Treats severe stroke (Zhong Feng), characterized by flaccidity or spasticity of the body, inability to open the eyes, and a stiff tongue with aphasia.
Shaoyao (Paeoniae Radix), Zhigancao (Prepared Glycyrrhizae Radix et Rhizoma) [each 1 liang], Mahuang (Ephedrae Herba, removed roots and joints), Baizhu (Atractylodis Macrocephalae Rhizoma) [each 1 liang], Fangfeng (Saposhnikoviae Radix, removed bristles), Fangji (Stephaniae Tetrandrae Radix) [each 1.5 liang], Fuzi (Aconiti Lateralis Radix Praeparata) [each 1 liang], Tianxiong (Aconiti Radix) [each 1 liang].
The above herbs are chopped like hemp seeds. Each dose is 5 qianbi (approx. 2-3 grams), decocted in 1.5 sheng (approx. 300 ml) of water with 0.5 fen (approx. 1.5 grams) of sliced fresh ginger. Decoct until reduced to 0.8 sheng, strain, and take warm on an empty stomach at midnight and wu hour (noon/midnight depending on context, here likely referring to night hours). After walking for about five li (approx. 2.5 km), consume hot ginger porridge to assist the medicinal effect. Aim for slight sweating; avoid exposure to external wind.
Treats stroke causing coma and obstruction, loss of motor control of limbs, and deviation of the mouth and eyes.
Muxiang (Aucklandiae Radix), BaiFuling (Poria with Hostwood, peeled), Chuanxiong (Ligusticum Chuanxiong Rhizoma), Jili (Tribuli Fructus, stir-fried, removed spines), Renshen (Ginseng Radix et Rhizoma), Duhuo (Angelicae Pubescentis Radix, removed rhizome), Huangqi (Astragali Radix, chopped, 1 liang), Baizi Ren (Platycladi Semen, 2 liang).
The above herbs are ground into fine powder. Each dose is 1 qianbi, taken with warm wine. The dose may be increased to 2 or 3 qianbi. Take on an empty stomach at midnight and before sleep at wu hour.
Treats stroke accompanied by sweating but without fever.
Guizhi (Cinnamomi Ramulus), Fuzi (Aconiti Lateralis Radix Praeparata), Gancao (Glycyrrhizae Radix et Rhizoma).
Double the dosage of this formula. Acupoint Taixi (KI3) should be needled. This syndrome is classified as stroke affecting the Shaoyang meridian. Decoct in water and take orally.
Treats aphagia and inability to speak, caused by wind pathogen lodging in the Spleen meridian and ascending to enter the "guan ji" (mechanism, i.e., root of the tongue). Due to blockage of the guan ge (diaphragm/upper burner), the person suffers from essence depletion and forgetfulness.
Bai Jiangcan (Bombyx Batryticatus, stir-fried, 0.5 liang), Qianghuo (Notopteryri Rhizoma et Radix, 3 fen), Shexiang (Moschus, 0.5 qian).
The above herbs are ground into fine powder. Each dose is 2 qian, first mixed evenly with a small amount of fresh ginger juice, then soaked in boiling water, allowed to cool to a warm temperature, and taken. Simultaneously, apply real Changpu (Acori Tatarinowii Rhizoma) powder frequently under the root of the tongue.
Use 7 pieces of Bai Jiangcan, ground into powder, taken with wine in a measure of 1 cunbi (approx. 2-3 grams), resulting in immediate efficacy.
Decoct Huangqi (Astragali Radix) and Fangfeng (Saposhnikoviae Radix) in several hu (large volume) of water, place under the bed for fumigation.
In ancient times, Xu Yizong of the Tang Dynasty served Chen Dynasty as Military Adjutant to Prince Xincai. At that time, Empress Dowager Liu suffered from wind-stroke and could not speak; her pulse was deep and her jaw was clenched. Yizong said: "Since oral medication cannot be administered, it is appropriate to use steam fumigation. The medicinal qi will penetrate the cou li (skin and interstices), and recovery can be expected within one shichen (2 hours)." Thus, he prepared several hu of this decoction, placed it under the bed, creating a smoky vapor. That evening, she regained the ability to speak. The efficacy of such medicinal fumigation is thus demonstrated.
