prescription: Dahuang (Rhei Radix et Rhizoma) and Huangbai (Phellodendri Cortex) powdered, mixed with Shengdi juice for external application. Change when dry.
Another method: Young wild ramie roots (Yezhu Gen), any amount, washed clean. Pounded together with salt for external application. Use more salt for severe injuries. Effective.
I once saw seven people crushed by a overturned cart lying on the ground groaning. One was unconscious. All were given hot diluted urine (Tongbian) to drink. All were fine. I also encountered someone severely injured, confused and drowsy, reviving after a long time. Chest fullness felt like pounding, breath obstructed. Immediately drinking a bowl of hot diluted urine (Tongbian) relieved chest fullness and unblocked qi. Only lower abdominal pain remained. Given one dose of Fuyuan Huoxue Tang (Revitalize the Collaterals to Invigorate Blood Decoction) (33). Passed several sheng of bloody stool. Swelling and pain completely receded. Took blood-nourishing and qi-regulating herbs and recovered.
Generally, for trauma, regardless of strength or weakness, and regardless of blood stasis, hot diluted urine (Tongbian) assisted with wine is suitable to push out the old and generate the new. Its efficacy is great. If hypochondria hurt, or fever and irritability occur, dry mouth preferring cold drinks, drinking a bowl of hot diluted urine (Tongbian) surpasses other medicines. Although other medicines may also be effective, identifying the presence or absence of blood stasis is not always accurate, potentially leading to misdiagnosis. Diluted urine (Tongbian) does not disturb zang-fu organs nor injure qi and blood. It is said that soldiers exercising or falling from horses benefit greatly from taking it. Also, for any swelling, pain, or trauma, applying pounded scallion (Cong Bai) heated externally is excellent. Materia Medica states: Scallion treats trauma.
A person fell from a horse, injuring head and arm. Scallion was pounded, fried hot, and applied to the affected area, massaged with hot hands. Took Moyao Jiangsheng Dan (Myrrh Supreme Sage Pill) (106) and recovered.
A person accidentally severed a section of the little finger. Trismus, opisthotonos, and unconsciousness occurred. Yuzhen San (Jade True Powder) (123) and Qingzhou Baiwan (Qingzhou White Pills) (108), one dose each, were ineffective. This was also due to medicinal power failing to reach the site. Urgently, garlic was pounded, wrapped around the injured finger, and moxa was applied. After a long time, pain was felt. Still, one dose of Baiwan (White Pills) and several doses of Tuoli San (Support the Interior Powder) led to recovery.
