A patient with a scald (burn) suddenly applied cooling medicines externally, which exacerbated abdominal distension and loss of appetite. I prescribed Renshen Baidu San (Ginseng Decoction for Resolving Toxins) (46) with Mutong (Akebiae Caulis) and Shanzhi (Gardeniae Fructus). Externally, Baiye (Platycladi Cacumen) was stir-fried into powder, mixed with Sesame Oil, and applied, leading to gradual recovery.
I once used the floating fat from boiled Da Zhi (likely referring to a specific oil or resin preparation) mixed with Yinzhu (Cinnabar) for better efficacy. If cooling medicines are used to force fire toxins inward, it often leads to untreatable conditions. For scalds and burns, ash residue from the second leaching process can be applied to the affected area.
Another prescription: Coat Huangbai (Phellodendri Cortex) with pig bile from the twelfth lunar month, dry by roasting, grind into powder, and apply externally.
Lengshuang San (Cold Frost Powder) for Scalds and Burns Causing Skin Ulceration and Severe Pain: Muli (Ostreae Concha), Hanshui Shi (Calcitum, raw), Ming Puxiao (Natrii Sulfas), Qingdai (Indigo Naturalis) (each 1 liang), Qingfen (Calomelas) (1 qian). Mix with fresh water, or blend with oil; if wet, apply dry patches. Pain stops immediately.
Another patient suffered from lacquer dermatitis accompanied by vomiting due to weak middle qi invaded by lacquer toxin. I treated him with Liujunzi Tang (Six Gentlemen Decoction) plus Sharen (Amomi Fructus Rotundus), Huoxiang (Agastaches Herba), and Shaoyao (Paeoniae Radix Alba) fried with wine. He did not follow my advice but took Lianqiao Xiaodu San (Forsythia Detoxification Powder), causing severe vomiting. Upon re-consultation, I used the previous prescription and applied Tie Xiu Mo (Iron Rust Powder) mixed with Sesame Oil externally, resulting in recovery.
If the pulse is floating with fever or spasms, disperse exterior pathogens.
If the pulse is sunken with fever and constipation, release the exterior and purge the interior.
If there is fever with red and scanty urination, promote diuresis and detoxify.
A child suffered from this condition with pain, fever, and a floating-rapid pulse. Toxic fluid was drained, and Huangbai (Phellodendri Cortex) and Huashi (Talcum) powder was applied. The child drank Jingfang Baidu San (Schizonepeta and Saposhnikovia Decoction for Resolving Toxins) (7) for two doses and recovered. (This diagnosis relied on symptoms.)
Another patient had pain and fever, which worsened after taking wind-dispelling and heat-clearing medicines. The pulse was sunken and solid, indicating internal pathogens. Fangfeng Tongsheng San (Saposhnikovia Universal Relief Powder) (6) was administered for one dose, leading to rapid symptom reduction. Subsequently, Jingfang Baidu San (Schizonepeta and Saposhnikovia Decoction for Resolving Toxins) (7) for two doses brought stability. (This diagnosis relied on symptoms and pulse.)
This condition is caused by wind-heat; examining the exterior and interior ensures correct treatment.
A child presented with redness, fever, and skin lesions. Huangbai (Phellodendri Cortex) and Huashi (Talcum) powder was applied. Dalianqiao Tang (Large Forsythia Decoction) (71) was given for two doses, showing slight improvement, followed by Jinyinhua San (Honeysuckle Powder) (170) for two doses until full recovery. (This diagnosis relied on symptoms.)
A child over ten years old developed several blisters on the side of the back, with fever and a rapid pulse. This was caused by lung and stomach wind-heat, named Tianpao Chuang (Heavenly Blister Disease). Jingfang Baidu San (Schizonepeta and Saposhnikovia Decoction for Resolving Toxins) (7) with Qin (Scutellariae Radix) and Lian (Isodontiae Rhizoma) was prescribed. Externally, toxic fluid was removed, and Jinhuang San (Golden Yellow Powder) (66) was applied. Recovery occurred after four doses. (This diagnosis relied on symptoms.)
Another patient had lesions on the abdomen spreading to the lower back, with pain, preference for cold drinks, rapid pulse, and an even stronger pulse upon palpation, indicating excess in both exterior and interior. Fangfeng Tongsheng San (Sapothnikovia Universal Relief Powder) (6) was given for one dose, along with external application of the previous remedy, reducing the severity by more than half. Jingfang Baidu San (Schizonepeta and Saposhnikovia Decoction for Resolving Toxins) (7) for two doses led to complete recovery.
