Whenever pediatric malnutrition (Gan syndrome) is internal, presenting with dry and gritty eyes, abdominal distension, dysentery of variable color, sometimes resembling rice water sediment, and progressive emaciation. This condition is treatable. If there is redness and ulceration under the nose, self-rubbing of the nose, sores on the head forming crusts with pain and itching that gradually spread to both ears, frequent redness of the eyes, sparse hair, tight and shiny scalp skin, a large head with a thin neck, and emaciated body and muscles, this is also treatable. If the lips and mouth are eroded, gums show five colors or turn completely black, white sores appear under the tongue with perforations, foul breath occurs occasionally, and gums gradually darken and rot, this is certainly treatable. If the lower orifice (anus) is open, sometimes red and ulcerated with unbearable itching, and diarrhea is irregular, this is also treatable. If Gan erosion affects the spine, fingers itch, the child bites nails, hair stands up in tufts, the spine protrudes like a saw, accompanied by occasional abdominal distension and diarrhea.

For pediatric Liver Gan: if the eyeballs show blue veins, hardness under the left hypochondrium, frequent spitting of saliva, and black qi rushing around the corners of the eyes, it is untreatable. For Heart Gan: if the child loves startled crying, constantly desires water, likes spicy food, has pulsations near the ears, and black spots on the tongue, it is untreatable. For Spleen Gan: if the abdomen is large, lips lack blood color, the philtrum is flat and full, diarrhea is unceasing, food and water are not digested, the child craves eating dirt, skin is withered with exposed bones, it is untreatable. For Lung Gan: if there is coughing with reverse flow of Qi and rapid breathing, frequent spitting of white foam, spots on the body the size of millet grains turning black, it is untreatable. For Kidney Gan: if the child likes sour and salty foods, drinks water without limit, urine resembles cow's milk, teeth are dark green/black, ears and brain area are dry, shoulders are hunched with withered bones, it is untreatable. Furthermore, there are five terminal signs for the Five Gans: 1. No pain felt at the bottom of the toes; 2. Holding hands and feet but lacking strength to lift them; 3. Disease persists, whole body is not warm; 4. Viscera discharge green saliva and foam ceaselessly; 5. Tendons and sinews extend but lack strength. Such conditions are all untreatable. When using medicine, physicians must be extremely cautious and detailed.
