Zhuanmai (Twisted Pulse Disease) arises from excessive drinking leading to drunkenness, restless sleep at night, frequent fright and nausea, and improper pillow positioning during turning over. At the onset, the pathology manifests as a twisting of the pulses in the neck and nape, with the body feeling as if trembling, causing alternating chills and fever; its root lies in the Small Intestine.
For treating Zhuanmai manifesting in the neck and nape, accompanied by chills, fever, and suppuration, Lianqiao San (Forsythia Powder) is indicated.
Lianqiao (Forsythiae Fructus, 1 liang 5 qian) Xuan Shen (Scrophulariae Radix, 1 liang 5 qian) Mu Xiang (Aucklandiae Radix, 1 liang 5 qian) Chuan Shengma (Cimicifugae Rhizoma, 1 liang) Zhi Ke (Aurantii Fructus, 1 liang 5 qian, stir-fried with bran until slightly yellow, pith removed) Kunbu (Sargassum Thallus, 1 liang 5 qian, washed to remove saltiness) Chuan Da Huang (Rhei Radix et Rhizoma, 2 liang, cut into pieces and slightly stir-fried) Dama Ren (Cannabis Semen, 2 liang)
The above herbs are finely sifted to make a powder. Each day, on an empty stomach, take 1 qian mixed with rice water.
For treating Zhuanmai manifesting in the neck with swelling and pain, accompanied by chills, fever, and continuous discharge of pus, Banmao Wan (Mylabris Pill) is indicated.
Banmao (Mylabris, 1 fen, stir-fried with glutinous rice until the rice turns yellow, head, feet, and wings removed) Renshen (Ginseng Radix, 3 qian, radix removed) Di Dan (Euphorbiae Pekinensis Radix, 1 fen, stir-fried with glutinous rice until the rice turns yellow, head, feet, and wings removed) Dang Gui (Angelicae Sinensis Radix, 3 qian) Chuan Shengma (Cimicifugae Rhizoma, 3 qian) Maidong (Ophiopogonis Radix, 1 liang, heart removed and dried) Baizhu (Atractylodis Macrocephalae Rhizoma, 3 qian) Guixin (Cinnamomi Ramulus, 3 qian) Xuduan (Dipsaci Radix, 3 qian)
The above herbs are pounded and sifted into powder. The ground medicines are mixed uniformly, combined with refined honey, and pounded three or two hundred times. Form pills the size of Sophora seeds. Each dose consists of ten pills taken with warm wine, administered three times daily.
For treating Zhuanmai, Loulu Gao (Radix Isatidis Ointment) is indicated.
Loulu (Radix Isatidis, 2 jin) Ben (Ligustici Rhizoma et Radix, 2 jin) Bai Ma Fen (Equi Stercus, 0.5 jin) Niu Fen (Bovis Stercus, 0.5 jin) Zhu Fen (Porci Stercus, 0.5 jin) Ji Fen (Galli Stercus, 0.5 jin) Yang Fen (Caprae Stercus, 0.5 jin)
