When a heat disease reaches the seventh day, the transmission through the three Yin and three Yang meridians is complete. According to pathological laws, the illness should resolve at this stage. If the pathogen remains uneliminated, it indicates that the disease has entered the "second cycle" (Zai Jing Bing). A "second cycle disease" refers to the meridians receiving pathogenic invasion again.
For heat disease on the seventh day presenting with irritability, thirst, and phlegm-heat in the chest, Banxia San (Pinellia Powder) is indicated.
Banxia (Pinelliae Rhizoma, 0.5 liang, washed seven times with decoction to remove slipperiness), Chi Shaoyao (Paeoniae Radix Rubra, 1 liang), Qianhu (Peucedani Radix, 0.5 liang, radix removed), Huangqin (Scutellariae Radix, 0.5 liang), Renshen (Ginseng Radix et Rhizoma, 1 liang, radix removed), Fuling (Poria, 0.5 liang).
The above ingredients are crushed, sifted, and made into a powder. Each dose consists of 5 qian, boiled in one da zhan of water with 100 grains of Jingmi (Oryza Sativa var. Glutinosa Semen) and Xiaomai (Tritici Fructus), plus 0.5 fen of Shengjiang (Zingiberis Rhizoma Recens). Decoct until one-fifth remains, filter dregs, and take warm without regard to time.
For heat disease unresolved after seven days, presenting with alternating chills and fever, fullness and discomfort in the chest and hypochondrium, anorexia, mental restlessness, and nausea, Qianhu San (Peucedani Powder) is indicated.
Qianhu (Peucedani Radix, 1 liang, radix removed), Banxia (Pinelliae Rhizoma, 0.5 liang, washed seven times with decoction to remove slipperiness), Huangqin (Scutellariae Radix, 3 fen), Renshen (Ginseng Radix et Rhizoma, 3 fen, radix removed), Chi Shaoyao (Paeoniae Radix Rubra, 3 fen), Guixin (Cinnamomi Cortex, 3 fen), Gancao (Glycyrrhizae Radix et Rhizoma, 0.5 liang, honey-fried until slightly red and chopped).
The above ingredients are crushed, sifted, and made into a powder. Each dose consists of 5 qian, boiled in one da zhan of water with 0.5 fen of Shengjiang (Zingiberis Rhizoma Recens) and three Zao (Jujubae Fructus). Decoct until one-fifth remains, filter dregs, and take warm without regard to time.
For heat disease on the seventh day presenting with generalized body pain and persistent high fever, Digupi San (Cortex Lycii Powder) is indicated.
Digupi (Lycii Cortex, 1 liang), Zhike (Aurantii Fructus, 1 liang, stir-bran fried until slightly yellow and pith removed), Chuan Dahuang (Rhei Radix et Rhizoma, 1 liang, chopped and lightly stir-fried), Chi Shaoyao (Paeoniae Radix Rubra, 0.5 liang), Chaihu (Bupleuri Radix, 0.5 liang, fried until slightly red and chopped).
The above ingredients are coarsely crushed, sifted through a coarse sieve, and made into a powder. Each dose consists of 5 qian, boiled in one da zhan of water. Decoct until one-fifth remains, filter dregs, and take warm without regard to time.
For heat disease on the seventh day presenting with irregular alternation of cold and heat, deficiency-heat in the abdomen, retention of toxic qi, and mental unrest, Sheng Dihuang Jian (Fresh Rehmannia Decoction) is indicated.
Sheng Dihuang Zhi (Juice of Rehmanniae Radix Recens, 0.5 sheng), Sheng Maidong (Radix Ophiopogonis Recens, 0.5 sheng), Mi (Honey, 0.5 sheng), Renshen (Ginseng Radix et Rhizoma, 1 liang, radix removed), Baizhu (Atractylodis Macrocephalae Rhizoma, 1 liang), Guixin (Cinnamomi Cortex, 1 liang), Caixin (Vegetable Heart, chopped).
All ingredients are chopped. Boil in three da wan of water until one da wan remains, strain to remove dregs. Add Mi (Honey), Sheng Dihuang Zhi (Juice of Rehmanniae Radix Recens), and Sheng Maidong (Radix Ophiopogonis Recens). Simmer together until the desired consistency is achieved. Allow to cool. Take 0.5 he without regard to time. The therapeutic goal is to induce sweating.
For heat disease on the seventh day presenting with limb pain, persistent heat toxicity, alternating chills and fever, fluctuating severity of symptoms, and eruptions resembling sores, Biejia San (Trionycis Carapax Powder) is indicated.
Biejia (Trionycis Carapax, 2 liang, coated with vinegar and fried until yellow, skirt removed), Tianmendong (Radix Asparagi, 1 liang, heart removed), Renshen (Ginseng Radix et Rhizoma, 1 liang, radix removed), Shigao (Gypsum Fibrosum, 2 liang), Huangqi (Astragali Radix, 1 liang, chopped), Wumei Rou (Semen Pruni Mume, 1 liang, lightly stir-fried), Hengshan (Radix Dichroae, 1 liang), Niuxi (Achyranthis Bidentatae Radix, 1 liang, sprouts removed), Gancao (Glycyrrhizae Radix et Rhizoma, 1 liang, honey-fried until slightly red and chopped).
The above ingredients are crushed, sifted, and made into a powder. Each dose consists of 5 qian, boiled in one da zhan of water with more than thirty leaves of Zhuye (Phyllostachys Bambusoides Folium) and 50 grains of Chi Dou (Sojae Semen Praeparatum). Decoct until one-fifth remains, filter dregs, and take warm without regard to time.
