Ophthalmia with redness and ulceration is caused by wind-heat pathogens. Initially presenting as red eyes, if the condition persists without healing, it is externally due to injury from wind-cold pathogens and internally due to accumulated heat in the Lung and Spleen.
For liver wind-heat causing ophthalmia with redness, ulceration, pain, and swelling, Longnao San (Dragon Brain Powder) is indicated.
Borneolum Syntheticum (1 fen, finely ground) Calculus Bovis (1 fen, finely ground) Cinnabaris (1 fen, finely ground) Tianshuhuang (Half liang, finely ground) Radix Paeoniae Rubra (Half liang) Semen Plantaginis (Half liang) Radix Scutellariae (1 liang, sliced, slightly stir-fried)
Crush the above herbs, pass them through a fine sieve to make a powder, and grind thoroughly to mix evenly. After meals, decoct Bamboo Leaves in water, take one qian of the powder mixed with this decoction. Avoid roasted, fried, hot-natured wheat products, and toxic or slippery fish and meat.
For sudden onset of heat rushing upward into the eyes, causing pain, redness, swelling, and formation of nebula, Xianlingpi Wan (Epimedium Pill) is indicated.
Radix Epimedii (2 liang) Flos Chrysanthemi Indici Radix Scutellariae Semen Plantaginis Gypsum Fibrosum (finely ground, washed with water for sedimentation) Radix Scrophulariae Fructus Cassiae (Cornu Saigae Tataricae Slice)
Crush the above herbs, pass them through a sieve to make a powder. Mix with refined honey and pound 300-200 times. Form pills the size of Sophora seeds. After meals, take 20 pills with warm water.
For liver and lung qi stagnation with heat causing circumcorneal redness, ulceration, pain, swelling, or nebula formation:
Rhizoma Picrorhizae Radix Glycyrrhizae Preparata (stir-fried until slightly red, sliced) Rhizoma Coptidis (remove stamens) Cortex Phellodendri Amurensis (sliced, each half liang) Mel (2 liang) Junci Medulla (2 stems) Folium Bambusae (light green variety)
Except for Borneolum Syntheticum and Mel, crush and sieve the remaining herbs into a powder. Use three large bowls of snow water, add Mel, and decoct in a silver pan until reduced to one medium bowl. Filter out the dregs.
For chronic ophthalmia that does not heal, characterized by redness, ulceration, intermittent itching, pain, swelling, and blurred vision:
Minium (1 fen, finely ground) Natrii Sulfas (Half liang, finely ground) Mel (5 liang)
In a copper vessel, boil slowly over low fire for more than ten boils, stirring occasionally. Filter through new cotton wool. Allow to cool before instilling into the eyes.
Another prescription.
Rhizoma Coptidis (1 liang, remove stamens, crush into powder) Ancient Cash Coins (5 coins) Mel (3 liang)
Place the herbs in a bamboo tube and steam over a rice steamer for the duration of cooking a meal. Filter through cotton wool to obtain the juice for instillation.
Another prescription.
Rhizoma Coptidis (Half liang, remove stamens) Cortex Phellodendri Amurensis (Half liang, sliced) Ancient Cash Coins (7 coins) Fructus Pruni Persicae Seed (37 seeds, soaked in hot water, remove coarse skin, finely ground)

Finely slice the herbs. Decoct three large bowls of water until reduced to one medium bowl. Filter through cotton wool to remove dregs. Instill 3-4 times daily for efficacy.
For epiphora due to wind, redness, ulceration, pain, and long-term formation of cataracts/nepha:
Lapis Aluminis (Arsenical Alum) Realgar Lapis Aragonitae (Crush finely, wash with water for sedimentation) Sal Marinum (Blue variety) Borneolum Syntheticum (finely ground) Fructus Cassiae Radix Zingiberis Preparata (Pao, sliced, each 1 fen)
Crush and sieve the herbs into a powder. Add Borneolum Syntheticum and grind evenly. Mix with refined honey to form pills the size of hemp seeds. Take one pill, dissolve in a small amount of warm water, and instill.
