Wu stated: Qi and blood flow smoothly when exposed to fragrant odors, but move in reverse when exposed to foul odors. Generally speaking, sores and ulcers often arise because Ying (nutritive) Qi fails to circulate properly, becoming reversed within the muscle tissue, stagnating and accumulating into pus. Upon exposure to fragrant scents, Qi and blood can flow freely. Therefore, one should frequently administer Wuxiang Liaoqiao Tang (Five-Spice Forsythia Decoction), Wanjin San (Ten-Thousand-Gold Powder), and Jinfen San (Gold-Dust Powder). Any sore that inherently has a fishy or foul odor will worsen if exposed to such smells; if the toxin enters the stomach causing coughing and counterflow of Qi, the ancients' use of these methods was indeed reasonable. Furthermore, by adjusting the diet to be fragrant and delicious to benefit the Spleen Earth and nourish the True Yuan (Original Essence), one can ensure safety from worry.
My commentary: The discussion on the aforementioned symptoms can be found in chapters eighteen and nineteen.
Li stated: Patients suffering from Ju (deep-seated abscesses) must abstain from alcohol, wheat products, roasted/fried foods, cured meats, Fa Jiu (medicinal wine), cold/raw items, greasy foods, as well as chicken, goose, and fishy seafood. Regarding daily life and the Seven Emotions (Joy, Anger, Worry, Thought, Sadness, Fear, Fright), extreme caution is required. The bedroom should be clean and fragrant to promote smooth Qi and blood circulation. One must also avoid contact with monks, Taoist priests, mourners, postpartum women, menstruating women, and animals such as chickens, dogs, and cats. If a back carbuncle makes it difficult to lean on a table for rest, it is advisable to use ten dou of mung beans made into a bag and lie upon it to detoxify and cool the Heart.
My commentary: The previous discussion emphasized careful living habits and bland diets, likely because patients tend to overexert their limbs and crave foods that are either too hot or too cold in nature. This exacerbates Fire pathogens and weakens Stomach Qi, leading to prolonged illness. Who could blame them? It is better to prevent this in advance and heed the words of the ancients. For instance, Mung Bean is cold in nature and treats Dan Du (erysipelas), irritability, heat, wind-rashes, or real-heat irritability and thirst caused by mineral drugs, where appetite remains normal and the syndrome belongs to Pure Yang. It should not be used otherwise.
Li stated: I often see physicians in the world who possess excellent prescriptions but keep them secret, or alter them to confuse people, which is truly despicable. I recall that someone kept the formula for Xihua Maifan Shigao (Xihua Wheat-Bread Stone Plaster) secret until death, a selfish attitude criticized by gentlemen. In the past, Zhao Jiaoxuan, courtesy name Jixiu, came to serve as the magistrate of Longquan. Traveling at full speed, he suffered from Bi Nu (epistaxis/nosebleed), losing several sheng of blood daily at sunrise. My elder brother instructed him to take Ou Zhi Dihuang Gao (Lotus Root Juice and Rehmannia Paste). Zhao asked other doctors, who claimed it was a forbidden remedy. He changed doctors multiple times without effect. My elder brother secretly provided the juice, and after three days, the illness resolved. My brother said, "This is the prescription previously offered." Zhao exclaimed in amazement, "If not for the doctor's deceptive tactics confusing me, had I believed this prescription earlier, how could I have suffered so long?" Now, based on formulas that have proven effective repeatedly and should not be changed: for vomiting due to Heat Toxin rushing to the Heart, use Nituo San (Internal Support Powder, also known as Wanquan San); for vomiting due to Spleen-Stomach deficiency, use Jiahe San; for Heat Toxin swelling and pain, use Maifan Shigao; for Yin deficiency causing thirst, use Jiawei Bawei Wan (Modified Eight-Ingredient Pill). These four formulas were often obscured by physicians, preventing their use. Deeply pitying the suffering of patients, I cite Mr. Zhao's case as an example.
I privately believe that a "formula" implies imitation—imitating the cause of disease to create a prescription. It does not mean the *Neijing* lacks formulas. If one rigidly adheres to ancient formulas to treat modern diseases, it is like dismantling an old house to build a new one; the dimensions would hardly fit without a carpenter's skill. Even if they fit, claiming them as unchangeable eternal formulas suggests they are divine arts for universal salvation by the former kings, why keep them secret or private? I observed Master Taiwu treating a monk from Southern Yunnan who traveled far through Jiangsu and Zhejiang, falling ill from homesickness. Master Taiwu provided food and medicine, and gifted a yi (twenty taels) of gold to fund his return. This is naturally what our medical profession should do. Today's physicians are either stuck in the past, stingy with secrets, or greedy for profit, offending moral principles greatly.
Shenxian Zhuidu Wan (Immortal Poison-Chasing Pill, also known as Shenghou Dan, Yushu Dan, Jiedu Dan, Wanbing Wan, Zijin Ding.)
Indications are listed below.
