(This condition is also known as "Riding Carbuncle" or "Mouse Stealing Feces".)
It arises at the junction of the anterior to the anus, the root of the genitals, and the posterior orifice. Initially, it presents as fine granules, gradually enlarging to the size of a lotus seed, and after several days, becomes as large as a plum or peach. If the lesion is red and painful, [a specific formula is implied but missing in the text] should be taken, with rapid healing being the priority. If the lesion is white and hard, use *Xiao Jin Dan* (Little Golden Pill) to dissipate it. If it has ulcerated, take 3 qian of Deer Antler Glue (Lujiaojiao), dissolved in wine and taken on an empty stomach in the morning. These can be alternated daily, or one may use formulas that warm and tonify Qi and Blood, alternating with *Xiao Jin Dan* morning and evening, which is also excellent, continuing until痊愈 (recovery). Once this condition ulcerates, if essence leaks out, it will form a fistula (Lou syndrome). Stool and urine will leak from here, leading to death when blood vessels are exhausted. This condition resembles consumptive disease (Qie Sun). While most fistulas are treatable, this one is particularly difficult; treatment may cause the fistula to enlarge further, eventually becoming a "Sea Bottom Fistula."
"Hanging Carbuncle" (Xuanyong) often results from retaining semen and raising Qi, described as desiring ejaculation but suppressing it, transforming into pus and blood. It is extremely difficult to treat because the skin and flesh in this area differ from other parts, making recovery from injury slow. If initially red and painful, use *Zhu Xie Zhi Shen Dan* (Divine Formula for Expelling Evil): Silver Flower (Jinyinhua) 4 liang, Dandelion (Gongying) 2 liang, Ginseng (Renshen) 1 liang, Raw Licorice Root (Sheng Gancao) 1 liang, Angelica Sinensis (Danggui) 1 liang, Rhubarb (Dahuang) 5 qian, and Trichosanthes Root (Tianhuafen) 2 qian. Decoct in water and take one dose to eliminate toxicity, and two doses for complete cure. For ulcerated cases, three doses suffice for healing. This formula uses Silver Flower and Dandelion to assist Ginseng and Angelica, while using 5 qian of Rhubarb to attack and purge. It seems unsuitable for deficient persons, but during the initial stage when toxin is strong and Zheng Qi (Upright Qi) is not yet depleted, using herbs that tonify while purging is appropriate. If treatment is delayed or hesitation occurs, waiting until pus and blood flow out and Zheng Qi is exhausted before using Ginseng and Astragalus (Huangqi) to tonify Qi often requires several jin (catty) without success. Why not use tonifying herbs early within the context of resolving toxicity, preserving Zheng Qi while dispersing the toxin?
Guolao Jian (Decoction of the Old Man)
Powdered Licorice Root (Fen Gancao) (1 liang, cut into sections, soaked in mountain spring water, and slowly roasted through.)
Take the above single herb, finely chopped, decocted in pure wine (wine without sediment/additives).
