At the initial stage, lesions are small as large as a chestnut or as large as a chess piece, presenting as blisters filled with fluid (a type of vesicle/bulla containing fluid). Those with red coloration are called Huochi Chuang (Fire-Red Sore); those with white tops and red bases are named Tianpao Chuang (Heaven Blister Sore). Some occur on the head and face, treatable with a decoction (remove Gleditsiae Spina [Zaocis Spina], Manis Squama [Pangolin Scale], add Scutellariae Radix [Huangqin] and Akebiae Caulis [Mutong]). Apply Shizhen San externally to the affected area for immediate healing. Tianpao Chuang arises from wind-heat toxic factors lodging in the skin and struggling with blood and qi. It begins like a scald from hot water forming blisters; once ruptured and exuding fluid, it becomes a sore. Treatment uses Rehmanniae Radix [Shandi], Cimicifugae Rhizoma [Shengma], Gardeniae Fructus [Shanzhi], Isatidis Folium [Lanye], and Rheum Palmatum [Dahuang], each one liang, crushed. Fry them in eight liang of lard over moderate heat until the color changes, strain the residue, store in a porcelain vessel, and apply to the lesion.
Shizhen San: Gypsum Fibrosum [Sheng Shigao] (one liang), Calomelas [Qingfen], Phellodendri Cortex [Huangbai], Sepiae Endoconcha [Haipiaoxiao] (each five qian). Grind all together into fine powder. Wash the affected area with Licorice Decoction [Gancao Tang], then sprinkle this powder on it. Xianfang Huoming Yin (General Formula 1).
At the initial stage, the shape resembles a cat's eye, shimmering with luster, without pus or blood, characterized by intermittent pain and itching. This is caused by stagnant heat congealed by external cold. It is suitable to apply Zhenjun Miaotie San. Consuming more chicken, fish, chives, and garlic will lead to self-healing.
Zhenjun Miaotie San: Fagopyrum Esculentum Flour [Qiaomian] (one liang), Sulfur [Liuhuang], Triticum Aestivum Flour [Baimian] (one liang). Mix all ingredients together, knead with clear water to achieve appropriate moisture content, roll into thin slices, dry slightly in the sun, wrap in single paper, and air-dry in the shade. Take, grind into fine powder, and mix with sesame oil for application.
Initially presenting as hard white blisters, gradually enlarging to resemble fish spines, mostly occurring between muscles and bones. This is due to deficiency and emaciation of yang qi, with cold-dampness struggling and knotting, hence the onset is usually slow, only in the skin... Xiaojin Dan (General Formula 52).
Qichuang (Lacquer Dermatitis) arises because human couli (skin pores and interstices) are not dense, leading to infection by the acrid-hot toxicity of lacquer. Initially, there is itching and swelling; scratching causes it to gradually resemble urticaria with red coloration, spreading throughout the limbs, red skin... Alternatively, apply raw crab yellow directly; it heals after a few applications.
In individuals with weak stomachs, if Qichuang invades the middle qi, vomiting and inability to eat often occur. Use Liujunzi Tang added with Amomi Fructus [Sharen], Agastache Herba [Huoxiang], and White Peony Root [Baishao] stir-fried with wine, decocted and taken orally to restore peace. Avoid water baths, greasy foods, and stimulating foods (fa wu). Liujunzi Tang (General Formula 21).