Wenge (i.e., Wu Beizi/Galla Chinensis, crushed, washed, dried, and powdered, 3 qian) Shancigu (Colchicum autumnale, peeled, cleaned, and powdered, 2 liang) Shexiang (Musk, 3 qian, ground separately) Qianjinzijie (Semen Crotonis Tiglii, also known as Xuisui Zi, husked, ground, oil removed to leave the residue/paste, 1 liang) Hongya Daji (Euphorbia pekinensis, rhizome removed, dried, and powdered, 1.5 liang)
Mix the above with thick rice gruel to form pills, divided into forty grains. Take one pill per dose, ground with well water or Mint Decoction. Stop with congee after one or two bowel movements. Prepare during Dragon Boat Festival, Qixi, Double Ninth, or auspicious days (Tiande/Yuede). Must be prepared in a clean room with incense burned. Efficacy cannot be fully described. Everyone should carry this medicine. In the valleys of Liangguang, there is a grass called Humeng or Duanchang (Heartbreak). Some contract snake venom and develop malignant tumors called Niannian Yao (Year-Fixing Medicine). An adulteress favored by a northern man placed poison in her food before parting, warning him, "You must come at a specific time." If he arrived on time, she would provide an antidote; if he missed the date, he would die. Hence, Year-Fixing Medicine. Taking one pill of the aforementioned medicine cures the condition. Once, a woman suffered from chronic consumption (Laozai) eaten by corpse-worms. She took one pill, vomited and defecated many small worms, then took Suhexiang Wan (Styrax Pill) and recovered. For toxins from mushrooms, arsenic, dead cattle/horses, river fish, epidemic fevers, mountain miasma, throat closure/wind, hanging, drowning, or blunt trauma, if the heart is still slightly warm and less than one night has passed, grinding one pill with Ginger-Honey Water and administering it restores consciousness. For carbuncles, ulcers, burns, snake/insect/dog/beast bites, grind with flowing eastward water for topical application and ingestion. For mania, ghost qi, or ghost pregnancy, grind with warm wine. All are effective. In my opinion, this medicine truly has miraculous effects for accumulated toxins, poisoning, and deep-rooted entanglement by toxic insects. However, for diseases arising internally, one must carefully assess deficiency and excess before use.
Zhuan Du San (Transform-Toxin Powder)
Treats all carbuncles and sores, promoting the expulsion of the toxin root to prevent progression.
Cheshi (Tridacna gigas, purple-backed and thick ones are best, fixed with salt mud, calcined until red, cleaned, and powdered, 1 liang) Qingfen (Calomel, 5 fen) Gancao (Licorice Root, 1 fen)
Take 4 qian per dose. Grind one Trichosanthes fruit (Gualou) finely, decoct in wine, and take at dawn (Wugeng). Continue until foul matter is expelled. If ineffective, repeat. Its efficacy surpasses Wuxiang Liaoqiao Tang. Use Wuxiang Liaoqiao Tang for slow cases, and Shenxian Jie Fa (Immortal Interception Method) for urgent cases.
My commentary: The previous two formulas are suitable only when toxins are accumulated internally and Yuan Qi is not deficient. Careful assessment is necessary.
Sun Simiao's Single Boiled Dahuang Tang (Rhubarb Decoction)
Disperses heat and extracts toxins, treating constipation due to internal accumulation of Heat Toxin.
Jinwen Dahuang (Rhubarb Radix et Rhizoma, soaked in wine and peeled)
Decoct in water and take. This is a crucial method for rapid purgation.
My commentary: The previous formula is a cold-cooling detoxifying agent. Assess Yuan Qi deficiency/excess and whether the syndrome is interior or exterior before use.
Attached Case Records
Brother of Commissioner Shen Tianyi suffered from a back carbuncle. Pulse was deep and excess. Swelling was hard and numb, with constipation. This indicated toxin accumulation internally. Used Dahuang and Baizhi, named Wanquan San. After one dose, stasis was expelled, and the sore instantly subsided.
Official Kuang suffered from a boil nearing cure but had constipation. After taking Dahuang and other drugs, he lost his appetite. I used Qi and Blood tonics plus Taoren and Mahong, without effect. Then inserted pig bile deeply into the rectum, followed by nourishing Qi and Blood, resulting in recovery.
Shenxian Jie Fa (Immortal Interception Method)
Treats carbuncles, back infections, and all malignant sores. Taken prophylactically to prevent toxin penetration inward.
Zhen Mayou (True Sesame Oil, 1 jin, boiled in silver/copper vessel for ten boils, cooled)
Take 2 bowls of wine mixed with 5 cups of oil, drink hot through the mouth. Consume within one day, or spread over several days for slower cases. Wu Anshi said, "My family has used it for three generations without failure." Hunters also say that for poisoned arrow wounds, drinking sesame oil quickly eliminates the poison. Zheng Xueyu Defu used it repeatedly with great success.
My commentary: Sesame oil directs downward Heat Toxin from the Triple Burner and unblocks stagnation in the Large and Small Intestines. For early-stage sores with internal toxin accumulation and constipation, using it to unblock the toxin is indeed a good method. However, if stools are loose or there is Empty Heat in the Triple Burner, it is unsuitable. Users must assess carefully.

