In cases of heat-induced vomiting and diarrhea, drying agents should not be administered. In Shaoyang disease (Taiyang cold damage) accompanied by fever, cooling agents should not be immediately prescribed. When eruptive diseases (chuang zhen) present with convulsions, they should not be treated as fright disorders (jing feng). These are all instances where the external manifestations are similar but the internal pathogenesis differs. The *Suwen* states: "Irritability, pain, and itching arising from sores and ulcers all originate from the Heart." Thus, eruptive diseases stem from the Heart, and convulsions also stem from the Heart.
When the toxic factors of eruptive diseases first arise, excessive Heart Fire generates Wind; generating Wind leads to palpitations and fright, and in severe cases, convulsions and spasms. Vigorous steaming of heat causes irritability, manifesting as eruptive lesions accompanied by pain and itching. Therefore, among the syndromes of convulsions, irritability, pain/itching, and eruptions, those governed by the Heart must be present without omission. If initially, toxic factors accumulate in the Heart, causing palpitations and convulsions, and the physician fails to distinguish between interior and exterior signs or does not prepare for eruptive diseases, but instead administers cold-natured drugs such as Calx Argenti (Silver Powder), Borneolum Syntheticum (Bingpian), Moschus (Shexiang), Indigo Naturalis (Qingdai), Cinnabar (Zhusha), and Natrii Sulfas (Xiaoshi), the Heart becomes cold and the muscle layer contracts. Consequently, the toxic factors remain trapped inside, preventing the eruptive diseases from emerging through the skin despite the convulsions. This condition resembles Fright Wind (Jing Feng); medication should be administered with precautions for eruptive diseases. It is common for early-stage eruptive diseases to resemble Fright Wind; if physicians fail to examine carefully and treat them as Fright Wind, the harm inflicted is profound.
Generally, eruptive diseases are always mixed with other symptoms. Initially, there may be coughing, phlegm, mental palpitations, irritability, vomiting, red lips, flushed cheeks, thirst, cold ears, cold feet, rapid pulse, and white tongue coating. If these several signs are present, cold-natured Heart-cooling drugs like Silver Powder must absolutely not be used. Once the Heart becomes cold and the muscle layer contracts, how can the trapped pathogens be expelled? Furthermore, children who are usually healthy and free of disease but suddenly develop convulsions likely have eruptive diseases; this requires particularly careful examination. Since the toxic factors are abundant internally, one should use formulas that disperse toxic factors, such as Xingxing San (Stimulating Powder), Xiaodu San (Detoxifying Powder), Shengma Tang (Cimicifuga Decoction), and Hongmian San (Red Cotton Powder), combined with herbs that promote Qi circulation, promote urination, and dispel Wind. Although these remedies may not yield immediate results, they will not ruin the treatment outcome. As the progression unfolds, once the heat is vented through the skin, the Heart Qi will naturally settle.
If convulsions arise from the struggle between Cold-Wind and Interior Heat, each syndrome should be treated according to its specific presentation, following the previous methods but ideally adding herbs that harmonize Qi. Zhang Shi stated: "When the Qi of eruptive diseases is harmonized, they will emerge; when Qi moves freely, Ying and Wei (Nutritive and Defensive Qi) flow without stagnation." Even if Cold is present, it will be dispersed, and the eruptive diseases will naturally emerge.
