Severe epistaxis (severe nosebleed) is caused by deficiency of blood and qi with heat pathogen. The Heart stores blood, the Lung governs qi, and opens into the nose. Blood and qi move together, circulating within the meridians.
For severe epistaxis that does not stop, it is appropriate to take Yuanzhi San (Polygala Powder).
Herbs: Polygalae Radix (Yuanzhi, 远志) (half liang, remove heart), Paeoniae Radix Alba (Baishao Yao, 白芍药) (three fen), Cinnamomi Cortex (Guixin, 桂心) (one fen), Asparagi Radix (Tianmendong, 天门冬) (half liang, remove heart), Ophiopogonis Radix (Maimendong, 麦门冬) (half liang, remove heart), Corii Fructus (Ejiao, 阿胶 - note: text implies Ejiao but lists Corii or similar contextually, usually Ejiao for bleeding, here translated as processed animal glue based on "micro-red and crushed"), Rheum Palmatum (Chuan Dahuang, 川大黄) (two kernels, bran-fried until slightly yellow).
The above herbs are pounded, sifted into a powder. Each dose is three qian, decocted in one zhong of water until five fen remains, filtered, and taken warm at any time.
For severe epistaxis where blood flows continuously from the mouth and ears, Kushen San (Sophora Powder).
Herbs: Sophorae Flavescentis Radix (Kushen, 苦参) (one liang), Coptidis Rhizoma (Huanglian, 黄连) (one liang, remove mustache), Rheum Palmatum (Chuan Dahuang, 川大黄) (half liang, crushed and slightly fried), Gardeniae Fructus (Zhizi Ren, 栀子仁) (half liang), Platycladi Cacumen (Baiye, 柏叶) (half liang), Auricularia (Sanger, 桑耳).
The above herbs are pounded, sifted into a powder. Each dose is three qian, decocted in one zhong of water until five fen remains, filtered, then mixed with fresh Rehmanniae Juice (Sheng Dihuang Zhi, 生地黄汁) (one he) and stirred evenly.
For severe epistaxis that has not stopped, with blood loss of several sheng and unconsciousness, it is appropriate to take Ejiao San (Donkey-hide Gelatin Powder).
Herbs: Corii Fructus/Ejiao (Ejiao, 阿胶) (three fen, crushed and fried until yellow and dry), Cinnamomi Cortex (Guixin, 桂心) (half liang), Asari Radix (Xixin, 细辛) (half liang), Os Draconis (Bailonggu, 白龙骨) (half liang), Angelicae Sinensis Radix (Danggui, 当归) (half liang), Hair tuft (Luankoufa, 乱发 - burned to ash).

The above herbs are decocted together in one zhong of prepared Soja Semen Praeparatum juice (Chi Zhi, 豉汁) until seven fen remains, filtered, and divided into two warm doses taken at any time.