Take 5 qian per dose, decoct with ginger water. Strain and take on an empty stomach. Expulsion of foul matter is ideal.
My commentary: These three formulas are exterior-releasing, interior-purgative, Qi-moving, and detoxifying agents. They were likely used at the time according to specific syndromes.
Liuli Cheshi San (Six-Ingredient Tridacna Powder)
Cheshi Tridacna Gigas (four shells, fixed with yellow mud and calcined) Dengxin Junci Medulla Gancao Jie Glycyrrhizae Radix et Rhizoma (2 qian) Gualou Trichosanthis Fructus (one fruit, crushed)
Decoct two cups of wine to reduce to 80%. Add one spoon of honey, 2 qian of Cheshi powder, and a little Nifen (Calomel). Take on an empty stomach. Expulsion of evil blood is ideal.
My commentary: The previous formula is suitable for sores with internal toxin accumulation, constipation, and sufficient Yuan Qi.
Zhitong Lingbao San (Pain-Relieving Spiritual Treasure Powder)

Take 1 liang per dose, decoct with half water and half wine.
My commentary: The previous formula treats pain caused by aggregation of toxin vapor in swelling sores. Use with caution after ulceration. Guixiyao is also known as Biluo or Luoshi (Parthenocissus tricuspidata).
Shenxiao Mahong Wan (Spiritually Effective Hemp Seed Pill)
Huomaren Cannabis Fructus (finely ground) Dahuang Rheum Radix et Rhizoma (baked, 3 qian) Renshen Ginseng Radix et Rhizoma (2 qian) Hezi Terminalia Chebula Retusa Fruit (baked flesh, 1 liang)
Grind into powder, mix with Huomaren and refined honey to form pills the size of paulownia seeds. Take 30 pills per dose with boiling water.
My commentary: The previous formula moistens dryness in the Large Intestine. It should not be used for those with depleted blood and fluids. Even with Renshen and Hezi, reliance is difficult.
Qingxin Neigu Jinfen San (Clear-Heart Internal-Solidifying Gold-Dust Powder, also known as Jinhua San)

Grind into powder. Take 1.5 qian per dose, mixed with honey water.
My commentary: The previous formula is usable if there is heat-toxin swelling and pain with normal appetite.
Zhuti Tang (Pig Trotter Decoction)
Treats all ulcers, clears heat-toxin, removes necrotic tissue, and resolves foulness. Highly effective.
Baizhi Angelicae Dahuricae Radix Gancao Glycyrrhizae Radix et Rhizoma Qianghuo Notopterygii Rhizoma et Radix Lufengfang Vespa Nidus (with bees inside) Chishaoyao Paeoniae Radix Rubra Danggui Angelicae Sinensis Radix (equal parts)

Xiyao Shenxiao San (Washing Spiritually Effective Powder)
Shechuangzi Cnidii Fructus (2 liang) Poxiao Natrii Sulfas (1 liang)

Huangbai Phellodendri Cortex (charred black) Chuanshanji Pangolin Scale (stir-fried yellow, 1 liang) Binglang Arecae Semen Muxiang Aucklandiae Radix Jineijin Gallus Gallus Gigerium Endothelium Corneum (two pieces)
Grind into powder. Apply a small amount inside the wound 5-7 times daily for effect.
My commentary: The previous formula should be used only when toxin vapor is exhausted; otherwise, it may aggravate evil. Using Danggui Gao (Angelica Ointment) is better.
Shenmi Xianmai San (Divine Mystery Collapsing-Vessel Powder)
Huangqi Astragali Radix Renshen Ginseng Radix et Rhizoma Chuanxiong Ligustici Chuanxiong Rhizoma Danggui Angelicae Sinensis Radix (wine-washed) Chishaoyao Paeoniae Radix Rubra Fencao Glycyrrhizae Radix et Rhizoma Digupi Lycii Cortex Wujia皮 Acanthopanacis Cortex Rendongye Lonicerae Japonicae Folium Juhong Citri Reticulatae Pericarpium Rubrum (each 1 liang) Ruxiang Olibanum Moyho Myrrha (each 5 qian)
Take 5-7 qian per dose, decoct with half water and half wine. Take continuously for 5-7 doses. Highly effective.
