Whenever the duration of a cold damage (Shanghan) disease is prolonged, heat accumulates in the abdomen. Due to insufficient food intake, the stomach and intestines become empty, causing the "Three Worms" (intestinal parasites) to move about seeking food, eroding the five zang organs and the lower orifices. The manifestations include: teeth lacking luster, the entire tongue coated with white fur, severe cases presenting with sores inside the lips, heavy limbs, mental confusion, and excessive sleepiness. These are all indications that worms are eroding the anus, leading to anal ulceration.
For treating cold damage where worms erode the lower orifices, accompanied by incessant restlessness, stifling sensation, itching, and pain, the Tao Ren San (Peach Kernel Powder) formula is suitable.
Ingredients: Tao Ren (Peach Seed, 2 liang, soaked in hot water, skin and pointed ends removed, retaining double kernels, stir-fried with bran until slightly yellow), Huai Zi (Sophora Fruit, 1 or 2 liang, slightly stir-fried), Shu Ai (Mugwort Leaf, 2 liang, slightly stir-fried), Huang Lian (Coptis Rhizome, 1 liang, whiskers removed, slightly stir-fried).
Preparation and Administration: Pound and sieve the above ingredients into a powder. Take 3 qian per dose. Add 1 zhong of water, decoct until reduced to 0.5 zhong, strain, and take warm before meals.
For treating cold damage causing sores in the lower orifices, erosion of the anus, and pain, the Xiong Huang San (Realgar Powder) formula is suitable.
Ingredients: Xiong Huang (Realgar, 1 liang, finely ground), Qing Xi Zi (Celosia Seed, 1.5 liang), Huang Lian (Coptis Rhizome, 1.5 liang, whiskers removed), Ku Shen (Sophora Root, 1.5 liang, sliced), Tao Ren (Peach Seed, 3 fen, soaked in hot water, skin and pointed ends removed, retaining double kernels, stir-fried with bran until slightly yellow).
Preparation and Administration: Pound and sieve the above ingredients into a fine powder. Before meals, mix 2 qian with millet broth and ingest. Additionally, wrap the powder in cotton cloth into a size similar to an apricot kernel, and insert it into the lower orifice for good effect.
For treating cold damage causing painful oppression in the heart, dysentery, and sores/ulceration in the lower orifices.
Ingredients: Li Lu (Veratrum Rhizome, 2 liang), Gan Qi (Dried Resin of Toxicodendron, 0.5 liang, crushed and stir-fried until smoke emerges), Pian Zhu (Bamboo Sheath/Daizhe Tu, likely referring to a bamboo-related herb, 0.5 liang, dried).
Preparation and Administration: Pound and sieve the above ingredients into a fine powder. Before meals, mix 2 qian with millet broth and ingest.
For treating cold damage causing mental restlessness and stifling sensation, sores in the lower orifices, and pain, the She Xiang San (Musk Powder) formula is suitable.
Ingredients: She Xiang (Musk, 1 fen, finely ground), Xiong Huang (Realgar, 1 fen, finely ground), Zhu Sha (Cinnabar, 1 fen, finely ground), Ling Yang Jiao (Antelope Horn, 1 fen), Qing Xi Zi (Celosia Seed, 1 fen), Huang Lian (Coptis Rhizome, 1 fen, whiskers removed), Chuan Sheng Ma (Bupleurum Root, 1 fen), Tao Ren (Peach Seed, 1 fen, soaked in hot water, skin and pointed ends removed, retaining double kernels, stir-fried with bran until slightly yellow), Bei Chi (Concha Meretricis, 1 fen).
Preparation and Administration: Pound and sieve the above ingredients into a fine powder. Before meals, decoct wheat beverage, mix with 2 qian of powder, and ingest.
For treating early-stage cold damage accompanied by sores in the lower orifices and pain, the Qing Xi San (Celosia Seed Powder) formula is suitable.

Preparation and Administration: Pound and sieve the above ingredients into a fine powder. Before meals, mix 2 qian with millet broth and ingest.
For treating cold damage causing sores inside the lower lip, worm erosion of the lower orifices, pain, and occasional dysentery, the E Jiao San (Donkey-hide Gelatin Powder) formula is suitable.
Ingredients: E Jiao (Donkey-hide Gelatin, 3 fen, crushed and stir-fried until yellow and dry), Huang Bo (Phellodendron Bark, 0.5 liang, slightly roasted and sliced), Dang Gui (Angelica Sinensis Root, 0.5 liang, sliced and slightly stir-fried), Bing Lang (Areca Seed, 0.5 liang), Mu Xiang (Costus Root, 3 or 0.5 liang), Long Gu (Fossilized Bone, 0.5 liang), Huai Zi (Sophora Fruit, 1 liang, slightly stir-fried).

Ingredients: Li Lu (Veratrum Rhizome, 0.5 liang, rhizome head removed), Gui Xin (Cinnamon Twig Heart, 1 liang), Ba Dou (Croton Seed, 1 fen, skin and heart removed, ground and pressed between paper to remove oil), Fu Zi (Prepared Aconite Root, 1 liang, processed until cracked, skin and navel removed).
Preparation and Administration: Pound and sieve the solid ingredients into powder. Mix with the ground Ba Dou evenly. Combine with refined honey to form pills the size of sophora seeds. Before meals, take 1 pill with millet broth.

Regarding cold damage disease manifesting as spitting blood, sores like millet grains inside the upper lip, and painful oppression in the heart: this indicates worms eroding the five zang organs from above. If sores appear inside the lower lip:
Method 1: Take one chicken egg, crack a small bean, extract the white, and mix with half he of cooked lacquer. Stir well. Swallow on an empty stomach. After a meal's duration or half a day, worms may be excreted or vomited. Once the worms are cleared, the fever subsides and the disease heals. If untreated, it can be fatal.
Method 2:
Ingredients: Zhu Dan (Pig Gallbladder, 2 pieces, juice extracted).
Administration: Mix with one zhong of vinegar, add the bile, boil for three to five boils, let cool to warm temperature. Take one he on an empty stomach; the worms will die naturally.
