According to various treatises on postpartum care: For a woman entering the first month after childbirth (the confinement period), alcohol must be strictly avoided, lest it cause mental confusion during delivery. At the time of labor, one should not panic or act hastily, to avoid startling the mother and disturbing the fetus. Only two or three close relatives who are familiar with midwifery, steady, cautious, and mature should assist her. The mother should sit or lie down as she pleases and eat whatever she can manage, to prevent exhaustion and emaciation at the time of delivery. If abdominal pain gradually intensifies, she should endure it patiently and take one or two doses of Huatai Yubai Pi San (Slippery Fetus Elm Bark Powder). The method of taking medicine is crucial: do not administer it too early; wait for the appropriate timing. Furthermore, attendants must not support the woman to walk slowly. If walking becomes slightly difficult, she should lean against an object to stand, and after a moment, resume walking with assistance, until the pain radiates to the waist. At that point, immediately take one dose of Cuisheng Dan (Labor-Inducing Pill), and continue to walk with assistance. When the contractions become severe and standing is difficult, recognizing that delivery is imminent, assume the "Anxiang Shangcao" position (a specific posture for preparation for birth). When assuming this position, do not do so too early; if done prematurely, the fetus will have difficulty turning within the womb. One must carefully observe whether birth is truly imminent before allowing someone to hold her waist. The person holding the waist must remain upright and not tilt, so that the baby follows the natural physiological path and delivers easily. Once lying down, keep the back flat against the mat and the body uncurled, so the fetus does not deviate from its path. Failure to adhere to these proper labor protocols will inevitably lead to dystocia and various complications.
According to classical texts: A woman’s concern is not limited to labor; postpartum care requires meticulous management. Do not neglect care simply because no immediate complications occurred during delivery. Negligence, though seemingly minor at first like the tip of an autumn hair, can result in illnesses as heavy as Mount Song or Mount Tai. Immediately after delivery, refrain from asking about the gender of the infant. First, check for symptoms of blood syncope (yunchun) and administer Ya Xueyun Yao (Blood-Suppressing Syncope Medicine). After some time, consume congee, then take Sishun Lizhong Wan (Four-Oriented Middle-Warming Pill). Subsequently, have someone massage from the epigastrium down to the umbilical and abdominal region, five to seven times daily. Then, administer medicine according to the syndrome, alternating between congee and medicine, taken frequently. The patient should sit with eyes closed, leaning against an object, with attendants on both sides for support. Even if sleepy, wake her frequently; only after one shichen (two hours) should she go to bed, and even then, she should sleep with knees flexed and the head of the bed elevated. Use thick bedding and seal gaps to ward off "thief wind" (pathogenic external factors). Within the first lunar month of postpartum, frequently expose her to vinegar smoke to prevent dizziness and faintness. After two months, gradually introduce flavorful foods, such as congee made from rich chicken broth, continuing until the hundredth day. Regularly take Danggui Wan (Angelica Sinensis Pill), Danggui Jianzhong Tang (Angelica Twig-Decoction to Build the Middle), and Sishun Lizhong Wan, once daily each, to nourish the zang organs and tonify the blood vessels. After three months, soft-cooked meat may be consumed. Within the full month postpartum, one must especially avoid arbitrary diet, emotional extremes (anger or joy), forceful hair combing, loud speaking, physical labor akin to craftsmanship, and defecation/urination in toilets. By adhering to such regimen and care, leading up to the hundred-day mark, the organs and bowels will recover balance. Failure to follow this will result in residual postpartum diseases.
