Generally speaking, xiao-ke (wasting-thirst syndrome) accompanied by irritability is caused by deficiency of kidney qi and extreme heat in the heart. The kidney governs water, while the heart governs fire. If kidney water is exhausted, it cannot control heart fire; heart fire flares upward and disturbs the mind, leading to stagnation of heart qi and generation of internal heat. These conditions all result from long-term deficiency in the lower jiao (lower burner), where deficiency generates heat. Accumulated heat, if untreated, leads to wasting-thirst, fever, mental restlessness, and insufficient drinking desire.
